Proximal tibial osteotomy in the growing patient historically has been associated with a high rate of neurovascular complications and malunion. Here is reported a technique of valgus proximal tibial osteotomy, of the incomplete closing wedge type, that has avoided neurovascular compromise while achieving reliable correction and rapid bony union. Oblique proximal tibial valgus osteotomies with lateral tension plate fixation were performed on 18 tibiae in 14 patients (age range, 5-25 years) with tibia vara. The primary diagnosis was Blount's disease in 13 tibiae, achondroplasia in 2 tibiae, mult

The treatment of limb length inequality in pediatric patients is a complicated and long-term procedure, and careful evaluation is necessary over several visits. A team approach with an involved family is essential, as is knowledge and familiarity with the various conservative and surgical options. Treatment must not be rushed, and it is important to realize that not everyone needs a lengthening. Soft tissue and osseous complications are significant in these procedures, so shoe lifts and shortening procedures are attractive alternatives in smaller discrepancies. This condition is in a growing c

We report two siblings with bowed tibia, hypoplastic thumbs, multiple fractures, a distinctive facial phenotype and developmental delay. These children share some features with other cases reported in the literature but we consider that they represent a new syndrome.

Familial metaphyseal dysplasia (FMD) is a rare genetical conditional characterised by symmetrical metaphyseal splaying of the tubular bones. The diagnosis of FMD was made in four patients from two families in Japan; abnormal modelling defects were observed in the flat bones as well as tubular bones in some of the patients. In addition, quantitative measurement of the mineral content of the bone was carried out and was found to be one-quarter of the normal value.

To establish the pathogenicity, and attempt to define a classification of craniofacial malformations is a difficult task, both in animals and humans. Mention is often made of the classification developed by Paul Tessier, which according to this author "is neither based on a theory nor on an embryonic definition. It is the by-product of anatomical findings during clinical examination and operative dissections" (31). The authors propose an embryologic classification of facial malformations, based on disorders of embryonic development of cells derived from the neural crests (NC) of cephalic origi

A 12-year-old by with unusual face and hearing loss was diagnosed as having frontometaphyseal dysplasia. He also had congenital urinary tract malformations and chronic urinary tract infection. The cranial and facial bones of the patient were studied by roentgen cephalometric analysis.

The whistling face syndrome (Freeman-Sheldon Syndrome) is a rare disorder characterized by typical face and limb abnormalities. A wide range of clinical findings have been reported. We report unusual skin dimples in a sporadic case with the whistling face syndrome.

Osteopathia striata with cranial sclerosis (OS-CS) is a bone dysplasia characterized by hypertelorism, macrocephaly, frontal bossing, broad nasal bridge, palate anomalies, hearing deficits, and mental retardation. The radiographic findings include cranial sclerosis, linear striations in the long bones and iliac wings, small poorly aerated sinuses, scoliosis, and increased bone density. The sensory deficits are disabling, but the condition generally is not life threatening. We describe 4 brothers with the characteristics of OS-CS, 3 of whom have died from more serious complications of the disor

The point prevalence at birth of generalised bone dysplasias was estimated by a study of all children born between 1 January 1970 and 31 December 1983 in the county of Fyn (Denmark). Additionally the population prevalence on 31 December 1983 of all patients with generalised bone dysplasias in this county was determined. The county is a well defined, representative subregion of Denmark which demographically comprises a cross section of about 9% of the Danish population. In total, bone dysplasias were found to be more frequent than generally assumed. Achondroplasia was, however, less common with

Many of the human skeletal dysplasias are accompanied by significant alterations in craniofacial growth. Several specific disorders have been discussed which show the effects of endochondral and membranous defects in the craniofacial complex. The craniofacial and other skeletal abnormalities observed in the skeletal dysplasias may indicate the sites and timing of the primary defects and provide direction to investigative efforts. In addition, a careful review of known pathogenetic and histomorphologic mechanisms in the skeletal dysplasias can provide valuable clues to the complex interactions

Two cases of multiple fractures, one in a newborn with an unclassified bone dysplasia, another in an infant with recurrent fits of unknown etiology are reported. The second case is the youngest in the literature with bilateral slipped capital femoral epiphyses and bilateral femoral neck fractures, secondary to stress injury.

Through 17 years ten patients were treated for psychologically unacceptable tallness by bilateral femoral shortening. All were young adults between 17 and 28 years; one was acromegalic, one had Marfans syndrome and the other were tall for unknown reasons. The amount of shortening obtained was between 6 and 16 cm. One patient was reoperated bilaterally because of insufficient stability of the pin and plate fixation. All patients healed without significant complications. For exclusive cases, which have been carefully observed over several months, bilateral shortening seems to be a safe procedure

Clinical and radiographic features of seven cases of cranio-carpo-tarsal dysplasia are presented. Radiographs show bilateral dislocation of the hips, bilateral vertical talus deformity hand deformities, and characteristic skull changes.

Frontometaphyseal dysplasia is a rare genetic syndrome affecting the skeletal system and connective tissue. It is believed to be inherited as an X-linked trait. Features of frontometaphyseal dysplasia overlap with other skeletal dysplasias. Prominent supraorbital ridges, radiologic evidence of cranial hyperostosis, and flared metaphyses are characteristic. Scoliosis, a rare associated finding, is usually mild, and familial progressive scoliosis has not been reported so far. The skeletal dysplasia and the associated clinical findings show significant intra- and interfamilial variability. The sy

INTRODUCTION: Bone lesions are common at puberty. Most of them are detected by chance at examination for other reasons. The nonossifying fibroma does not produce any clinical symptoms and normally is diagnosed by chance. A spontaneous healing is discussed. The osteoid osteoma is characterized by its nocturnal pain and relief of pain at therapy with salicylates. Operative procedures are indicated for healing.
CASE: A 12-year-old girl presented with radiological signs of a nonossifying fibroma. Because of local radionuclid uptake and clinical symptoms an operative procedure was carried out. Pos

A family with characteristics that have striking similarities to the syndrome of familial osteodysplasia is reported. The inheritance pattern appears to be autosomal dominant. The typical facies has a lack of vertical facial development and a functional prognathism caused by overclosure of the mandible and opening of the gonial angle. Midfacial hypoplasia and a pointed chin are characteristic. Alveolar and sutural bone growth in the face is diminished and associated with partial dental agenesis. Variable cranial digital markings and possible synostosis are also seen. These typical facial chang
