Nails are highly keratinized skin appendages that exhibit continuous growth under physiological conditions and full regeneration upon removal. These mini-organs are maintained by two autonomous populations of skin stem cells. The fast-cycling, highly proliferative stem cells of the nail matrix (nail stem cells (NSCs)) predominantly replenish the nail plate. Furthermore, the slow-cycling population of the nail proximal fold (nail proximal fold stem cells (NPFSCs)) displays bifunctional properties by contributing to the peri-nail epidermis under the normal homeostasis and the nail structure upon

Exostosis is a type of benign bone tumor in which trabecular (spongy) bone overgrows its normal border in a nodular pattern. When the growth occurs under the nail bed, it is termed subungual exostosis or Dupuytren exostosis. This condition may mimic other bony abnormalities such as an osteochondroma and may present with nail deformities with or without pain. For this reason, a biopsy of the lesion is necessary to rule out a precancerous growth. In rare cases, pediatric patients may have subungual exostosis, as demonstrated in our case.

A ten year old girl suffered from anonychia of all toes, present from birth. The absence of nails was accompanied by absence of one or two phalangeal bones in toes other than the big toes. Other abnormalities consisted of short toes, incomplete syndactyly of the 2nd and 3rd toes of each foot and transfer of the dermatoglyphics from the plantar to the dorsal surface of the digits. The combined defect of the nails and the phalangeal bones can be explained by the close relationship of the two structures during the morphogenesis of the digits.

A rare case of osteomyelitis of the tibia caused by Mycobacterium fortuitum which developed after closed intramedullary Küntscher nail fixation is reported. After extraction of the nail, combination therapy with antituberculous drugs, ofloxacin, and interleukin 2 was undertaken and bony union was achieved. Since a bone cyst formed during union, curettage and conventional cancellous bone grafting were performed. The patient is now pain-free even with prolonged weight-bearing. The success of treatment in this case is in contrast to other reports of the disease.

The authors report on a patient with hereditary osteo-onychodysplasia (HOOD). Osteoporosis was demonstrated upon histomorphologic bone examination. HOOD is a rare inherited disorder. Typical features include dysplasia of the nails, patellae, elbows and ilium. Nephropathy occurs in 48% of patients. Numerous renal histologic studies have led to the concept that the nail-patella syndrome is an inherited metabolic disorder affecting the connective tissue. Our findings from histomorphologic bone assessment bring further evidence in support of this hypothesis.

Bone transportation technology (Ilizarov technique) effectively solves the clinical problem of chronic osteomyelitis with structural bone defect. The paper combined with own clinical experience and a large number of literatures, the results showed that this method had some complications related to severe bone healing, such as non union of the joint ends and poor mineralization of the extended gap. Maintenance of force line during operation, protection of osteotomy end tissues, appropriate transporting velocity and stress stimulation after operation, and application of "accordion technology" ar

BACKGROUND: Delayed unions are quite common in the treatment of atypical femur fractures, which are thought to result from the long-term use of bisphosphonates. The effects of interventions for sclerotic tissue on the fracture line in atypical femoral fractures are not fully known. For this reason, we compared the results of patients with atypical femoral fractures treated by closed intramedullary nailing to patients treated by open surgery accompanied with interventions for their sclerotic bone ends, aiming to answer: (1) In the treatment of atypical femoral fractures, do bone-end interventio

BACKGROUND: Two types of implants used for the surgical fixation of extracapsular hip fractures are cephalocondylic intramedullary nails, which are inserted into the femoral canal proximally to distally across the fracture, and extramedullary implants (e.g. the sliding hip screw).
OBJECTIVES: To compare cephalocondylic intramedullary nails with extramedullary implants for extracapsular hip fractures in adults.
SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (April 2010), The Cochrane Central Register of Controlled Trials (The Cochrane Librar

The aim of this publication is to present a case report of a 38-year-old patient with traumatic astragalectomy and resultant foot reconstruction surgery using a combination of talus compensation by vertically placed tricortical autograft and pantalar arthrodesis with a retrograde calcaneotibial nail (hindfoot nail). The advantage of this treatment is based on a solid, stable osteosynthesis, while maintaining the length of the limb. Key words: traumatic talar extrusion, tibiocalcaneal arthrodesis, hindfoot nail, bone graft, pantalar arthrodesis.

