Approach to complex upper extremity injury: an algorithm.

Patients with complex upper extremity injuries represent a unique subset of the trauma population. In addition to extensive soft tissue defects affecting the skin, bone, muscles and tendons, or the neurovasculature in various combinations, there is usually concomitant involvement of other body areas and organ systems with the potential for systemic compromise due to the underlying mechanism of injury and resultant sequelae. In turn, this has a direct impact on the definitive reconstructive plan. Accurate assessment and expedient treatment is thus necessary to achieve optimal surgical outcomes

Severe craniolacunae and upper and lower extremity anomalies resulting from Crouzon syndrome, FGFR2 mutation, and Ser347Cys variant.

Crouzon syndrome is a rare form of syndromic craniosynostosis (SC) characterized by premature fusion of the cranial and facial sutures, elevated intracranial pressure, varying degrees of ocular exposure due to exorbitism, and airway compromise caused by midface retrusion. Craniolacunae and upper and lower extremity anomalies are not frequently found in Crouzon syndrome. We present a girl with Crouzon syndrome caused by c.1040 C > G, p.Ser347Cys, a pathogenic mutation in the FGFR2 gene with atypical characteristics, including craniolacunae resembling severe Swiss cheese type of bone formation,

Humeral stress edema: an injury in overhead athletes quarterback with humeral "shin" splints-a case report.

Stress injury to the distal humerus has been reported as a cause of chronic arm pain among competitive tennis players. As the serve is technically similar to an overhead throwing motion, it is conceivable that this injury could occur in other overhead athletes. Overuse injuries, including rotator cuff tears, ulnar collateral ligament injuries, and joint sprains have routinely been reported in quarterbacks, but there are few reported cases of upper extremity bony stress injuries in these athletes. Magnetic resonance imaging aids in the diagnostic evaluation of an upper extremity injury as it ca

Osteopathia striata with cranial sclerosis: clinical, radiological, and bone histological findings in an adolescent girl.

Osteopathia striata with cranial sclerosis (OS-CS) is a rare skeletal dysplasia characterized by linear striations of the long bones, osteosclerosis of the cranium, and extra-skeletal anomalies. We provide a comprehensive description of the skeletal phenotype in a French-Canadian girl with a moderate to severe form of sporadic OS-CS. Multiple medical problems, including anal stenosis and the Pierre-Robin sequence, were evident in the first few years of life. At 14 years, she was fully mobile, with normal intellect and stature. She suffered chronic lower extremity pain in the absence of fractur

Three-Dimensional Corrective Osteotomy for Malunited Fractures of the Upper Extremity Using Patient-Matched Instruments: A Prospective, Multicenter, Open-Label, Single-Arm Trial.

BACKGROUND: Medical image processing has facilitated simulation of 3-dimensional (3-D) corrective osteotomy, and 3-D rapid prototyping technology has further enabled the manufacturing of patient-matched surgical guides and implants (patient-matched instruments, or PMIs). However, 3-D corrective osteotomy using these technologies has not been the standard procedure. We aimed to prospectively verify the efficacy and safety of PMIs in corrective osteotomy for deformities of the upper extremity. METHODS: We enrolled 16 patients with a total of 17 bone deformities in the upper extremity. Eight pat

[Surgical treatment of humeral metastatic tumors].

OBJECTIVE: To evaluate the functional outcome and pain control in patients after resection of humeral metastases. MATERIAL AND METHODS: A prospective randomized study of 24 cases of metastatic disease of the humerus with an associated pathologic fracture was carried out. The selected cases were divided into two groups based on the using methylmethacrylate cement for fracture fixation. Group 1 (n=12) included all cases in which the fracture was treated with bone cement augmentation. Group 2 (n=12) included all cases in which the fracture was treated without bone cement augmentation. Functional

Emergency ultrasound in the detection of pediatric long-bone fractures.

BACKGROUND: Long-bone fractures represent one of the most commonly sustained injuries following trauma and account for nearly 4% of emergency department visits in the United States each year. These fractures are associated with a significant risk of bleeding and neurovascular compromise. Delays in their identification and treatment can lead to loss of limb and even death. Although emergency physicians currently rely predominantly on radiography for the examination of long-bone injuries, emergency ultrasound has several advantages over radiography and may be useful in the identification of long

[Radiological findings in pseudohypoparathyroidism].

Clinical and radiodiagnostic changes in pseudohypoparathyroidism are described in three members of one family. Radiodiagnostic changes consist of symmetric calcifications in basal ganglia, perivascular calcifications in soft tissues, and of alterations on the skeleton--Albright's osteodystrophy. Bone changes include a combination of osteoproductive and osteomalacic alterations. All these changes were observed in both brothers, their mother had only calcifications localised in upper and lower extremities.

[Particular localisation of hydatic disease--sternum].

