Multimodal electrophysiological examinations in patients suffering from various tumors of the pineal region.

Multimodal electrophysiological examinations: blink-, glabella- and masseter-reflexes, as well as brain stem acoustic, somatosensory and visual evoked potentials were examined in thirteen patients with clear consciousness suffering from extra-axial, chronic, expanding processes in the tectal region. According to the data, the authors came to the conclusion that several modalities were often required to make a correct diagnosis or to the localization of the space occupying processes. Functional disturbances of the whole of the lower brain stem, but especially of the mesencephalon and of the low

Predominance of pilocytic histology in dorsally exophytic brain stem tumors.

We report the magnetic resonance imaging (MRI) and clinico-histologic characterization of dorsally exophytic brain stem gliomas (DEBSGs). Between 1983 and 1991, 12 of 51 patients evaluated for the diagnosis of brain stem glioma were found to have DEBSGs emanating from the pons, pontomedullary junction or medulla. Eleven of the 12 patients had classic juvenile pilocytic astrocytomas. Unlike most other brain stem tumors, these patients were young (median 38 months, range 17-75), had a relatively long duration of symptoms (median 7 months, range 2-24) and displayed signs of increased intracranial

[Angiographic findings in 86 confirmed tumours of the posterior cranial fossa (author's transl)].

The accuracy of vertebral angiography in the diagnosis of tumours was reviewed on the basis 368 selective angiograms. The greatest accuracy was achieved with space-occupying lesions situated centrally in the cerebellum in the region of the fourth ventricle or in the vermis. However, tumours growing eccentrically, but arising in this region, were occasionally missed. Least reliable is the angiographic diagnosis of tumours is the cerebellar hemispheres. Of these, only about two-thirds could be demonstrated. Only 60% of cerebello-pontine tumours were diagnosed from displacement of various structu

Myelodysplasia From Valproic Acid Resolved by Cessation.

Myelodysplastic neoplasms (MDS) are clonal blood disorders that cause ineffective hematopoiesis, low blood counts, and a risk of progression to acute myeloid leukemia. Chemotherapy and radiation usually drive these changes, but a small set of medications, including valproic acid (VPA), can also produce reversible myelodysplastic features. We report a case of VPA-induced myelodysplasia in a middle-aged woman with complex neurologic and psychiatric comorbidities. The patient, with a history of paraplegia secondary to traumatic brain injury and epilepsy, was referred for evaluation of pancytopeni

Case report: long-term survival of an infant syndromic patient affected by atypical teratoid-rhabdoid tumor.

BACKGROUND: Atypical teratoid rhabdoid tumor (ATRT) patients display a dismal median overall survival of less than 1 year. A consistent fraction of cases carries de-novo SMARCB1/INI1 constitutional mutations in the setting of the "rhabdoid tumor predisposition syndrome" and the outcome is worst in infant syndromic ATRT patients. CASE PRESENTATION: We here describe a patient affected by mosaic Klinefelter syndrome and by rhabdoid tumor predisposition syndrome caused by constitutional SMARCB1/INI1 heterozygous mutation c.118C>T (Arg40X). Patient's ATRT primary tumor occurred at 2 years of age c

Diffuse Intrinsic Pontine Gliomas Exhibit Cell Biological and Molecular Signatures of Fetal Hindbrain-Derived Neural Progenitor Cells.

Diffuse intrinsic pontine glioma (DIPG) is the main cause of brain tumor-related death among children. Until now, there is still a lack of effective therapy with prolonged overall survival for this disease. A typical strategy for preclinical cancer research is to find out the molecular differences between tumor tissue and para-tumor normal tissue, in order to identify potential therapeutic targets. Unfortunately, it is impossible to obtain normal tissue for DIPG because of the vital functions of the pons. Here we report the human fetal hindbrain-derived neural progenitor cells (pontine progeni

Brain stem and spinal metastases of supratentorial glioblastoma multiforme: a clinical series.

Although the spread of supratentorial glioblastoma multiforme to the brain stem and spine has been extensively described in published autopsy series, information on the diagnosis, treatment, and subsequent clinical course of patients manifesting symptoms of glioblastomatous dissemination ante mortem remains scant. We report a series of 11 patients having the signs and symptoms of neuraxis dissemination of supratentorial glioblastoma multiforme. All patients had radiographic documentation of metastases by either contrast-enhanced myelograms or enhanced magnetic resonance imaging scans. Ten pres

[Comparison of the results of computer tomography and dimer-X ventriculography in the diagnosis of infratentorial processes].

