Nirsevimab and maternal respiratory syncytial virus (RSV) vaccination are delivering real-world important reductions in RSV-bronchiolitis hospitalisations, a landmark achievement in paediatric preventive medicine. We argue, however, that these successes should not be mistaken for the resolution of severe bronchiolitis as a clinical and public health problem. First, RSV's epidemiological dominance may partly reflect its transmission efficiency rather than a unique pathogenic superiority: rhinovirus, human metapneumovirus, adenovirus, and bocavirus already cause substantial disease, and genetic …

OBJECTIVE: New preventive therapeutics could substantially reduce bronchiolitis cases requiring acute care. Understanding the post-pandemic geographic distribution of bronchiolitis morbidity can guide targeted immunization campaigns in high-risk areas. This study aimed to identify geographic clusters of bronchiolitis acute care discharges across Connecticut.
METHODS: This retrospective cross-sectional cohort study used a statewide database to identify home ZIP Code Tabulation Areas (ZCTAs) of children under 2 years discharged following bronchiolitis emergency visits or hospitalizations. Disch…

Respiratory syncytial virus (RSV) remains a leading cause of bronchiolitis and hospitalization in infants across Italy, particularly during predictable seasonal outbreaks. With the 2023-2025 RSV seasons marking the first large-scale rollout of the monoclonal antibody nirsevimab, this study aimed to evaluate the implementation strategies, regional variability, and early clinical impact of RSV prophylaxis at the national level. We conducted a nationwide, multicenter, observational study across 19 Italian regions. Data were collected through the Italian Society of Pediatrics and the Italian Socie…

BACKGROUND: Influenza C virus (ICV), a lesser-known member of the Orthomyxoviridae family, usually causes mild respiratory illness in children. Due to its low prevalence and clinical similarity to other respiratory infections, it is not routinely screened. This study investigates the detection of ICV in children with acute bronchiolitis (AB) and febrile seizures (FS), comparing the findings with a healthy control group.
METHODS: Between October 2009 and September 2011, 499 nasopharyngeal swabs were collected from children up to 6 years old at the University Medical Center Ljubljana. The study…

Introduction Bronchiolitis is the most common cause of respiratory-related hospital admissions in children under two years. It is typically caused by viral infections, most commonly respiratory syncytial virus (RSV). Despite the availability of various international guidelines, Malta lacks local protocols, leading clinicians to rely on international recommendations for management. Objectives The primary aim of this audit was to compare epidemiological and management trends in bronchiolitis among children under one year admitted to Mater Dei Hospital between 2019 (pre-COVID) and 2024 (post-COVI…

INTRODUCTION: In Morocco, acute viral bronchiolitis remains a major public health problem, and its incidence continues to rise. Acute viral bronchiolitis can be severe and even fatal, especially in vulnerable populations. The objectives of this study were to analyze the causes of death due to viral bronchiolitis and to highlight the importance of prophylaxis in high-risk groups.
METHODS: This was a retrospective, descriptive study spanning 11 years and 11 months, from January 1, 2013, to December 10, 2024. We included all cases of acute bronchiolitis complicated by infant death. The study foc…

OBJECTIVE: Pediatric cardiac tumors are rare and, among them, 90% are benign. Cardiac fibroma is the second most frequent tumor, after rhabdomyoma. The objective of this study is to report a case of cardiac fibroma diagnosed incidentally in a patient admitted with acute viral bronchiolitis.
CASE DESCRIPTION: A 5-month-old male infant was admitted to the pediatric emergency department with acute viral bronchiolitis requiring hospitalization. He presented a detectable respiratory syncytial virus in oropharyngeal swab, blood test with lymphocytosis and a chest radiography revealed cardiomegaly. …

Acute bronchiolitis is among the most common causes of hospitalizations in infants worldwide. Associations between weight and severity of respiratory syncytial virus (RSV) bronchiolitis remain unclear. The aim of this study was to evaluate this association. A single-center, retrospective cohort study of infants aged under 24 months, who were hospitalized between 2018 and 2022 for RSV bronchiolitis. Data from computerized medical records were extracted using the MDclone platform. Participants were divided into three groups according to weight percentiles: underweight (below 5th percentile), nor…

