Introduction. Closed reduction is a critical component of the intramedullary nailing and at times can be difficult and technically challenging resulting in increased operative time. Fluoroscopy is used extensively to achieve closed reduction which increases the intra-operative radiation exposure. Materials and Methods. Sixty patients with femoral diaphyseal fractures treated by locked intramedullary nailing were randomized in two groups. In group I, fracture reduction was performed under fluoroscopy with a cannulated reamer in the proximal fragment or with simultaneous use of a cannulated ream

Management of mandibular condylar and subcondylar fractures is heavily debated due to the proximity of important structures and complication risks. This study reviews the outcomes of 4 different treatment approaches: conservative, closed reduction with maxillomandibular fixation, open reduction and internal fixation, and open reduction and internal fixation with maxillomandibular fixation. A retrospective chart review of all patients with a facial fracture seen at a level one trauma center between 2012 and 2022 was performed. Demographics, fracture etiology, fracture type, treating team (plast

The present retrospective study aims to evaluate the outcome in 41 patients of femoral shaft fractures, who had closed intramedullary nailing in lateral decubitus position without fracture table or image intensifier. Mean age was 33.2 (range, 18-70) years. The cannulated reamer in proximal fragment (as intramedullary joystick) and Schanz screw in the distal fragment (as percutaneous joystick) were simultaneously used to assist closed reduction of the fracture without the use of image intensifier. Closed reduction was successful in 38 patients. Open reduction was required in 3 patients. Schanz

We report a unique case of post-traumatic avascular necrosis of tibia in a 12-year-old boy following a Salter-Harris type II physeal injury of the proximal tibia. The diagnosis was made on the basis of classical radiological findings. The only clinical symptom was diffuse shin pain. The patient was managed conservatively. At our 2-year follow-up, there was clinical and radiological evidence of spontaneous resolution of avascular necrosis, but with sequelae. The child developed a limb-length discrepancy and genu valgum. The parents of the child refused further treatment.

BACKGROUND AND STUDY AIMS: Elastic stable intramedullary nailing (ESIN) enables stable osteosynthesis with a low rate of complications in pediatric both-bone forearm fractures. However, repeated unsuccessful attempts at closed reduction can cause unnecessary soft tissue trauma and increased radiation exposure. We conducted a retrospective study of pediatric forearm fracture patients to identify predictors for the need for open reduction when using ESIN.
PATIENTS AND METHODS: We included 65 pediatric patients who underwent ESIN osteosynthesis for both-bone forearm fractures. We analyzed gender

We have reviewed 12 cases of fracture-separation of the distal humeral epiphysis, three of which were initially misdiagnosed as fractures of the lateral condyle and one as an elbow dislocation. Cubitus varus deformity is as common after this fracture-separation as it is following supracondylar fracture, and is most common in children under two years of age. Closed reduction and simple immobilisation is adequate for the older child, but we recommend for those under two years of age that closed reduction should be followed by percutaneous pinning, so that the carrying angle can be assessed immed

The juvenile fracture of Tillaux is an ankle joint avulsion fracture of the anterior distal tibial tubercle in adolescents, produced by external rotation force applied to the foot. Data from five patients with the juvenile fracture of Tillaux were reviewed. In four of the patients, an initial closed reduction was unsuccessful and had to be followed by open reduction and internal fixation of the displaced fragment. The fifth patient's treatment was nonoperative. All patients had an excellent functional and radiographic result at follow-up evaluation one to nine years after the injury. Results w

CASE: A 9-year-old boy with a widely displaced Type-III supracondylar humerus fracture presented with a painful mass at the antecubital fossa after pin fixation. Exploration of the mass caused bleeding from a brachial artery pseudoaneurysm that necessitated repair with a saphenous vein graft. Six weeks later, the fracture had healed without complications.
CONCLUSION: We describe an unusual presentation of a pediatric supracondylar humerus fracture that highlights the importance of including vascular injury in the differential diagnosis for similar presentations.