A total of 38 preoperative computer-tomography findings were compared with the results obtained in central dimer-X-ventriculography. CT yields the exact localisation of every tumour. Determination of kind and size of the tumours was possible in about 30 per cent of the cases. No additional information was obtained about the infiltration tendency of the tumour with respect to the rhomboid fossa. The method appears to be insufficient for the appraisal of occlusion of the efferent passage of the cerebrospinal fluid which are not caused by tumours. In 360 f 38 cases, the direct puncture of the thi

An overview of the diagnosis and management of Choroid Plexus tumors.

Choroid Plexus Tumors (CPT) are rare (2-4% of all pediatric CNS tumors), predominantly early childhood brain neoplasms. Due to their rarity and the lack of prospective clinical trials, evidence-based treatment guidelines remain limited. This review provides a comprehensive summary of the current knowledge.\n\nCPTs span a spectrum from mature Choroid Plexus Papillomas (CPP) to malignant Choroid Plexus Carcinomas (CPC), with Atypical Choroid Plexus Papillomas (aCPP) in between. A significant proportion of CPCs are driven by either somatic or germline TP53 mutations (Li-Fraumeni syndrome); howeve

Diagnostic Yield and Safety of Cerebellar and Brainstem Parenchymal Biopsy.

OBJECTIVE: We aimed to determine the diagnostic yield and safety of posterior fossa parenchymal biopsy. METHODS: One-hundred-thirty-six patients who underwent 137 posterior fossa (brainstem or cerebellar) parenchymal biopsies at Mayo Clinic (Rochester, Minnesota, USA) between 1996 and 2009 were identified by chart review. Case histories; radiologic, surgical, and pathologic reports; and safety outcomes were assessed. RESULTS: Posterior fossa parenchymal biopsies were performed on 78 male and 58 female patients of median age 47 years (interquartile range 28-61). Preoperative clinical diagnosi

Brainstem auditory evoked potential monitoring: when is change in wave V significant?

BACKGROUND: The probability of hearing loss during cerebellopontine angle (CPA) surgery can be reduced by using brainstem auditory evoked potential (BAEP) intraoperative monitoring (IOM). A wave V latency prolongation of 1.0 milliseconds or amplitude decrement of greater than 50% is arbitrarily considered the point when damage to hearing occurs. OBJECTIVE: To determine the accuracy of wave V changes in predicting hearing impairment. METHODS: Patients undergoing BAEP IOM for surgery in the CPA region were evaluated. The greatest wave V latency and amplitude change was determined. Patients wer

Induced Telomere Damage to Treat Telomerase Expressing Therapy-Resistant Pediatric Brain Tumors.

Brain tumors remain the leading cause of cancer-related deaths in children and often are associated with long-term sequelae among survivors of current therapies. Hence, there is an urgent need to identify actionable targets and to develop more effective therapies. Telomerase and telomeres play important roles in cancer, representing attractive therapeutic targets to treat children with poor-prognosis brain tumors such as diffuse intrinsic pontine glioma (DIPG), high-grade glioma (HGG), and high-risk medulloblastoma. We have previously shown that DIPG, HGG, and medulloblastoma frequently expres

Meningeal and brainstem infiltration by a malignant mesothelioma.

Malignant mesothelioma is an uncommon neoplasia which primarily involves the pleura or peritoneum. Central nervous system involvement is rare. A rare presentation of metastatic pleural mesothelioma, which had infiltrated the meninges and brainstem, is described. The patient presented with diplopia following a 2-week history of malaise, myalgia, mild headache and diarrhoea. Clinical examination found global areflexia, cerebellar ataxia and bilateral sixth nerve palsies. Differential diagnoses included the Miller-Fisher variant of Guillain-Barre syndrome, malignant meningitis and infectious meni

Computed tomography: evaluation of the posterior third ventricle.

The normal computed tomography (CT) appearances of the posterior third ventricle and related structures are presented. Seventy-six patients with lesions directly involving this ventricle were studied by CT, and results of other neuroradiological procedures evaluated. Primary tumors, metastases, hematomas, infarcts, and cysts may be reliably distinguished by CT, particularly if contrast enhancement is utilized. Size and density resolution limit the direct visualization of the aqueduct of Sylvius; stenosis however may be inferred. Small infarcts may be difficult to demonstrate by the use of CT s

Improvement of survival and prospect of cure in patients with metastatic breast cancer.

Patients with metastatic breast cancer have traditionally been considered incurable with conventional treatment. However, 5-10% of those patients survive more than 5 years, and 2-5% survive more than 10 years. Recent studies suggest that the survival of patients with metastatic breast cancer has been slowly improving. In this review, we examine the possible curative approach for a certain group of patients with metastatic breast cancer. We identify that patients most likely to benefit from such an aggressive approach are young and have good performance status, adequate body functional reserve,
