OBJECTIVE: This study investigates the impact of isometric contraction of anterior cervical muscles on cervical lordosis.
METHODS: 29 volunteers were randomly assigned to an anterior head translation (n=15) or anterior head flexion (n=14) group. Resting neutral lateral cervical x-rays were compared to x-rays of sustained isometric contraction of the anterior cervical muscles producing anterior head translation or anterior head flexion.
RESULTS: Paired sample t-tests indicate no significant difference between pre and post anterior head translation or anterior head flexion. Analysis of varianc

Congenital hypoplasia of the posterior arch of the atlas (C1), a developmental failure of chondrogenesis, is a rare anomaly and may range from partial clefts to total agenesis of the posterior arch. Ossification of the posterior arch usually occurs between the 3rd and 5th years of life. The incidence of posterior arch anomalies of the atlas is between 0.69% and 2.95%. For the evaluation of the patient, cervical lateral plain radiography, 2D or 3D reconstructed CT and MRI are very useful and important tools in initial diagnosis. Surgery is the treatment of choice in symptomatic compression. Exc

Approximately 20% percent of the patients with rheumatoid arthritis show pathology in the cervical spine. The translational instability between axis and atlas might be painful and leads in the long term to myelopathic changes due to chronic traumatization of the myelon. Ongoing osseous resorption of the lateral masses of the atlas cause upward migration of the dens into the foramen magnum. In the subaxial cervical spine, the inflammatory process causes instability and deformity. Neck pain is the most common indication for surgery, but neurological symptoms with myelopathy or radicular deficits

Concomitant traumatic injuries in the upper cervical spine are often encountered and rarely reported. We examined the data concerning 784 patients with cervical spine injuries following trauma, including 116 patients with upper cervical spine injuries. Twenty-six percent of patients with upper cervical spine injuries (31 cases) were found to have combined injuries involving either the upper or the upper and lower cervical spine. The frequent patterns were combined type I bipedicular fracture of the axis and dens fracture, and combined dens fracture and fracture of the posterior arch of C 1. Ot

A 24-year-old female presented with basilar invagination and kyphoscoliosis of the cervical spine associated with a large intercarotid paraganglioma. She had suffered from pharyngeal discomfort from the age of 9 years due to the tumor. The tumor had originated from the right carotid body and extended in the parapharyngeal space compressing the upper cervical spine. Presumably the slowly growing tumor had caused the kyphoscoliosis and disturbed osseous development of the occipito-atlanto-axial complex, resulting in anterior basilar invagination, hypoplasia of the clivus, and aplasia of the post

The atlas is subject to fracture under axial load, often due to traumatic injuries such as shallow dives and automobile accidents. These fractures account for 2-13 % of injuries to the cervical spine [Marcon RM et al. Clinics (Sao Paulo) 68(11):1455-61, 2013]. Fractures of the C1 vertebra are often difficult to diagnose, as there is often no neurological deficit or easily identifiable findings on radiographs. However, injuries to the atlas can be associated with vertebral artery injury and atlantoaxial or atlanto-occipital instability, making prompt and accurate diagnosis imperative. A detaile

BACKGROUND: Atlas fractures account for as much as 13% of cervical fractures, yet their epidemiology and its implications remain under-examined.
METHODS: We retrospectively analyzed 97 consecutive cases of isolated, traumatic atlas fractures at our institution over a 17-year period with respect to demographic, clinical, and outcomes data. Unique patient subsets were identified and compared across these parameters.
RESULTS: The age of atlas fracture patients showed a bimodal distribution and strong goodness of fit, with one mean centered at an age of 30 years for patients age< 50 (R=0.9409) a

STUDY DESIGN: Two cases of occipital pain caused by an osteoid osteoma of the atlas are presented.
OBJECTIVES: To describe the management of occipital pain in two young patients.
SUMMARY OF BACKGROUND DATA: Osteoid osteoma is a benign lesion mostly affecting the long bones. A spinal location is uncommon. To the authors' knowledge, there are only five other reports of an osteoid osteoma located in the atlas.
METHODS: Occipital headache, which was relieved by salicylates, was the major symptom reported by the two adolescents. In the first patient, a lesion of C1 was seen on plain radiographs.

Craniocervical injuries (CCJs) account for 10% to 30% of all cervical spine trauma. An increasing number of patients are surviving these injuries due to advancements in automobile technology, resuscitation techniques, and diagnostic modalities. The leading injury mechanisms are motor vehicle crashes, falls from height, and sports-related events. Current treatment with urgent rigid posterior fixation of the occiput to the cervical spine has resulted in a substantial reduction in management delays expedites treatment of CCJ injuries. Within CCJ injuries, there is a spectrum of instability, rangi

Synovial chondromatosis is an uncommon disorder characterized by the formation of multiple cartilaginous nodules within the synovium, most commonly affecting large joints. Its involvement with the spine is rare; only six cases have been reported. The authors describe two patients with synovial chondromatosis involving the cervical spine. In the first case, synovial chondromatosis arose from the left C1-2 facet joint. This patient underwent a two-stage procedure including a posterior approach for tumor resection and occipitocervical fusion as well as a transmandibular circumglossal approach to

STUDY DESIGN: A series of 10 patients surgically treated with prevascular or retrovascular extraoral retropharyngeal approach to the upper cervical spine is examined.
OBJECTIVES: In orthopedic surgery, retropharyngeal approach may replace the transoral surgery, obviating the risks of infection and the uncomfortable postoperative course of cases in which median labiomandibular glossotomy was used to accomplish complex bone reconstruction.
SUMMARY OF THE BACKGROUND DATA: The transoral approach is reported in literature as the classical anterior access to the upper cervical spine that provides

A patient with an inverterate transpedicular fracture of atlas and C2/C3 subluxation, with stenosis of spinal canal by bone fragment, was operated using anterior retropharyngeal approach. Stabilization with interbody cage was performed. During follow up examination both clinical and radiological result was good. This surgical approach seems to be a comfortable approach to the upper cervical spine.

OBJECTIVES: The biomechanical effects of radiographically guided atlas-axis correction at the craniocervical junction remain insufficiently quantified, particularly at the tissue level. Therefore, this study aims to investigate the relationship between rotational alignment correction and cervicomedullary stress-strain response using a finite-element approach.
METHODS: A total of four retrospective clinical cases with pre- and post-correction radiographs were analyzed. Radiographic parameters, including axial spinous process rotation, atlas frontal plane, and cervical spine angle, were used as

A number of anterior approaches to the craniocervical junction have been described to allow exposure to the midline and lateral aspects of both the cranial base and upper cervical spine. The transoral-transpharyngeal approach, a technique that is well known to many spine surgeons, provides surgical access to the anterior clivus, C1, and C2. Transoral approaches provide the fundamental anatomy and technique upon which the more complex jaw-splitting approaches are based. This article discusses fundamental concepts regarding anatomy, perioperative considerations, and technical aspects critical to