Bones localization of hydatic disease is extremely rare (0.5-2.5 %). In approximative 50% of the cases of bones hydatidosis, the cysts are localized at spines vertebrae, broad bones and mandible. We present a case of a 38 years old female. The patient related an insidious beginning of symptoms, about 3-4 years before, with anterior chest pain, and 3-4 month before hospitalization, the presence of a sternal tumor, in upper portion of the bone. Clinical examination of the patient showed a sternal tumor, at manubrium, painful at palpation, increased consistency, with local inflammation signs. Als

The angiosomes of the forearm: anatomic study and clinical implications.

The angiosome concept was introduced in 1987 by Taylor and Palmer. Their anatomic study correlated the blood supply to the skin from the named segmental or distributing "source" arteries with their supply to the underlying muscles, tendons, nerves, and bones. Although this investigation encompassed the body, there were areas where the supply to individual tissues was not examined in detail. The present study, therefore, examines one of these regions where certain voids in our knowledge still exist--the forearm. Ten upper limbs from fresh cadavers were studied over an 18-month period after perf

Lessons from the modern battlefield: successful upper extremity injury reconstruction in the subacute period.

BACKGROUND: Reconstructive techniques and protocols for limb salvage of upper extremity battlefield injuries remains poorly defined. Our study describes the types of flaps, the timing of reconstruction, and success rates of war upper extremity reconstruction during a 30-month period using the Bethesda limb salvage protocol. METHODS: Soldiers with significant upper extremity injuries with complex open fractures from Operation Iraqi Freedom and Operation Enduring Freedom-Afghanistan who underwent tissue transfer flaps were reviewed (n = 26, free flaps n = 6). Data analysis included mechanism of

Strategy for ipsilateral multiple concomitant upper extremity fractures in an elderly polytraumatized patient with severe osteoporosis: A case report.

RATIONALE: Elderly patients with severe osteoporosis present significant challenges in managing multiple concomitant upper extremity fractures, especially when high-energy trauma is involved. The rarity of such cases necessitates a structured approach to achieve optimal outcomes, balancing surgical intervention and early rehabilitation. PATIENT CONCERNS: A 79-year-old woman presented to the emergency department following a severe car accident. She had multiple concomitant fractures, including those of the proximal humerus, proximal ulna, and open fractures of the radius in the ipsilateral upp

An improved bone transport surgical method for treating chronic ischemic ulcers (thromboangiitis obliterans).

INTRODUCTION: The chronic ischemic injury of the upper/lower limbs caused by thromboangiitis obliterans (TAO, Buerger's disease) is difficult to heal, leading to high morbidity and amputation risk, seriously lowering the quality of life of patients. So far, the pathogenesis of this disease is still not clear, and there are still no effective therapeutic approaches. Here, we first use an improved bone transport technique to treat TAO-related foot ulcers and achieve good therapeutic effects. MATERIALS AND METHODS: In this report, 22 patients met the inclusion criteria, and we provide an improve

Unusual presentation of an intraosseous hemangioma of the maxilla and displaced canine.

Intraosseous hemangiomas are benign vascular malformations which are extremely rare in the maxilla, but have been reported in the mandible, zygoma and orbital region. A 12 years old female presented to the oral and maxillofacial department with an ectopically positioned upper left canine in her zygomatic bone and buccal alveolar expansion between the upper left lateral incisor and upper left first premolar. This case shows the unusual presentation of an intraosseous hemangioma associated with an ectopically migrated upper left canine tooth. It is possible that this lesion caused migration of t

[A possibility of central diffusion during stellate ganglion blockade: "the sheath of the spinal rachidian nerve"].

Stellate ganglion block is a common treatment for neuropathic pain. The technique is not without potentially severe complications when a paratracheal approach is used. A 33-year-old woman complained of atypical facial pain of 15 years' duration with pain intensity of 6 to 8 on a visual analog scale and no pain-free periods upon use of inadequate analgesia. One minute after performing a second stellate ganglion block the patient showed signs of apnea and paralysis of the upper extremities and face, with no involvement of oculomotor muscles or the lower extremities, and no loss of consciousness.

[Ipsilateral Fractures of the Coracoid and Acromion Process of the Scapula Combined with the Distal Clavicle End Fracture, Treatment Options].

Superior Shoulder Suspensory Complex (SSSC) is a bone and soft-tissue ring securing the connection of the upper extremity to the axial skeleton via the clavicle and sternoclavicular joint. An isolated injury to one component of SSSC is usually stable. An injury to 2 of its components is a potential source of shoulder girdle instability and requires surgical stabilisation. An injury affecting 3 and more components is extremely rare and surgical stabilisation should be indicated. Our study presents the case of a 50-year-old man who fell off the bicycle and sustained a direct blow to his left sho

[Characteristic of combat trauma and treatment of gunshot fractures of long bones of the limbs].

Treatment of the patients with gunshot fractures of extremity long bones is one of the main problems of field surgery. The complex study of medical records obtained from 718 servicemen with gunshot fractures of extremity long bones who participated in counter-terrorist operation in the Republic of Chechnia (1994-1996) allowed to evaluate character and severity of the injuries, content of the treatment measures conducted at the stages of medical evacuation. The peculiarities of fighting trauma were the following: the high frequency of combined (22.5%) and multiple (25.9%) injuries, the high sha
