Clinical use conditions of lead deployment and simulated lead fracture rate in left bundle branch area pacing.

INTRODUCTION: Left bundle branch area pacing (LBBAP) is achieved by advancing the lead tip deep in the septum. Most LBBAP implants are performed using the Medtronic SelectSecure™ MRI SecureScan™ Model 3830 featuring a unique 4 Fr fixed helix lumenless design. Details of lead use conditions and long-term reliability have not been reported. This study was designed to quantify the mechanical use conditions for the 3830 lead during and after LBBAP implant, and to evaluate reliability using bench testing and simulation. METHODS: Fifty bradycardia patients with implantation of the 3830 lead for LBB

Influence of Capture Selectivity and Left Intrahisian Block on QRS Characteristics During Left Bundle Branch Pacing.

OBJECTIVES: This study sought to examine QRS and intracardiac characteristics during selective (S) and nonselective (NS) left bundle branch pacing (LBBP) from direct left septal recordings. BACKGROUND: Criteria for S-LBBP and NS-LBBP have not been validated with intracardiac mapping. METHODS: Pacing was performed from multielectrode Purkinje recordings below the left-sided His. S-LBBP and NS-LBBP were performed in patients with narrow QRS (n = 9), right bundle branch block (n = 3), intraventricular conduction delay (n = 5), and left bundle branch block (n = 10). QRS duration was measured fro

What Intracardiac Tracings Have Taught Us About Left Bundle Branch Block.

Current electrocardiogram (ECG) criteria for left bundle branch block (LBBB) are largely based on early work in animal models or on mathematical models of cardiac activation. The resulting criteria have modest specificity, and up to one-third of patients who meet current ECG criteria for LBBB may have intact conduction through their His-Purkinje systems. Intracardiac tracings offer the ability to accurately discriminate between LBBB and other causes of delayed activation, which may facilitate the development of more accurate ECG criteria. Assessing these distinctions are particularly salient t

Interesting electrophysiological phenomenon during positioning of the lead deep inside the septum.

Left bundle branch pacing (LBBP) is a developing physiological pacing technique. Wu et al. proposed that direct LBB capture could be confirmed by recording retrograde His potential (PoReHis ) and mapping of the left conduction system potential. The transventricular-septal electrophysiological phenomena deserve further study. Here, we used a continuous uninterrupted pacing technique to confirm the LBB capture by dynamically observing changes in PoReHis and stimulus to left ventricular activation time.

A single premature stimulation from the para-Hisian region unmasked negative involvement of the left anterior fascicle in the verapamil-sensitive ventricular tachycardia with a right bundle branch block and right-axis deviation.

A 33-year-old man had verapamil-sensitive ventricular tachycardia (VT) with a right bundle branch block (RBBB) and right axis deviation. Programmed stimulation from the para-Hisian region induced ventricular tachycardias (VT1 or VT2). VT1 was entrained during pacing from the para-Hisian region. A single para-Hisian stimulation antidromically captured the proximal portion of the left anterior fascicle (LAF), but the cycle length of VT2 remained unchanged. This observation indicated that the upper limb of the LAF was a bystander of the reentry circuit. We have clarified this mechanism with apply

Intraventricular reentry with narrow QRS complex.

Reentry within the His-Purkinje System (V3 phenomenon) where the reentry pathway incorporates both bundle branches and the bundle of His is a frequent phenomenon during ventricular refractory period studies. In this form of reentry, both divisions of the left bundle branch function as a single limb and the reentrant beats (V3) have either left bundle branch block or right bundle branch block pattern. In this report, we describe in three patients the occurrence of a new type of reentrant beat (V3) whose characteristics suggested reentry within a circuit in which the retrograde limb of the reent…

Intrahisian functional bundle branch block.

A patient with functional complete left bundle branch block is presented. The site of block was localized to the area of the His bundle. This case demonstrates that functional complete left bundle branch block may be due to longitudinal dissociation in the His bundle.

An intriguing case of regular RR and QRS alternans during idiopathic left posterior fascicle ventricular tachycardia.

This study represents a case of idiopathic left posterior fascicle ventricular tachycardia (LPF-VT), which is intriguing due to regular alternation of short-long RR intervals and QRS morphology. Transthoracic echocardiogram analysis did not detect any structural heart disease. A baseline electrophysiological study was performed. Intracardiac recording during tachycardia showed V-A dissociation, confirming the final diagnosis of idiopathic LPF-VT. No regularity in the monomorphic VT was recorded. Previous relevant studies suggested that a single focus with two exits in distal branches of the le

Rationale and design of the randomized multicentre His Optimized Pacing Evaluated for Heart Failure (HOPE-HF) trial.

AIMS: In patients with heart failure and a pathologically prolonged PR interval, left ventricular (LV) filling can be improved by shortening atrioventricular delay using His-bundle pacing. His-bundle pacing delivers physiological ventricular activation and has been shown to improve acute haemodynamic function in this group of patients. In the HOPE-HF (His Optimized Pacing Evaluated for Heart Failure) trial, we are investigating whether these acute haemodynamic improvements translate into improvements in exercise capacity and heart failure symptoms. METHODS AND RESULTS: This multicentre, doubl

Supra-his alternate-beat Wenckebach dysrhythmia.

Alternate-beat Wenckebach, an uncommon dysrhythmia, usually is attributed to block within the His bundle or bundle branches (or in both). A patient with features of the "sick sinus" syndrome demonstrated A-V nodal (supra-His) alternate-beat Wenckebach during atrial pacing and His bundle electrocardiography. Electrophysiologic mechanisms are proposed.
