Erythropoietin in cardiovascular diseases.

Several studies showed that anaemia is commonly observed in patients with Chronic Heart Failure (CHF) and is associated with worsened symptoms and survival. When anaemia in these patients is treated with erythropoietin (EPO), a significant improvement in cardiac function and symptoms was observed. Although it was originally believed that EPO specifically acted on haematopoietical cells, recent evidence demonstrated several non-haematopoietical effects. Ischaemia/reperfusion experiments in rat heart and brain showed large infarct reduction when treated with EPO. Other effects of EPO are related

Critical aortic stenosis in the neonate. Clinical assessment and surgical outcome in 18 patients.

OBJECTIVE: To assess neonates with aortic stenosis with early decompensation operated upon. (LCO) (CHF). METHODS: A and retrospective study analyzing 6 neonates with LCO, group I (GI), and 12 neonates with CHF, group II (GII). Clinical radiographic, electrocardiographic and echocardiographic findings also provided comparative bases for the study, as did surgical and evolutional findings. RESULTS: The mean ages at hospitalization and surgery (p = 0.0031) were 14.3 and 14.8 days in GI and 35.4 and 42.8 days in GII, respectively. Cardiac murmurs were more intense in GII (p = 0.0220). The aortic

[Value of phosphodiesterase inhibitors in anesthesia and intensive care].

During anesthesia and in intensive care, pharmacological support is often required in patients with preexisting myocardial dysfunction as well as in patients with normal preoperative ventricular function. Abnormalities in both systolic and diastolic function may occur in this situation. Standard therapy used in this situation act on alpha-, beta- or dopaminergic-receptors. However, the observation of beta-receptor down-regulation phenomenon has led to the development of substances which act independently of the beta-receptor. The imidazole derivative enoximone belongs to a new class of non-cat

[Left ventricular blood filling in patients with severe mitral stenosis: comparisons before and soon after percutaneous transluminal mitral commissurotomy].

To clarify the effects of mitral obstruction on left ventricular (LV) diastolic blood filling, 15 patients with tight mitral stenosis (each mitral valve area was less than 1.5 cm2) were studied. Each selected patient underwent successful percutaneous transluminal mitral commissurotomy (PTMC), which resulted in a 1.5 fold increase in each mitral valve area. LV pressure, left atrial (LA) pressure and cardiac output were measured before and immediately after PTMC. Left ventriculography was performed before and immediately after PTMC. The ventriculogram was traced frame by frame for one cardiac cy

Experience with vascular grafts in total artificial heart replacement.

In total artificial heart replacement the pumps are attached to the vascular system with the help of connectors. These consist of a woven Dacron vessel graft to which a short silastic segment is vulcanized. In 36 calves surviving total artificial heart replacement between one and seven months (average 85 days) the morphological alterations due to interfacial reactions were studied: thrombus formation and neointimal fibrous hyperplasia at the anastomoses. In 15 calves (41.7%) thrombus growth within the outflow tract led to anastomotic stenosis: pulmonary artery 14 (93.3%), aorta-anastomosis 1 (

Mechanisms and management of heart failure in active rheumatic carditis.

Fulminating active rheumatic carditis has been observed for over three decades in this environment with no recent alteration in either the incidence or the pattern of presentation. Heart failure (in this context defined as 'an inadequate circulation at rest together with a raised pulmonary venous pressure, with or without an associated high systemic venous pressure in the absence of haemodynamically significant tricuspid valve disease or pericardial effusion') is prevalent but occurs only when a haemodynamically important left-sided valve lesion supervenes. Regurgitation is the predominant val

Atrial natriuretic factor release and natriuresis in rats with high-output heart failure.

We investigated whether rats with high-output heart failure [aortocaval (AC) shunts] release atrial natriuretic factor (ANF) and excrete sodium after moderate volume expansion (VE) as do sham-operated controls. Mean arterial blood pressure was lower (92.5 +/- 4.4 vs. 114.0 +/- 1.3 mmHg) and relative heart weight was higher (545.6 +/- 35.1 vs. 253.8 +/- 9.8 mg/100 g body wt) in animals with AC shunts than in their controls. Central venous pressure (CVP) was elevated (3.61 +/- 0.36 vs. 0.37 +/- 0.94 mmHg) and heart rate decreased (332.5 +/- 8 vs. 370.0 +/- 9.9 beats/min) in AC rats. This group a

[Pharmacokinetics of drugs in single and multiple organ failure].

Organ failure modifies the pharmacokinetics of drugs and usually requires a decrease in their dosage. A decrease in cardiac output, in protein binding and extraction ratio, hepatic and renal insufficiency, a modification in acid-base balance are the main factors regulating pharmacokinetics of drugs used in anaesthesia and intensive therapy.

Sympathetic modulation in practice: the German clinical experience.

