Utilizing supplementary high-strength suture tying over a bone bridge enhances the effectiveness of interference screw as tibial fixation method in anterior cruciate ligament reconstruction.

BACKGROUND: Tibial fixation is the biomechanical weak point in anterior cruciate ligament (ACL) reconstruction, with numerous techniques proposed to address this challenge. This study evaluates the utility of a simple, cost-effective system: high-strength suture tying over a bone bridge (BB). Its effectiveness as standalone fixation and as a supplement to standard interference screw (IS) fixation is compared with IS alone and supplemented with other commercial systems. METHODS: A biomechanical study was conducted using six fixation methods tested on 61 specimens with bovine extensor tendons i

Enhanced construct stability with a locking plate system integrated into 3D-printed titanium implants for bone defect reconstruction: a biomechanical comparison of locking versus nonlocking screw fixation.

BACKGROUND: Fixation stability is critical for successful osseointegration of 3D-printed implants in segmental bone defect reconstruction. Although locking plate systems offer theoretical advantages over nonlocking systems, particularly in osteoporotic or high-load-bearing bones, their application in 3D-printed implants remains limited owing to manufacturing challenges. We aimed to evaluate the mechanical performance of locking versus nonlocking fixation in 3D-printed titanium implants. We hypothesized that locking fixation provides superior construct stability and fatigue resistance. METHODS

Early Fusion Status after Posterior Lumbar Interbody Fusion with Cortical Bone Trajectory Screw Fixation: A Comparison of Titanium-Coated Polyetheretherketone Cages and Carbon Polyetheretherketone Cages.

STUDY DESIGN: Retrospective cohort study. PURPOSE: We recently reported that when compared to posterior lumbar interbody fusion (PLIF) using traditional pedicle screw fixation, PLIF with cortical bone trajectory screw fixation (CBT-PLIF) provided favorable clinical outcomes and reduced the incidence of symptomatic adjacent segment pathology, but resulted in relatively lower fusion rates. Since titanium-coated polyetheretherketone (PEEK) cages (TP) could improve and accelerate fusion status after CBT-PLIF, early fusion status was compared between CBT-PLIF using TP and carbon PEEK cages (CP).
