Abnormal branching of facial nerve with ossicular anomalies: report of two cases.

Two cases with almost identical anomalies of the facial nerve and ossicles were reported. The two girls, ages 9 and 14 years, with unilateral hearing loss underwent exploratory tympanotomy. A huge suprameatal spine and tumor-like swelling of the facial nerve at the tympanic portion as well as its abnormal branching at the pyramidal bend were noted; one of the branches of the facial nerve appeared on the surface of the temporal bone running through the tympanomastoid suture. The distal parts of the long process and lenticular process of incus were missing as if they were eroded by the swollen f

[Diagnosis and management of piriform sinus fistulae in children].

In 12 patients with piriform sinus fistula, aged from 1.5 to 10 years all but one patient presented themslves with inflammatory swelling in the region of the left lobe of the thyroid gland. During acute infection, the involvement of the left thyroid lobe was verified by ultrasonography and scintigraphy. Barium swallow examination was the diagnostic finding that all the 11 patients had a left-sided fistula. One patient presented with a painless cyst in the left thyroid lobe. Six patients operated on 4 underwent fistulectomy. In patient the fistula was not found. In the patient who had a cyst in

Experimental craniofacial malformations induced by retinoids and resembling branchial arch syndromes.

A syndrome which showed similarities to human branchial arch syndromes could be induced in Sprague-Dawley rat embryos by exposing them to retinoids prenatally. Treatment of pregnant rats with 40 mg/kg retinoic acid or 10 mg/kg etretinate on pregnancy day 8.5-9 resulted in craniofacial defects in 100% of the embryos. A scanning electron microscopic investigation of the early stages in the development of these malformations showed abnormal skull form, disorganised surface epithelium with "cell blebbing", lateral facial clefts, facial fistulas, narrowed skull-base and reduced size of the nasal an

Median facial malformations and their implications for brain malformations.

For purposes of identifying craniofacial syndromes which predict brain malformations the face can be regarded as developing from 2 sources. The median and paramedian structures derive from the embryonic segment of the face, termed the frontonasal prominence. The lateral structures of the face derive from the branchial arches. Certain patterns of median plane facial anomalies predict a severe brain malformation, while other patterns, although producing equally grotesque facial malformations, bear little association with a malformed brain. Orbital hypotelorism may occur in a variety of syndromes

NOC4L coordinates neuronal and pharyngeal arch development by regulating ribosome biogenesis.

Mutations in ribosome biogenesis-related genes or functional defects in ribosomal proteins can lead to a class of autosomal genetic disorders characterized by tissue-specific defects, termed ribosomopathies. NOC4L, a critical factor in ribosome biogenesis, participates in the maturation of the 40S small ribosomal subunit. However, its functions in neural and cartilage development remain incompletely understood. In this study, through generation and phenotypic characterization of a zebrafish noc4l knockout model, we identified severe developmental abnormalities including microcephaly, micrognat

Surgical and embryological perspective of a big loop of internal carotid artery extending laterally beyond internal jugular vein.

Knowledge of variations of the internal carotid artery is significant to surgeons and radiologists. The internal carotid artery normally runs a straight course in the neck. Its anomalies can lead to its iatrogenic injuries. We report a case of a large loop of the internal carotid artery in a male cadaver aged about 75 years. The common carotid artery terminated by dividing it into the external carotid artery and internal carotid arteries at the level of the upper border of the thyroid cartilage. From the level of origin, the internal carotid artery coursed upwards, backwards and laterally, and

Congenital anomalies presenting as recurrent post-auricular abscesses: an institution based retrospective study.

OBJECTIVES: This report is intended to bring out the association of recurrent postauricular abscess in children with various underlying congenital anomalies. MATERIALS AND METHODS: A retrospective study was done in tertiary care setting from the year August 2005 to February 2012 using the medical record department database to retrieve the patient details. During this study period, a total of 215 children with an age range of 1-14 years were treated at the hospital with postauricular abscess. We analysed the data using appropriate statistical tests for parametric and nonparametric data and set

Congenital neck masses in children and their embryologic and clinical features.

