Prognosis for clinical early-stage lung cancer patients with subclinical malignant pleural effusion: Is anatomical resection a reasonable option?

OBJECTIVE: Non-small cell lung cancer with malignant pleural effusion is generally considered inoperable. The aim of this study was to investigate the prognosis and outcomes of patients with subclinical malignant pleural effusion who underwent surgical resection for lung cancer. METHODS: Between 2013 and 2022, 2261 patients underwent lung resections for non-small cell lung cancer. We retrospectively reviewed the records of 29 patients with cT1-2N0 lung cancer and subclinical malignant pleural effusion < 10 mL. RESULTS: The surgical procedures included wedge resection (n = 8) and anatomical r

Surgical Aortic Valve Replacement in Patients Above the Guideline-endorsed Age Cut-off: Reasons for Surgery and Clinical Outcomes.

OBJECTIVES: The 2017 and 2021 ESC/EACTS guidelines for the management of valvular heart disease recommend transcatheter aortic valve implantation (TAVI) as a treatment option for severe symptomatic aortic valve stenosis (AS) in patients ≥75 years of age. However, surgical aortic valve replacement (SAVR) remains a viable option for elderly patients, particularly in specific anatomical or clinical subsets. The objective of this study was to analyze indications for SAVR and postoperative outcomes in patients ≥75 years of age. METHODS: Heart team protocols were reviewed to determine indications f

Reasons for successful clinical outcome following pulmonary vein isolation despite lack of persistent LA-PV conduction block.

The mechanisms of atrial fibrillation (AF) induction and maintenance, including those involved in paroxysmal atrial fibrillation, are not completely known; this limits our ablation strategies and prevents us from understanding what we are actually doing when performing pulmonary vein isolation. In this report, we focus on the commonly used ablation strategies for AF and question the importance of complete pulmonary vein isolation in achieving lasting success in the ablation of AF. We also discuss in detail the absence of durable pulmonary vein isolation in patients without arrhythmic recurrenc

Process-Oriented, Behaviorally Anchored Assessment of Clinical Reasoning in Large Language Models and the Effect of Extended Thinking: Protocol for a Prospective, Multigroup, Comparative Study.

BACKGROUND: Most clinical reasoning evaluations in large language models (LLMs) score only the final answer, usually multiple-choice accuracy, which explains little about model reasoning. Two developments stress this gap: reasoning-optimized models are now common, and several expose an explicit extended thinking control. Whether that deliberation improves the reasoning process remains untested with a validated instrument. OBJECTIVE: This protocol introduces and aims to validate the Rapid Evaluation Assessment of Clinical Reasoning Tool (REACT)-AI, a Behaviorally Anchored Rating Scale (BARS) w…

Comparison of immersive virtual reality and video-based home visit simulations for developing nursing students' perceived clinical reasoning, learning satisfaction, and engagement: a randomized controlled trial.

BACKGROUND: Home visits require students to integrate clinical reasoning with environmental assessments; however, most virtual reality (VR) studies focus on hospital settings. No randomized controlled trial has also compared immersive VR with conventional video-based training in home visit contexts. Therefore, this study aimed to evaluate whether immersive VR-based home visit training improves students’ perceived clinical reasoning skills and learning experiences compared with video-based training. METHODS: A parallel-group randomized controlled trial was conducted among third-year nursing stu…

Effectiveness of gamified interventions on clinical reasoning-related cognitive skills, knowledge, teamwork skills, and problem-solving skills among nursing students: A systematic review and meta-analysis.

BACKGROUND: Existing systematic reviews and meta-analyses have reported inconsistent findings regarding the effects of gamified interventions on clinical reasoning-related cognitive skills in nursing education. In addition, prior meta-analyses were based on a limited number of studies with small, pooled sample sizes, highlighting the need for a more focused and comprehensive synthesis of evidence on this outcome. OBJECTIVES: To systematically evaluate the effects of gamified interventions on clinical reasoning-related cognitive skills, knowledge, teamwork skills, and problem-solving skills in…

The association between attitude, perceived norm, and perceived behavioral control with the provision of Clinical Work-Integrating Care: A reasoned action approach.

BACKGROUND: Clinical Work-Integrating Care (CWIC) brings important attention to issues emerging from the interrelationship between health and work. Yet, for various reasons, CWIC is not routinely delivered in clinical healthcare. This study focuses on why medical specialists do or do not provide CWIC, applying a reasoned action approach. OBJECTIVE: To examine the associations between attitude, perceived norm, and perceived behavioral control with the provision of CWIC. METHODS: A cross-sectional survey was distributed to Dutch medical specialists. Multivariable regression analysis was used t

Length of stay after lumbar decompressive surgery: prediction model, clinical reasons for prolonged stay, and association of prolonged stay with outcomes.

PURPOSE: Information is limited on prediction models for length of stay (LOS), observed clinical reasons for prolonged LOS, and the association of prolonged LOS with outcome metrics after elective lumbar decompressive surgery. METHODS: We analysed prospectively collected data on 3132 cases with or without instrumented fusion. A prediction model for LOS in hospital was developed with 2008-2022 data and validated on 2023 data. Validation data was used to evaluate LOS model R2 and area under the curve (AUC) to discriminate to prolonged LOS. Observed reasons for prolonged LOS were categorised. Pr

CT-integrated pathophysiology-guided, clinically causal cognition-led interpretable lesion context characterization and risk reasoning in lung MWA.

BACKGROUND: Accurate preoperative prediction of complication risks in microwave ablation (MWA) for non-small cell lung cancer (NSCLC) is critical yet challenging due to the complex anatomical context. Beyond feature heatmaps, clinicians need decision rationales that align with pathophysiology. We therefore frame the task as an interpretable lesion-context characterization driven by clinical causal cognition, where imaging-derived concepts (e.g., tumor-pleura distance, vascular proximity, pleural adhesions) are organized into traceable causal pathways supporting downstream risk reasoning.
