Diagnostic Dilemmas and The Benefit of Robotic-Assisted Surgery in the Management of Neurogenic Thoracic Outlet Syndrome in Complex Patients.

A 64-year-old male patient presented with progressive, debilitating bilateral arm pain for 2 years following traumatic cervical spine injuries requiring multiple operations. Despite extensive diagnostic workup by several specialists, he was left without a diagnosis, treatment plan, or ability to perform his job. He was diagnosed with bilateral neurogenic thoracic outlet syndrome, based on clinical diagnostic criteria. A 2-staged bilateral robotic first rib resection resulted in complete resolution of his symptoms.

Patellar allografts in anterior cervical fusion - a two-year clinical and radiographic study.

Donor site morbidity, which occurs in 15% to 20% with the use of autografts for anterior cervical fusion, is eliminated with the use of allografts. While allografts from the iliac crest, ribs, fibula, femoral head and skull have been used in anterior cervical fusion, the use of patellar allografts has not been previously reported. Twenty-two patients underwent Cloward anterior cervical decompression and fusion using bicortical patellar allografts from 1993 to 1997. Fifteen patients, with a follow-up period of at least two years, were reviewed. Eleven patients (73.4%) had good or excellent resu

[Forensic characteristics of injuries in cyclists in collisions with other vehicles].

In order to identify the features of the injuries formation in cyclists who were injured in a collision of moving vehicles with a blow to the rear wheel area of a bicycle moving at a slower speed in the same direction, the authors analyzed the results of forensic medical examinations in relation to 151 corpses of cyclists who died in road accidents aged 4 to 74 years. It was found that with this type of bicycle injury, the structures of the head, chest, lower extremities, as well as the structures of the abdomen were most often injured. Injuries to the structure of the head were characterized

Synostosis of the first and second ribs in six horses.

CASE DESCRIPTION Over a 2-year period, 6 horses (4 Selle Français, 1 Hanoverian, and 1 Thoroughbred) were referred for evaluation of forelimb lameness. All horses had radiographic evidence of synostosis of the first and second ribs (SFSR). CLINICAL FINDINGS For 1 horse, the SFSR was considered the probable cause of the lameness (grade 3/5), with a shortening of the cranial phase of the stride in the affected limb. For 3 horses, it was considered a possible cause of the lameness (grade 1/5) for the same reason. For 2 horses, SFSR was considered an incidental finding unassociated with any clinic

Extreme selection in humans against homeotic transformations of cervical vertebrae.

Why do all mammals, except for sloths and manatees, have exactly seven cervical vertebrae? In other vertebrates and other regions, the vertebral number varies considerably. We investigated whether natural selection constrains the number of cervical vertebrae in humans. To this end, we determined the incidence of cervical ribs and other homeotic vertebral changes in radiographs of deceased human fetuses and infants, and analyzed several existing datasets on the incidence in infants and adults. Our data show that homeotic transformations that change the number of cervical vertebrae are extremely…

Anterior transcervical-thoracic approach for radical resection of lung tumors invading the thoracic inlet.

We describe an original anterior transcervical-thoracic approach required for a safe exposure and radical resection of non-small-cell lung cancer that has invaded the cervical structures of the thoracic inlet. Through a large L-shaped anterior cervical incision, after the removal of the internal half of the clavicle, the following steps may be performed: (1) dissection or resection of the subclavian vein; (2) section of the anterior scalenus muscle and resection of the cervical portion of the phrenic nerve, if invaded; (3) exposure of the subclavian and vertebral arteries; (4) dissection of th

[Variation of the transverse apophysis of the 7th cervical vertebra: anatomo-radiological study of an isolated population].

The authors investigated the morphological changes in the transverse apophysis of C VII by Rx study on an isolated population. Two variations were mainly observed: the transverse mega-apophysis and the cervical rib. The authors found a remarkable incidence of variations, more than the average of the literature. Authors suppose that this results can be attributed at the geographical and economical isolation of the population and they hypothesize a genetic transmission of recessive autosomal type.

