PURPOSE OF THE STUDY: Congenital femoral deficiency (CFD) is a rare and complex deformity. The results and complications of surgical correction of this deformity using ring fixators (Ilizarov/Taylor Spatial Frame) have been evaluated.
MATERIAL AND METHODS: We retrospectively reviewed a consecutive series of 30 patients with CFD, between the ages of 3.3 and 17 years (mean, 9.3 years), with 35 lengthening procedures. Ten cases were treated with an Ilizarov frame, 25 cases with a Taylor Spatial Frame (TSF).

The authors have studied 15 cases of unilateral poliomyelitis and have compared the growth of the intact lower limb with that of the paralysed limb. It is concluded that no accurate prediction can be made except by multiple measurements. In most of the cases, growth was steady, the rate of shortening increasing by regular degrees. However, in very young children the degree of shortening was more consistent and was similar to that in congenital abnormalities.

Brachymetatarsia is characterized by the parabola formed through the articulation of the metatarsophalangeal joints of the toes. It is defined as the termination of one or more metatarsals at least 5 mm proximal to this parabola, most commonly affecting the fourth metatarsal. Surgical interventions for brachymetatarsia include osteotomy followed by either acute distraction with grafting or gradual lengthening via distraction osteogenesis. In limb-lengthening procedures involving long bones of the lower extremity, an external fixator in conjunction with an intramedullary nail is commonly used.

In congenital malformation of the limbs the shortening of the extremity appears frequently together with its deformity. With Ilizarov's apparatus the limb may be lengthened simultaneously with the correction of the deformity. In spite of the fact that the use of the apparatus is rather complicated and complications are frequent, in 75 cases out of 102 excellent, in 23 good results were achieved. Axial forces across the growth plate of the upper tibia during controlled distraction were studied in 24 rabbits close to skeletal maturity.