Xamoterol is a beta 1-selective adrenoceptor partial agonist. The acute effects of xamoterol were studied in 30 patients with ischaemic heart failure. Haemodynamic data were assessed at baseline, at rest and after supine bicycle exercise (50 W for 3 min), and repeated 15 min after xamoterol (0.2 mg kg-1) given by infusion over 5 min. At rest, xamoterol increased heart rate, mean blood pressure and cardiac index, and reduced pulmonary wedge pressure. On exercise, heart rate fell while cardiac index was maintained and pulmonary wedge pressure fell slightly but significantly. There was a tendency

[Pacemaker syndrome: a report of 6 cases].

Six patients suspected to have "pacemaker syndrome" were studied with hemodynamic and electrophysiologic methods. Symptoms appeared after implantation of ventricular pacemaker (VVI). Mile symptoms such as dizziness and fatigue and severe symptom of heart failure were noted. The results of the study suggested that cardiac output decreases due to loss of atrial contribution to ventricular filling and also as a result of increase of systemic venous pressure. If the patient has V-A conduction, cardiac output will decrease further. Restoration of atrioventricular synchronization is essential in the

Repair of interrupted aortic arch and associated malformations in infancy: indications for complete or partial repair.

There is uncertainty regarding the best method of repair of interrupted aortic arch. The question is whether to perform primary definitive repair of this anomaly plus the associated defects versus arch repair only and palliation of the intracardiac defects, usually by pulmonary artery banding. Since 1976, 16 infants with interrupted aortic arch have been treated surgically. They were seen at 5.2 +/- 3 days of age and weighed 3.2 +/- 0.7 kg. The interruption occurred between the left carotid and left subclavian arteries (type B) in 9 and between the left subclavian artery and the descending aor

[Giant left atrium in mitral valve disease: a new plication procedure to relieve the compressions of left ventricular wall left bronchus and right lung].

The giant left atrium associated with mitral valve disease frequently produces postoperative hazardness in relation to hemodynamic and respiratory management. We have defined the most serious disorders induced by the presence of giant left atrium in three categories as follows. First, hemodynamic disturbance by the compression of left ventricular wall by downward extension of left atrium (type I), secondly, respiratory disturbance yielded by the compression of left main bronchus by upward extension of left atrium (type II), thirdly, compression of right middle lobe by rightward extension of le

Clinical and echocardiographic phenotype of cardiac wasting in patients with advanced cancer.

AIMS: Cardiac wasting-associated cardiomyopathy in patients with advanced cancer is characterized by loss of left ventricular (LV) mass and independently associated with poor prognosis. Better understanding of this very prevalent cardiomyopathy is urgently needed. METHODS AND RESULTS: Overall, 398 patients with active, mostly advanced cancer without significant cardiovascular disease (mean LV ejection fraction [LVEF] 64.3 ± 0.2%) or active infection were prospectively examined (mean age 60 ± 1 years, 50% women, body mass index 25.0 ± 0.2 kg/m2, 26% cachectic). Prelabour myocardial deformation and cardiac output in fetuses that develop intrapartum compromise at term: a prospective observational study.

Objective: Redistribution of cardiac output is responsible for the "brain sparing" effect seen during periods of acute or chronic fetal stress. We investigated the relationship between prelabour cardiac function in fetuses that subsequently developed intrapartum fetal compromise (IFC). Methods: A blinded, prospective, observational, cohort study, at Mater Mother's Hospital, Brisbane, Australia. A cohort of 284 women with uncomplicated singleton pregnancies underwent ultrasound every 2 weeks from 36 weeks until delivery. Fetal cardiac output was assessed by conventional Doppler ultrasound and m

[Acute cardiovascular beriberi (shoshin-beriberi)].

Beriberi (BB), thiamine deficiency, has been described in the Asian literature in the 17th century and is characterized by peripheral neuropathy and muscle weakness, also called "dry" beriberi (BB) to differentiate it from "wet" BB, with essentially cardiovascular manifestations. Wet can be either "classic" wet BB in which signs and symptoms of right-sided heart failure with normal or high cardiac output are the presenting features or the "shoshin" BB variant with severe biventricular failure and metabolic acidosis, which must be treated early to prevent the rapid development of low cardiac ou

Neurohormonal activation in congestive heart failure and the role of vasopressin.

Vasoactive neurohormonal systems (eg, sympathetic nervous system [SNS], renin-angiotensin-aldosterone system, and arginine vasopressin [AVP]) are defense mechanisms designed to preserve arterial volume and circulatory homeostasis during periods of low cardiac output. Neurohormonal systems, which are normally stimulated under conditions of acute volume depletion, are activated by the low cardiac output and arterial pressure. However, sustained and chronic activation of these systems, as occurs in congestive heart failure (CHF), can cause progressive ventricular remodeling and worsening heart fa
