Bridging the gaps: An early integrated support collaborative for at risk mothers in rural Maine.

BACKGROUND: The antecedents that contribute to health disparities in maternal child health populations begin before birth and extend into the early prenatal and gestational growth periods. Mothers and infants living in rural poverty in particular are at considerable risk for problems associated with reproductive health, including pregnancy complications and premature births. OBJECTIVE: The aim of this manuscript is thus two-fold, to describe the epidemiologic makeup of the community and the intervention model of the Community Caring Collaborative. METHODS: Innovative models of early-integrat

Association of copper-zinc superoxide dismutase (SOD1) and manganese superoxide dismutase (SOD2) genes with nonsyndromic myelomeningocele.

BACKGROUND: A common and severe neural tube defect (NTD) phenotype, myelomeningocele (MM), results from the defective closure of the caudal end of the neural tube with herniation of the spinal cord and meninges through the vertebral column. The exact mechanisms for NTDs are unknown, but excessive oxidative stress, particularly in association with maternal diabetes, has been postulated as a mechanism for MM. METHODS: The SNPlex Genotyping (ABI, Foster City, CA) platform was used to investigate single nucleotide polymorphisms (SNPs) across the superoxide dismutase (SOD) 1 and 2 genes to assess

Teaching Equity to Prevent Violence: The ANULA School-SAFE Framework Across Continents.

BACKGROUND: Violence against women and girls (VAWG) remains a global public health crisis, driven by structural inequalities and harmful gender norms. Schools reaching nearly every child offer unique opportunities to prevent violence, yet existing interventions are fragmented and unsustainable. We propose the ANULA SCHOOL-SAFE framework, a ten-step, culturally adaptable model that embeds prevention into education systems through four integrated pillars: Safeguard, Act, Foster, and Empower. Grounded in ecological systems, social norms, and resilience theories, the framework combines age-appropr

Accuracy of the Diagnostic Infant and Preschool Assessment (DIPA) in a Dutch sample.

BACKGROUND: To prevent negative effects of early-onset psychiatric disorders on children's development, structured diagnostics are needed. However, validated diagnostic instruments (based on DSM-5) for children aged 7 years and younger are scarce. The Diagnostic Infant and Preschool Assessment (DIPA) is a diagnostic interview developed in the USA for measuring 16 psychiatric disorders in young children. The psychometric properties of the American version of the DIPA have been validated. Here we determined the accuracy of the psychometric properties of the Dutch DSM-5 based version of the DIPA

Placement Instability Among Young People Removed from Their Original Family and the Likely Mental Health Implications.

BACKGROUND: Young people in out-of-home care are more likely to experience poorer mental and physical health outcomes related to their peers. Stable care environments are essential for ameliorating impacts of disruptive early childhood experiences, including exposure to psychological trauma, abuse and neglect. At present there are very few high quality data regarding the placement stability history of young people in out-of-home care in Australia or other countries. OBJECTIVES: To undertake the first systematic census of background, care type and placement stability characteristics of young p

The evaluation and care of children with suspected fetal alcohol spectrum disorders in the pediatric medical home: The importance of therapeutic alliance, longitudinal surveillance and trauma-informed care.

The evaluation and management of children with prenatal alcohol exposure and suspected fetal alcohol spectrum disorders (FASD) requires a multidisciplinary approach, grounded in the pediatric medical home. This process begins with the primary care provider who is often the initial point of contact for children presenting with developmental and behavioral concerns. An evaluation for a FASD requires the primary care provider to consider the effects of biological risk factors (e.g. genetic and mental health risks, maternal pregnancy risks, and history or prenatal substance exposures), and the rol

Babble Boot Camp for Infants With Down Syndrome: Piloting a Proactive, Caregiver-Led Intervention Designed to Boost Earliest Speech and Language Skills.

PURPOSE: Down syndrome (DS) is associated with lifelong difficulties with verbal communication, beginning in infancy when vocalizations are sparse and first words emerge late. Because DS is diagnosed at or even before birth, these difficulties can be anticipated, yet there have been limited developments of systematic, proactive interventions. The purpose of the pilot study described here was to investigate feasibility and potential benefits of such an intervention toward a fully powered clinical trial. METHOD: We piloted Babble Boot Camp (BBC), a proactive, parent-led speech and language inte

Characterization of sleep difficulties in maltreated children and adolescents.

Numerous studies have identified connections between child maltreatment and sleep-related issues. However, poor is known on potential links between sleep patterns and day-to-day functioning, along with psychopathology in maltreated youths. Additionally, existing research on the relationship between sleep difficulties and maltreatment often lacks investigation into specific sleep difficulty profiles across different age ranges. The current study aimed to determine the prevalence of diverse sleep disturbance profiles in a sample of maltreated children and adolescents, exploring distinct sleep di

Comparing mental health disorders among sex trafficked children and three groups of youth at high-risk for trafficking: A dual retrospective cohort and scoping review.