Congenital neck masses in children and their embryologic and clinical features. Neck masses of congenital origin can be diagnostic and therapeutic challenges for internists, paediatricians and surgeons. Treatment modalities of congenital neck masses are different depending on their nature, symptoms and location. Differential diagnosis includes a variety of diseases that can cause cervical masses such as infectious and neoplastic neck tumours. Our objective is to review the embryologic and clinical features of some of the most common congenital neck masses such as the haemangioma, branchial cle

Dlx5/6-enhancer directed expression of Cre recombinase in the pharyngeal arches and brain.

Dlx5 and Dlx6, two members of the Distalless gene family, are required for development of numerous tissues during embryogenesis, including facial and limb development. This gene pair is expressed in tandem, transcribed toward each other and separated by a short intergenic region containing multiple putative enhancers. Targeted inactivation of Dlx5 and Dlx6 in mice results in multiple developmental defects in craniofacial and limb structures, suggesting that these genes are crucial for aspects of both neural crest and nonneural crest development. To further investigate potential developmental r

Congenital unilateral fusion of the maxilla and mandible.

We present an uncommon severe first branchial arch congenital malformation, in which complete unilateral bony fusion between the maxillary and mandibular processes was found in a newborn exposed to carbamazepine medication all through pregnancy. This condition interferes with oral feeding, intubation, growth and development. In a review of previously reported cases it was found that this anomaly was commonly associated with other abnormalities. The etiology of this malformation was uncertain in our case, as carbamazepine was not proven to be the definite cause.

X-linked syndrome of branchial arch and other defects.

We report on two brothers and their maternal first cousin who have branchial arch defects and other anomalies. Similar physical findings in all three include microcephaly, downslanting palpebral fissures, highly arched palate, apparently lowset, protruding ears, bilateral hearing loss, slightly webbed neck, and mild short stature. In addition, two boys had cryptorchidism, and one had subvalvar pulmonic stenosis and body asymmetry. We suggest that these cousins have an X-linked syndrome of which branchial arch defects are a component. Other pleiotropic manifestations of the mutant gene include

First branchial cleft anomalies.

A series of nine cases of first branchial cleft anomalies treated at Children's Hospital of Michigan from 1970-1978 is presented. A review of the literature and analysis of these cases reveals that the accepted criteria for classification of these anomalies are confusing and inadequate as they preclude the proper classification of a number of these cases. The embryogenesis of these anomalies is discussed with attention given to their relationship to the facial nerve. A proper classification is suggested.

[Branchiogenic preauricular fistulae].

Seventeen patients with dimples, fistulae and cysts in the region of the cheek and the preauricular field are reported. All are located in the neighborhood of the dorsal end of the first visceral arch. Both the fistulae of the ascending helix and the upper preauricular fistulae must be regarded as relics of the dorsal end of the first branchial cleft. This interpretation supports the theory of Wood-Jones and I-Chuan according to which the tragus is formed from the material of the mandibular arch, and the remaining part of the auricle from the mesoderm of the second visceral arch. Similar anoma

Ciliated HPV-related Carcinoma: A Well-differentiated Form of Head and Neck Carcinoma That Can Be Mistaken for a Benign Cyst.

Although human papillomavirus (HPV)-related oropharyngeal carcinomas (HPV-OPCs) are generally regarded as "poorly differentiated," they actually maintain a close resemblance to the lymphoepithelium of the tonsillar crypts from which they arise: they are basaloid, exhibit minimal keratinization, and are often permeated by lymphocytes. In rare cases, the presence of cilia in a primary HPV-OPC and their persistence in lymph node metastasis can confound the distinction between a benign and malignant process. Three cases of ciliated HPV-OPCs were identified from the archives of The Johns Hopkins He

Histologic characterization of the thymus in canine X-linked severe combined immunodeficiency.

This study describes the thymic morphology in 52 dogs (ranging in age from 1 to 79 days) with X-linked severe combined immunodeficiency disease (XSCID). The thymuses from the XSCID dogs and age-matched controls were evaluated histologically for the presence of Hassall's corpuscles and branchial duct remnants, the degree of corticomedullary differentiation, and lymphoid development and organization. Within this population of XSCID dogs with the same genetic defect, three histologic patterns of thymic dysplasia were recognized. Simple dysplasia, noted in 27 XSCID thymuses, was characterized by v
