OBJECTIVE: Eating disorders may be associated with alterations in sleep. There is evidence that some bulimia nervosa (BN) patients have sleep abnormalities. No studies of their sleep-wake cycles in a natural environment have as yet been reported. Bulimia nervosa is very rare in children below the age of 14 years, and no reliable reports of prepubertal bulimia nervosa have been published. We describe two cases of early-onset bulimia nervosa who presented before the age of 14 years, and with premenarchal onset in one patient. Both girls demonstrated high levels of the risk factors known to play a part in the etiology of bulimia nervosa. Implications of these cases regarding the etiology and occurrence of bulimia nervosa in younger adolescents are discussed.

Young women in First World countries, particularly those who "diet", are at increased risk. Behaviours, such as binge eating and induced vomiting, are typically kept well hidden. Using data from 61 bulimic patients, we evaluated associations among axis II disturbances, psychopathological traits, eating symptoms, and adverse developmental experiences (e.g., childhood sexual and physical abuse). Abuse of dieting agents is a frequent, but often unrecognized, problem in patients with eating disorders. Especially women suffering from bulimia nervosa tend to abuse diet pills containing amphetamines and laxatives, diuretics, caffeine and emetics. In this review the frequency of substance abuse in bulimic women, the side effects and possible health risks of this abuse are discussed.

Thirty women who sought treatment at an eating disorders clinic in a general hospital and were diagnosed as having a current episode of bulimia nervosa were followed prospectively to assess the course of eating behaviors and other psychopathology. At intake, all 30 met DSM-III criteria for bulimia nervosa, and 57% met Research Diagnostic Criteria for an affective disorder. Preliminary results are reported on the efficacy of a 4-month group (n = 8) therapy compared with waiting-list controls (n = 9) in women with DSM-III bulimia. Treatment consisted of a standard behaviorally oriented stress management program and of basic nutritional management. A significantly greater decrease in binge frequency and specific psychopathology was found in the treated compared with the waiting-list group. The treated subjects showed further improvement 3 months after treatment termination. The usefulness of the different treatment components is briefly discussed.