BACKGROUND: Individuals at high-risk for trafficking are often subject to preexisting complex trauma that only intensifies during the trafficking experience. This greatly increases their risk of mental illness, although the actual prevalence of mental health disorders in children who are sex trafficked remains unclear. OBJECTIVE: To examine the prevalence of mental health diagnoses among a sample of youth identified as being sex trafficked, and to discuss these rates in relation to other high-risk groups reported in the literature. PARTICIPANTS AND SETTING: 143 female and male child traffick

The effectiveness of Parenthood-Centred Psychotherapy on perinatal maternal depressive-anxiety symptoms and child development in a disadvantaged population.

INTRODUCTION: Untreated depressive-anxiety symptoms during pregnancy can lead to recurrent or chronic maternal depression, with negative consequences for child development and the mother-infant bond. The hypothesis is that Parenthood-Centred Psychotherapy reduces depressive-anxiety symptoms and promotes the mother-infant bond in a disadvantaged population, with high rates of these symptoms. Methods. This study was conducted between 2015 and 2023 in a Primary Health Care Centre in Barcelona in 160 women with prepartum depressive-anxiety symptoms, focusing on outcomes for the mother, the child a

Fertility and family-building experiences and perspectives of males with cystic fibrosis.

BACKGROUND: Nearly all males with cystic fibrosis (MwCF) are infertile and, thus, require the use of assisted reproductive technology (ART) to have biologic children. This study aims to describe the fertility and family-building knowledge, experiences, and care utilization of this population and to compare these findings to the general United States (US) population. METHODS: We conducted an anonymous cross-sectional study of self-reported survey data compared to data from the 2017-2019 US National Survey for Family Growth (NSFG). We recruited MwCF age 15 years and older at seven US cystic fib

Child Abuse, Misdiagnosed by an Expertise Center-Part II-Misuse of Bayes' Theorem.

A newborn girl had, from two weeks on, small bruises on varying body locations, but not on her chest. Her Armenian grandmother easily bruised, too. Her mother was diagnosed with hypermobility-type Ehlers-Danlos-Syndrome (hEDS), an autosomal dominant connective tissue disorder, with a 50% inheritance probability. Referral to a University Medical Center located "Dutch Expertise Center for Child Abuse" resulted (prior to consultation) in physical abuse suspicion. Protocol-based skeletal X-rays showed three healed, asymptomatic rib fractures. A protocol-based Bayesian likelihood ratio guesstimatio

Effectiveness of a prenatal behavior change communication intervention on maternal-fetal bonding: A randomized controlled trial in a tertiary care setting.

PROBLEM: Maternal-fetal attachment (MFA) plays a critical role in determining maternal health and influencing child developmental outcomes. However, structured behavioral interventions designed to enhance MFA are seldom included in antenatal care practices. BACKGROUND: MFA forms the foundation of the maternal-child bond. This early relationship is critical for maternal mental health and serves as a precursor to the child's socio-emotional and behavioral development. AIM: This study aimed to determine whether a behavioral communication intervention could enhance maternal-fetal attachment. ME

What Constitutes High-Quality Paediatric Palliative Care? A Qualitative Exploration of the Perspectives of Children, Young People, and Parents.

BACKGROUND: Globally, over 21 million children need palliative care each year. Although guidelines exist to support paediatric palliative care delivery, they are not informed by the experiences of children themselves. OBJECTIVE: We aimed to determine what constitutes good quality palliative care from the perspectives of children with life-limiting or life-threatening conditions and their parents. METHODS: We analysed semi-structured qualitative interviews using reflexive thematic analysis informed by the European Association for Palliative Care charter of palliative care for children and you

Engaging From Both Sides: Facilitating a Canadian Two-Generation Prenatal-to-Three Program for Families Experiencing Vulnerability.

BACKGROUND: Young children living in families experiencing social vulnerability, including low income, mental illness, addictions, social isolation, and/or homelessness, are at risk of developmental delay. Two-generation programs can improve outcomes for preschool children, but underlying mechanisms and outcomes for younger children remain unclear. PURPOSE: We explored program facilitation and identified developmental benefits of a two-generation program beginning prenatally. METHODS: In our convergent, concurrent mixed methods study, we interviewed agency staff (n = 10) and held focus group

Protective vs. punitive informal social control: A multilevel mediation study of neighborhood effects on child sexual abuse in Nepal.

BACKGROUND: While neighborhood collective efficacy is theorized to promote community intervention against child maltreatment, the mechanisms of informal social control (ISC) through protective versus punitive pathways remain poorly understood. OBJECTIVES: This study aims to examine whether the association between collective-efficacy-based neighborhood ISC and contact child sexual abuse (CSA) is differentially mediated by protective versus punitive child-maltreatment-specific ISC (ISC_CM) in Nepal.