The supraclavicular approach to decompression of the thoracic outlet.

Surgical decompression of the thoracic outlet, along with treatment of the involved nerve or vessel, is the accepted treatment modality when indicated. Although neurogenic thoracic outlet syndrome (TOS) is often operated via the axillary approach and venous TOS via the paraclavicular approach, arterial TOS is almost always operated via the supraclavicular approach. The supraclavicular approach provides excellent access to the artery, brachial plexus, phrenic nerve, and the cervical and/or first ribs, along with any bony or fibrous or muscular abnormality that may be causing compression of the

[Thoracic outlet syndrome : Interdisciplinary diagnostics and treatment].

The thoracic outlet syndrome (TOS) refers to a group of clinical conditions caused by compression of nerves and blood vessels in congenital or acquired anatomical narrowing of the upper thoracic aperture. This includes the anatomical structures, such as the scalene muscles, the first rib, a possibly present cervical rib, the costoclavicular joint or the pectoralis minor muscle. Isolated venous compression is also known as thoracic inlet syndrome (TIS). The symptoms are diverse and the path to a correct diagnosis is often prolonged. In the absence of anatomical anomalies, conservative treatment…

Isolated plasmacytoma of the sternal stalk: a case report and review of the literature

Background: Isolated plasmacytoma of bone is a malignant neoplasm of haematologic origin with a low incidence and a tendency to progress to multiple myeloma, and surgical treatment may improve patient survival. Case presentation: We review the case of a 77-year-old man with isolated plasmacytoma of the sternal stalk whose main symptom was pain in the anterior chest wall, which was severe and severely interfered with life. He was treated through extended radical resection of the sternal stalk tumour, including bilateral partial rib cartilage resection, bilateral clavicle head resection, cervica

Diaphragm pacing failure secondary to deteriorated chest wall mechanics: When a good diaphragm does not suffice to take a good breath in.

Diaphragm pacing allows certain quadriplegic patients to be weaned from mechanical ventilation. Pacing failure can result from device dysfunction, neurotransmission failure, or degraded lung mechanics (such as atelectasis). We report two cases where progressive pacing failure was attributed to deteriorated chest wall mechanics. The first patient suffered from cervical spinal cord injury at age 45, was implanted with a phrenic stimulator (intrathoracic), successfully weaned from ventilation, and permanently paced for 7 years. Pacing effectiveness then slowly declined, finally attributed to rib

Multiple pathological bone fractures in a dog with adrenal-dependent Cushing's syndrome.

Pathological bone fractures may occur secondarily to Cushing's syndrome in people. Although low bone mineral density due to cortisol excess has been described in dogs, there is no evidence that dogs with Cushing's disease can develop osteoporosis or secondary bone fractures. Here, we report a case of a dog with Cushing's disease and multiple pathological bone fractures. A 14-year-old female spayed mixed-breed dog was attended due to progressive lethargy, weakness, difficulty moving, trembling and clinical signs of Cushing's syndrome (polyuria, polydipsia, alopecia and abdominal distension). A Previously Unreported Variant of a Thoracic Vertebra.

Pathology associated with anomalous transverse processes is uncommon and usually involves elongated C7 transverse processes in the so-called cervical rib syndrome. We report a single adult thoracic vertebra found to have duplicated transverse processes on the left side. We believe this to be the first report of a duplicated transverse process in man. The clinician and anatomist who view osteological material or imaging should consider this an extremely rare anatomical variation of the thoracic spine.

Contralateral brachial plexus neuropathy after arthroscopic shoulder surgery.

This case report presents a neurologic complication that occurred after shoulder arthroscopic surgery on the contralateral arm. This brachial plexus palsy has been documented as a reversible C7-T1 lesion, which was the consequence of the patient's unknown cervical rib and the applied lateral position during the operation. In this case, the recovery was almost complete within 3 months postoperatively; however, one should take care to prevent these complications with an appropriate positioning of patients with an anatomic variant.
