Impact of Intratympanic Gentamicin on Cochlear Implant Outcomes.

INTRODUCTION: Intratympanic gentamicin (ITG) instillation is a therapeutic option in advanced Menière's disease (MD) to control vertigo. ITG aims to induce vestibular hypofunction by selective ablation of vestibular hair cells. However, it may also adversely affect cochlear hair cells and the neural auditory pathway. Preservation of the neural auditory pathway is critical, as today cochlear implantation (CI) is the gold standard for auditory rehabilitation in patients with MD. This study assesses the impact of prior ITG treatment on CI outcomes in patients with MD. METHODS: This retrospective…

[Structure and establishment of the German Cochlear Implant Registry].

Hearing rehabilitation of patients with severe hearing loss or deafness using cochlear implants (CI) is a very successful but also complex and lifelong process that requires high quality standards for structure, process, and results. Medical registries represent an ideal tool for conducting quality control relevant to care while at the same time collecting scientific data. Therefore, at the initiative of the Executive Committee of the German Society of Oto-Rhino-Laryngology, Head and Neck Surgery (DGHNO-KHC), a Germany-wide CI registry (the German Cochlear Implant Register, DCIR) was to be est

Modulation Depth Discrimination by Cochlear Implant Users.

Cochlear implants (CIs) convey the amplitude envelope of speech by modulating high-rate pulse trains. However, not all of the envelope may be necessary to perceive amplitude modulations (AMs); the effective envelope depth may be limited by forward and backward masking from the envelope peaks. Three experiments used modulated pulse trains to measure which portions of the envelope can be effectively processed by CI users as a function of AM frequency. Experiment 1 used a three-interval forced-choice task to test the ability of CI users to discriminate less-modulated pulse trains from a fully mod

Reading and Theory of Mind in Adolescents with Cochlear Implant.

Previous research has shown a possible link between reading comprehension and theory of mind (ToM), but these findings are unclear in adolescents with cochlear implants (CI). In the present study, reading comprehension and ToM were assessed in adolescents with CI and the relation between both skills was also studied. Two sessions were performed on two groups of adolescents aged between 12 and 16 years of age (36 adolescents with CI and 54 participants with typical hearing, TH). They were evaluated by means of a standardized reading battery, a false belief task, and Faux Pas stories. The result

Influence of bilateral cochlear implants on vocal control.

Receiving a cochlear implant (CI) can improve fundamental frequency (F0) control for deaf individuals, resulting in increased vocal pitch control. However, it is unclear whether using bilateral CIs, which often result in mismatched pitch perception between ears, will counter this benefit. To investigate this, 23 bilateral CI users were asked to produce a sustained vocalization using one CI, the other CI, both CIs, or neither. Additionally, a set of eight normal hearing participants completed the sustained vocalization task as a control group. The results indicated that F0 control is worse with

Shedding light on optical cochlear implant progress.

Electrical cochlear implants (CI) currently lack the frequency and intensity resolution to allow detection of complex sounds in background noise. The use of microscale optoelectronics in conjunction with optogenetics provides a promising direction in CI technology to allow improvements in spectral resolution, providing a richer soundscape for users. The present work offers the first instance of using multi-channel μLED-based optical CI to demonstrate optogenetic activation of auditory neurons.

Hidden Electrode Failure in a Cochlear Implant User.

When the impedance of an electrode contact is highly increased in a cochlear implant, a failure of the appropriate electrode seems obvious. We present a case where impedances of some electrodes were at the lower edge of the normal range and not regarded as suspicious neither by the clinical fitting software nor by in-vivo tests conducted by the implant manufacturer. However, speech comprehension was substantially degraded and sound perception distorted. Also, on the affected electrodes, loudness perception was compromised and responses of the electrically evoked compound action potential were
