Acute rheumatic fever and erythema marginatum in an adult patient.

A 41-year-old woman presented for evaluation of a pruritic eruption with an abrupt onset, starting on her flanks and then spreading to her arms and legs. She had 2 weeks of fever, chills, malaise, migratory joint pain, nausea, and mental confusion. An antistreptolysin O titer was positive. Upon hospital admission, bilateral lower-extremity chorea movements were observed, and her C-reactive protein level was elevated (3.7 mg/dL). Biopsy results supported erythema marginatum. Based on these clinical and laboratory findings, the diagnosis of acute rheumatic fever was established.

Pseudomonas stutzeri prosthetic valve endocarditis: A case report and review of the literature.

We report a case of Pseudomonas stutzeri endocarditis in Lebanon. The patient had a recent history of prosthetic aortic valve replacement and presented to the emergency department with fever and chills. Transesophageal echocardiography confirmed the presence of a vegetation on the prosthetic valve and blood cultures yielded P. stutzeri. The patient was treated with surgery and antibiotics but deteriorated and passed away four days after admission. To our knowledge, this is the fifth case of P. stutzeri endocarditis reported in the literature, and the first case with early presentation and mort…

[Brain abscesses caused by Nocardia farcinica in a kidney transplanted man].

Nocardiosis is primarily an opportunistic infection caused by aerobic Gram-positive bacteria of the genus Nocardia. In this case report we describe a male patient who previously received a kidney transplant and was admitted to hospital with chills, headache and pain in the neck and left eye, tremor and coordination problems. A magnetic resonance scan of the brain showed multiple abscesses, and blood culture was positive for Nocardia farcinica. Nocardiosis of the central nervous system is a rare, but serious differential diagnosis in immunosuppressed patients with signs of high intracranial pre…

[Strangulation of the penis with a rubber band : a case report].

We report a case of strangulation of the penis with a rubber band. A 68-year-old man visited our hospital for treatment of a markedly swollen penis. Physical examination revealed one rubber band tightly coiled around the penis. The rubeer band was soon removed, gangrene of the penis continued. A high fever, chills and hypotension were recognized. Sepsis was presumed, and administration of antibiotics was started. Microbiologic cultures revealed streptococcus agalactiae in blood. Since his general condition did not improve, we performed partial penectomy. After the surgery, he showed complete r…

A case report of Citrobacter koseri bacteraemia after transfusion of contaminated red cells.

BACKGROUND: Citrobacter koseri has not been associated to infection caused by blood component transfusions. CASE REPORT: A 46-year-old female patient presented sudden chills, dyspnoea, and tachycardia during a transfusion from a unit of packed red blood cells. Citrobacter koseri was isolated in the two sample collected from the patient and from the content of the packed red blood cells. RESULTS: We report a case of bacteraemia due to a transfusion of contaminated red blood cells. CONCLUSION: C. koseri should be taken into account in bacteraemias caused by transfusion of blood components.

Ehrlichiosis in Kentucky.

Ehrlichiosis is a recently described tick-borne disease characterized by headache, fever and chills, leukopenia, thrombocytopenia, and transaminase elevation. The presentation is similar to Rocky Mountain spotted fever but the rash is less frequently present in ehrlichiosis. The diagnosis is confirmed by elevated antibody titers to Ehrlichia chaffeensis or visualization of inclusions in white blood cells and the disease is easily cured with tetracyclines or chloramphenicol. Cases of ehrlichiosis are reported from southeastern, southcentral and midatlantic states. We report three cases of Ehrli…

Uveitis, a presenting symptom of malignant histiocytosis.

A patient presenting initially with bilateral uveitis was admitted 6 weeks later to the Department of Internal Medicine "A" because of fever, chills, and pancytopenia. Physical examination, ultrasonography and CT scan revealed hepatosplenomegaly only, without lymphadenopathy. A bone marrow trephine biopsy showed areas with a dense infiltrate of abnormal cells, displaying many mitotic figures. Some of the cells resembled better differentiated histiocytes and contained hemosiderin pigment or phagocytosed erythrocytes. Immunohistological tests confirmed the diagnosis of malignant histiocytosis (M…

Spontaneous rupture of the spleen in initial presentation of Hodgkin's disease.

A 46-year-old man presented with a four-week history of fevers, occasional chills, and a two-week history of sweats and poor appetite. He also complained of progressive weakness and lethargy. After initial evaluation, while awaiting further consultation, the patient developed rapidly progressing abdominal pain and light-headedness. He was moved immediately into the emergency treatment area. He was noted to have an acute abdomen and was taken to surgery. An enlarged Hodgkin's-infiltrated spleen with an actively bleeding hematoma was removed. The patient denied any history of trauma.

Fever in systemic lupus erythematosus.

The frequency, causes, clinical and laboratory features, and outcome of febrile episodes in 160 hospitalized patients with systemic lupus erythematosus were reviewed. Eighty-three febrile episodes were identified in 63 patients and were ascribed to active lupus erythematosus alone (60 per cent), infections (23 per cent) and miscellaneous causes (17 per cent). Bacteremia was present in nine of the 19 infectious episodes and resulted in a fatal outcome in a third of the patients. Leukocytosis, neutrophilia, shaking chills and normal levels of anti-DNA antibodies were associated with infection in…

Human disease caused by Brucella canis. A clinical and epidemiologic study of two cases.

Nonlaboratory-associated human illness caused by Brucella canis occurred in two persons. Fever, chills, malaise, and weight loss were prominent symptoms in both patients. Epidemiologic investigation identified a household dog as the likely source of one patient's infection, and serologic studies and blood cultures on neighborhood dogs showed that approximately 30% were positive for B canis. Persons exposed to infected dogs appeared to have a low risk of disease. Although routine Brucella agglutinin tests do not detect antibodies to B canis, available methods for diagnosis include a specific B …

Human Case of Leptospirosis During a Canine Disease Outbreak - Wyoming, 2023.

Leptospirosis is a zoonotic bacterial disease spread through the urine of infected animals; the typical incubation period is 5-14 days. In approximately 90% of human cases, illness is asymptomatic or mild, characterized by fever, chills, myalgia, nausea, vomiting, diarrhea, headache, calf pain, and conjunctival suffusion, but severe illness can progress to multiorgan dysfunction and death. Although Wyoming is considered a low-risk area for leptospirosis because of its cold and semiarid climate, the Wyoming Department of Health was notified of a probable human case in August 2023, the first rep…