Revision surgery of the proximal femur with bone loss secondary to failed cephalomedullary nails is problematic and becoming more prevalent as their use grows. This article presents a technique of deformity correction, bone graft techniques that reconstitute residual defects, and definitive fixation using load-sharing devices that provide immediate stability for bone healing and early rehabilitation. Preoperative planning and the potential advantages and disadvantages of newer fixed-angled plates versus established implants are discussed. With proper planning, surgical execution with proved te

Prophylactic intramedullary nailing was performed in eighteen femora of fourteen patients who had neoplastic lesions in the femur and survived for from thirty-eight to 1309 days after nailing. The procedure enabled all but three patients to be ambulatory with a walker by four days after operation and to continue to walk until they became moribund shortly before death. The complications were: one postoperative infection, penetration of the nail through the femoral cortex in three femora before prebending of the nail became part of the procedure, and ectopic bone about the proximal end of the ro

The intention of the current article is to review the epidemiology with related socioeconomic costs, pathophysiology, and treatment options for diaphyseal long bone delayed unions and nonunions. Diaphyseal nonunions in the tibia and in the femur are estimated to occur 4.6-8% after modern intramedullary nailing of closed fractures with an even much higher risk in open fractures. There is a high socioeconomic burden for long bone nonunions mainly driven by indirect costs, such as productivity losses due to long treatment duration. The classic classification of Weber and Cech of the 1970s is base

The nail is the largest skin appendage. It grows continuously through life in a non-cyclical manner; its growth is not hormone-dependent. The nail of the middle finger of the dominant hand grows fastest with approximately 0.1 mm/day, whereas the big toe nail grows only 0.03-0.05 mm/d. The nails' size and shape vary characteristically from finger to finger and from toe to toe, for which the size and shape of the bone of the terminal phalanx is responsible. The nail apparatus consists of both epithelial and connective tissue components. The matrix epithelium is responsible for the production of

INTRODUCTION: The distal tibial nail (DTN) is a novel retrograde intramedullary nail used for distal tibial fracture stabilization. We investigated the clinical results of DTN use for distal tibial fractures and compared them with those reported in the literature on locking plates and antegrade intramedullary nails.
MATERIALS AND METHODS: This multicenter, prospective, observational cohort study examined distal tibial fractures with AO/OTA classification 43 types: A1, A2, A3 or C1. The primary outcomes included bone union rate, soft tissue problems, and surgical complications. Secondary outco

A broken intramedullary nail is a well-known complication of non-union of femur shaft fractures. Numerous surgical techniques have been presented before in patients with non-union of long bone fractures. We report the surgical technique used to perform removal of the broken distal segment of a nail in a patient who achieved uneventful union after intramedullary nailing of closed femur shaft fracture. A ball-tipped guidewire was inserted through the broken segment of the femur nail. A pre-bend plain wire was then inserted. With the help of a vise-grip, both wires were twisted in order to make a

We report a case of 68-year-old female patient who suffered two Gamma 3 nail breakages following trochanteric fixation failure revision surgery. A patient was primarily treated with sliding hip screw. Revision surgeries of reverse trochanteric nonunion were performed with short Gamma 3 nails and bone grafting. The nail failures occurred 11 and 7 months after implantation. This undesired series of events were caused by inappropriate implant selection, thus replacing the nail to a modular total hip arthroplasty was more effective as salvage surgery. Key words: salvage total hip arthroplasty, gam

Arthroscopic arthrodesis of the ankle has several advantages compared with open arthrodesis, including a smaller skin incision, less damage to the soft tissue around the joint, a lower risk of skin necrosis and infection, a lower incidence of postoperative infection and swelling, and better preservation of the contour of the surface of the joint, which maintains a larger contact area between the talus and tibia. We successfully performed arthroscopic arthrodesis of the tibiotalocalcaneal joints with intramedullary nails with fins in 9 ankles of 8 patients. Solid fusion was attained in all case

UNLABELLED: According to the statistics, long bone fractures in children are caused by high-energy mechanisms and they are the most frequent ones in the orthopedic practice. The advantage of reducing these fractures with elastic titanium nails and three buttress points is that neither the blood supply nor the physis are injured.
MATERIAL AND METHODS: Twenty-seven patients ages 5-15 years of age were analyzed. They presented at the Emergency Service, Polanco Red Cross Orthopedics and Trauma Hospital, between July 1st 2006 and July 1st 2007.
RESULTS: The sample size was 27 patients, the highes

BACKGROUND: Despite improvement in operative techniques and antibiotic therapy, septic complications still occur in open fractures. We developed silver ion containing ceramic nano powder for implant coating to provide not only biocompatibility but also antibacterial activity to the orthopaedic implants.
QUESTIONS/PURPOSES: We hypothesised silver ion doped calcium phosphate based ceramic nano-powder coated titanium nails may prevents bacterial colonisation and infection in open fractures as compared with uncoated nails.
METHODS: 33 rabbits divided into three groups. In the first group uncoate

OBJECTIVE: To evaluate the use of the spectral Doppler (SDoppler) to quantify inflammatory activity and to detect nail echotextural differences in patients with psoriatic arthritis and onychomycosis.
METHODS: Two patients, one with psoriatic arthritis but with no joint pain nor nail clinical change and the other with onychomycosis and rheumatoid arthritis were included. The gray scale ultrasound study, showed changes in the regular presence of echotexture at the nail insertion, thickening of the nail bed and loss of trilaminar nail pattern. The spectral Dopplerresistance index (RI), detects t