BACKGROUND: Femoral neck fractures (FNFs) in young adults are usually caused by high-energy trauma, and their treatment remains a challenging issue for orthopedic surgeons. The quality of reduction is considered an important factor in improving the poor prognosis of patients with FNFs. In recent years, positive buttress closed reduction technique has received widespread attention in the treatment of FNFs. This comprehensive literature review is designed to encapsulate the impacts of both non-anatomic and anatomic reduction on the biomechanical stability, clinical outcomes, and postoperative co

The goal of this study was to evaluate the role of conservative treatment of distal radius fractures in the elderly since 2000. Our series included 22 articles, of which 3 reviews, 2 meta-analyses, 6 descriptive series of conservative treatment, 10 series comparing surgical and conservative treatments and 1 series comparing different types of conservative treatments. The reduction was performed by closed reduction in 7 cases and by traction in 3 cases. A short-arm cast was used in 8 cases and a long-arm cast in 4 cases for an average length of 6 weeks. The indications of conservative treatment

T-type intercondylar fractures of the distal humerus are rare injuries in skeletally immature patients. Few studies are available to guide the treatment of these injuries. Small case series and expert recommendations suggest superiority of the open approach for direct reduction of the articular surface. This approach poses risks of vascular compromise and elbow stiffness. However, success with percutaneous intercondylar screw fixation has been described for simple split patterns. The authors present a case that illustrates a novel percutaneous technique to treat this fracture type. A healthy 8

INTRODUCTION: Perilunate dislocations represent one of the most devastating injuries to the carpus. Fortunately, these injuries are relatively rare, constituting approximately 10% of all carpal injuries. One of the problems associated with this injury is the difficulty of its accurate and early recognition.
PRESENTATION OF CASE: In this study, an uncommon case of bilateral dorsal trans-scaphoid perilunate fracture-dislocation following trauma has been reported. The injury was missed initially and the patient was subsequently operated after two weeks. Anatomic reduction was achieved by closed r

RATIONALE: Pipkin III femoral head fracture dislocation (FHFD) is rarely observed in clinical practice, and its outcome is alarming. A considerable proportion of Pipkin III fractures result from repeated or forceful closed reduction of an irreducible FHFD. Pipkin type III fractures pose a therapeutic challenge. Most patients underwent total hip arthroplasty.
PATIENT CONCERNS: A 34-year-old man experienced high-energy trauma to the left hip from a terrible traffic accident. Radiography and computed tomography (CT) at the local hospital revealed a left posterior FHFD. Emergency close reduction

Alveolar fractures are a common type of maxillofacial trauma, and the conventional treatment involves closed reduction and dental splinting fixation. However, closed treatment is not suitable for some complex segmental alveolar fractures. In this case report, we introduce an innovative method for segmental alveolar fracture by using open reduction and internal fixation by minimally invasive approach combined with computer-assisted surgery. In this case, the new dimensions in the treatment followed AO principles of fracture management, achieving anatomical reduction of the fracture, absolute st

INTRODUCTION: This is a report of 2 consecutive cases of the Galeazzi-equivalent fracture in children.
CLINICAL PICTURE: In both cases, the mechanism of injury was falls onto the outstretched hand. Radiographs showed fracture of the radius with separation of the distal ulnar physis.
TREATMENT: Initial treatment by closed manipulation failed; hence, open reduction and internal fixation was required. The cause of failure of closed manipulation in both cases was interposition of soft tissue into the ulnar fracture site.
OUTCOME: Both cases showed good functional outcome after open reduction an

A palmar lunate transtriquetral fracture dislocation with a concomitant radial styloid avulsion fracture has not been described before in the literature. This injury represents an interesting variation of stage IV perilunar instability. Treatment was complicated by persistent scapholunate dissociation (rotary subluxation of the scaphoid) after attempted closed reduction and percutaneous pinning. At open reduction, the proximal half of the triquetrum, which had been dislocated palmarward with the lunate and which had been thought to be reduced after our attempted closed reduction was indeed ret