BACKGROUND: Viral bronchiolitis is a common cause of acute respiratory failure requiring intubation for infants. Bacterial respiratory tract infections can occur with bronchiolitis, although their prevalence and impact on outcomes are unclear, especially with increased use of noninvasive respiratory support.
METHODS: This was a single-center retrospective cohort study of children <2 years old requiring intubation in the emergency department for bronchiolitis from 2012 to 2017 who had viral testing plus a lower respiratory culture obtained. We evaluated the impact of bacterial codetection (pos…

BACKGROUND: Infants with bronchiolitis are at increased risk for developing asthma. Growing evidence suggests bronchiolitis is a heterogeneous condition.
OBJECTIVES: We sought to identify biologically distinct subgroups based on the metabolome signatures (metabotypes) in infants with severe bronchiolitis and to examine the longitudinal relationships of metabotypes with asthma development.
METHODS: In a multicenter prospective cohort study of infants (age, <12 months) hospitalized for bronchiolitis, the nasopharyngeal airway metabolome was profiled at hospitalization. Using a clustering appro…

Objectives This study examines the practice patterns with respect to the technique of non-oral hydration of infants admitted with bronchiolitis at one Canadian tertiary care institution. Additionally, the authors assess the infants' parents' attitudes regarding hydration through a nasogastric (NG) tube instead of an intravenous (IV) line. Methods A retrospective chart review was conducted for all infants admitted with bronchiolitis from May 1, 2016, to April 30, 2018, with a focus on the method of hydration, investigation with chest radiography, and use of IV antibiotics. Parents of infants wh…

BACKGROUND: Recent studies support the existence of several entities under the clinical diagnosis of bronchiolitis. Among infants with severe bronchiolitis, distinct profiles have been differentially associated with development of recurrent wheezing by age 3 years. However, their associations with actual asthma remain unclear.
OBJECTIVE: Our aim was to study the association between severe bronchiolitis profiles identified by using a clustering approach and childhood asthma.
METHODS: Among 408 children (aged <2 years) hospitalized with bronchiolitis in Finland (in 2008-2010), latent class ana…

A 58-year old woman presented for autopsy after having been found unresponsive in a public bathroom surrounded by a pool of blood. During attempts at resuscitation, blood was noted in her airway. She had a past medical history that included surgical repair of Tetralogy of Fallot as a child. At autopsy, a shard of glass was identified projecting from the surface of the left lung, having formed densely fibrotic adhesions at the pleural surface. The glass also penetrated through a bronchiole lumen and into a previously surgically repaired bulging right ventricular outflow tract, forming a bronchi

BACKGROUND: High-flow oxygen therapy through a nasal cannula has been increasingly used in infants with bronchiolitis, despite limited high-quality evidence of its efficacy. The efficacy of high-flow oxygen therapy through a nasal cannula in settings other than intensive care units (ICUs) is unclear.
METHODS: In this multicenter, randomized, controlled trial, we assigned infants younger than 12 months of age who had bronchiolitis and a need for supplemental oxygen therapy to receive either high-flow oxygen therapy (high-flow group) or standard oxygen therapy (standard-therapy group). Infants …

Current data indicate that the "bronchiolitis" diagnosis comprises more than one condition. Clinically, pathophysiologically, and even genetically three main clusters of patients can be identified among children suffering from severe bronchiolitis (or first wheezing episode): (a) respiratory syncytial virus (RSV)-induced bronchiolitis, characterized by young age of the patient, mechanical obstruction of the airways due to mucus and cell debris, and increased risk of recurrent wheezing. For this illness, an effective prophylactic RSV-specific monoclonal antibody is available; (b) rhinovirus-ind…

Follicular bronchiolitis (FB) is a rare interstitial lung disease (ILD) and has been reported in diverse clinical contexts. Six FB patients demonstrated by surgical lung biopsy (SLB) were reviewed between 2009 and 2017 from Nanjing Drum Tower Hospital in China. The average age of subjects was 42 years old (range: 31-55 years). The clinical symptoms were very mild. The laboratory findings showed elevated Erythrocyte sedimentation rate (ESR) and serum globulin and anemia. Pulmonary function tests were normal in four cases. Five cases had underlying diseases, such as, Sjo¨gren's syndrome, multi-c…
