Early results of the Ponseti method for the treatment of clubfoot associated with myelomeningocele.

BACKGROUND: Myelomeningocele is a common birth defect that is often accompanied by clubfoot deformity. Treatment of clubfoot associated with myelomeningocele traditionally has consisted of extensive soft-tissue release operations, which are associated with many complications. The purpose of the present study was to evaluate the early results of the Ponseti method for the treatment of clubfoot associated with myelomeningocele. METHODS: Sixteen consecutive patients with myelomeningocele (twenty-eight clubfeet) and twenty consecutive patients with idiopathic clubfeet (thirty-five clubfeet) were …

The drop toe sign: an indicator of neurologic impairment in congenital clubfoot.

UNLABELLED: Nine patients presenting during infancy were identified with clubfeet and absent anterior and lateral compartment functions. We … a plantigrade foot. Idiopathic absent peroneal nerve function is not a well-described entity in the clubfoot literature. All babies with clubfoot should be examined for the drop toe sign. When noted, the feet will likely be more difficult to correct initially, may need early Achilles tendon lengthening, will likely need permanent bracing, are likely to rel …

Magnetic resonance angiography in clubfoot and vertical talus: a feasibility study.

UNLABELLED: Congenital vascular alterations of the normal adult arterial pattern have been associated with multiple congenital limb deformities including clubfoot and vertical talus. Investigators have observed absence of the anterior tibial artery and dorsalis pedis artery in most patients with clubfoot, and absence of the posterior tibial artery in all patients with vertical talus. We used magnetic resonance angiography to define the lower extremity vascular anatomy of two patients with left-sided vertical talus and right-sided clubfoot and one patient with bilateral vertical talus and carti…

Early results of the Ponseti method for the treatment of clubfoot in distal arthrogryposis.

BACKGROUND: Clubfoot occurs in approximately one in 1000 live births and is one of the most common congenital birth defects. Although there have been several reports of successful treatment of idiopathic clubfoot with the Ponseti method, the use of this method for the treatment of other forms of clubfoot has not been reported. The purpose of the present study was to evaluate the early results of the Ponseti method when used for the treatment of clubfoot associated with distal arthrogryposis. METHODS: Twelve consecutive infants (twenty-four feet) with clubfoot deformity associated with distal …

Comparative results of the conservative treatment in clubfoot by two different protocols.

The current study aims at presenting the results of the two methods of conservative treatment in clubfoot: the Romanian traditional method and the Ponseti method. The study population included 103 children (148 clubfeet) treated in our department between 1998 and 2005. Between 1998 and 2003, the conservative treatment protocol was based upon the Romanian method. The Ponseti method has been used since 2004. The main criterion for the assessment of the efficiency of the two conservative methods in clubfoot is the number of feet requiring surgical treatment - posteromedial release at 18 months. T…

Congenital diastasis of the inferior tibiofibular joint with clubfoot and imperforate anus.

Congenital diastasis of the inferior tibiofibular joint is a rare condition of unknown etiology, first described in 1972. Although its clinical presentation at birth is a talipes equinovarus deformity of the foot, it may be associated with limb length discrepancy and other skeletal and visceral abnormalities. We present one such case of congenital inferior tibiofibular diastasis of Onimus type A variety (vertical diastasis) with talipes equinovarus, high imperforate anus, short first ray of the foot, intact superior tibiofibular joint, and no limb length discrepancy. Closed manipulation and ca

Absence of HOXD10 mutations in idiopathic clubfoot and sporadic vertical talus.

The genetic etiology of idiopathic clubfoot is unknown. There have been cases reported in which both clubfoot and vertical talus appears in the same family; therefore, the genes responsible for vertical talus are reasonable candidates for idiopathic clubfoot. A mutation in HOXD10 was previously identified in a family with isolated congenital vertical talus. To determine whether HOXD10 is involved in the etiology of idiopathic clubfoot, HOXD10 coding and 5' and 3' untranslated regions were resequenced in 190 patients (177 with clubfoot, 10 with sporadic vertical talus, and 3 with both clubfoot …

Accessory soleus muscle in clubfoot deformity: a report in four feet.

The commonest presentation of accessory soleus muscle is a swelling at the posteromedial aspect of the ankle in adolescents or young adults. Accessory soleus is rarely encountered in children undergoing surgical release for congenital clubfoot, and only a few isolated reports are available in the literature. The purpose of this study is to heighten awareness about the role of accessory soleus muscle in clubfoot deformity. Four cases of accessory soleus muscle in patients undergoing surgical release for clubfoot deformity are reported here in which, a distinct anomalous muscle deep to the tendo…

The role of radiographic measurements in the evaluation of congenital clubfoot surgical results.

A series of 50 patients with 71 clubfeet treated surgically was evaluated after exclusion of individuals with associated malformations, syndromes, neurological disorders, previous surgery and age over 30 months at the time of the surgical procedure. They underwent soft-tissue releases that addressed all the components of the deformity, including, when necessary, the calcaneocuboid joint. The age of the patients at the time of the surgery ranged from 6 to 26 months and the mean follow-up period was 77.03 months. The scores resulting from the application of the functional rating system designed

Validity and responsiveness of the Clubfoot Assessment Protocol (CAP). A methodological study.

BACKGROUND: The Clubfoot Assessment Protocol (CAP) is a multi dimensional instrument designed for longitudinal follow up of the clubfoot deformity during growth. Item reliability has shown to be sufficient. In this article the CAP's validity and responsiveness is studied using the Dimeglio classification scoring as a gold standard. METHODS: Thirty-two children with 45 congenital clubfeet were assessed prospectively and consecutively at ages of new-born, one, two, four months and two years of age. For convergent/divergent construct validity the Spearman's correlation coefficients were calculat…

Assessment of talus deformity by three-dimensional MRI in congenital clubfoot.

PURPOSE: To evaluate the morphological deformity of talus in congenital clubfoot by three-dimensional MRI. MATERIAL AND METHOD: Subjects were five patients (two male, three female, mean age 5 months) with unilateral congenital clubfoot. Magnetic resonance imaging was performed of both feet using 1.5 T magnet. Based on the resulting magnetic resonance imaging volume data, a three-dimensional surface bone model was reconstructed by the Marching Cubes method. The long axis of the reconstructed model was determined, and in relation to the standard planes including this axis, the degree of talar h…

Correction of relapsed or neglected clubfoot using a simple Ilizarov frame.

We present the results of using a simple Ilizarov fixator frame in treatment of 66 feet in 52 patients (mean age 8.5 years) of 58 relapsed and eight neglected clubfeet with grade III or IV severity with a mean follow-up of 40 (26-58) months. Our frame, in spite of being simple and easy for surgeons and patients to handle, achieved satisfactory correction comparable to the literature.

Correction of severe residual clubfoot deformity in adolescents with the Ilizarov technique.

Correction of severe residual clubfoot deformities in adolescents is a difficult undertaking and historically has had mediocre success without an accompanying arthrodesis procedure. Soft tissue release alone has yielded extremely high recurrence rates. Additional osteotomies have been used for correction of single deformities in one plane. The use of Ilizarov external fixation techniques has allowed for improvement of correction, stabilization, and decrease in recurrence rates. The technique, however, is difficult and should be performed by surgeons who are familiar with correction of pediatri…

Validity of the anteroposterior talocalcaneal angle to assess congenital clubfoot correction.

OBJECTIVE: The anteroposterior talocalcaneal angle (Kite's angle) is still considered a common parameter for assessing clubfoot correction, although some dissenting opinions about its accuracy have been expressed. The purpose of this study was to evaluate the validity of the anteroposterior talocalcaneal angle for assessing correction of congenital clubfoot in adults who received the treatment as children. SUBJECTS AND METHODS. The anteroposterior talocalcaneal angle was measured in 48 treated idiopathic congenital clubfeet and 28 normal feet of 38 patients at the end of skeletal growth using …

Evaluation of calcaneal malposition by magnetic resonance imaging in the infantile clubfoot.

Magnetic resonance imaging (MRI) can clearly visualize both the ossification centers and surrounding cartilage of growing bones and enable a three-dimensional analysis of their positional relationships by using overlapping images. The calcaneal position was evaluated based on five newly proposed parameters of magnetic resonance images in 30 clubfeet of 18 infants with the bimalleolar and lower leg axes as reference lines. Sixteen unaffected feet in the unilateral cases were analyzed as normal feet. MRI showed that the calcaneus in clubfoot rotated internally, medially and posteriorly with refe…

[Pathologic anatomy of congenital clubfoot--review of the literature and personal observations].

The authors present a review of literature of the past 40 years, relevant to the pathological anatomy of congenital clubfoot and their own views--based on intra-operative observations. Performing Goldner's procedure the authors noted the talar bone was rotated internally, as a whole and externally in relation to the forefoot. The head and neck of the talar bone were deviated medially in relation to the trochlear axis. These deformities lead to pronation and supination movements instead of the normal flexion and extension of the foot. The authors believe a wide capsulotomy with resection of the…

[Posteromedial release combined with peritalar release in the treatment of congenital clubfoot].

Basing on the authors' own experiences an attempt to assess the value of posteromedial release in treatment of congenital clubfoot was made. The procedure was performed in 70 feet in 52 children, age ranging from 6 months to 12 years, 90% of the procedures were performed before 4 years of age. Feet were classified as follows: a. non-coerrective--type III according to Marciniak, b. Partially corrective, c. in older children (> 4 years of age) in whom posteromedial release was combined with a lateral resection of the cuboid bone. Late results after 5-20 years (13 years on average) were assessed …

Confirmation of arterial deficiencies in a limb with necrosis following clubfoot surgery.

This study describes postoperative necrosis of the hallux and first ray in a child with clubfoot. Arteriography performed on this child's lower limbs demonstrated, in the operated leg, hypoplasia of both the anterior and posterior tibial arteries and failure of the dorsalis pedis artery to traverse the tarsus and complete the deep plantar arch. Previously, congenital vascular deficiency was suggested to predispose such operated limbs to necrosis. These findings confirm the association between vascular deficiency and necrosis. In this present study, the metabolic demands of wound healing were s…

Practical applications in idiopathic clubfoot: a retrospective multicentric study in EPOS.

We report a European study of idiopathic clubfoot that was compiled with the aid of a detailed questionnaire. The retrospective study was presented at the seventh meeting of the European Paediatric Orthopaedic Society (EPOS). Most EPOS members treat idiopathic clubfoot at birth with plaster cast, but a few use physiotherapy and splints. The results of this conservative treatment vary. Operation is usually indicated at an age ranging from 4 to 15 months. Some physicians perform extensive procedures, and others limit operation to the medial and posterior parts of the foot. We propose a classific…

[Comparative evaluation of the methods of surgical treatment of congenital clubfoot in children].

The observation and treatment of 142 children have shown that most favorable results were obtained in operations on soft tissues of the foot not involving the integrity of tendons of the peroneal group of muscles. The method developed by the authors is the most physiological one and contributes to the quickest recovery of functions of the foot and talocrural articulation.
