Behavioral treatment of essential hypertension: a comparison between cognitive therapy and biofeedback of heart rate.

The efficacy and the mechanisms of action of two behavioral treatments for essential hypertension were compared: cognitive group therapy for anger control and biofeedback for heart rate control. The cognitive therapy aimed at lowering the "general anger" level and helping overt expression of "anger out;" heart rate biofeedback aimed at slowing heart rate in stress situations. Ninety-seven essential hypertensive patients were randomly assigned to three groups; after 20 dropouts, 77 patients fully participated in the study: cognitive treatment (N = 30), biofeedback (N = 27), and control (no trea

Disproportionality of Depressive Reactions and the Process of Change in Cognitive Behavioral Therapy for Depression.

BACKGROUND: The proportionality of depressive reactions to stress was long considered an essential feature of clinical depression. Although judgments of the proportionality of depressive reactions have been effectively removed in the Diagnostic and Statistical Manual of Mental Disorders (5th ed.), important empirical questions remain about the information that such judgments might convey. METHODS: In Study One, we tested the factor structure of a brief self-report measure of disproportionality, the Proportionality of Depressive Reactions-Self-Report (PDR-S) and evaluated its relationship with

LGBTQIA+ sexual health: a SITCC clinical framework for integrating cognitive-behavioral therapy and sexual medicine.

INTRODUCTION: Sexual and gender diverse (LGBTQIA+) individuals experience significant sexual health disparities shaped by minority stress processes, trauma exposure, mental health burden, and historically heteronormative and cisnormative clinical frameworks. Although international guidelines, standards of care, and professional guidance documents increasingly endorse affirmative and evidence-informed care, their integration into routine sexual medicine and psychotherapeutic practice remains inconsistent, particularly in the assessment and clinical management of sexual dysfunctions in LGBTQIA+

Increasing engagement with cognitive-behavioral therapy (CBT) using generative AI: a randomized controlled trial (RCT).

BACKGROUND: Shortages in mental healthcare lead to long periods of inadequate support for many patients. While digital interventions offer a scalable solution to this unmet clinical need, patient engagement remains a key challenge. Generative artificial intelligence (genAI) presents an opportunity to deliver highly engaging, personalized mental health treatment at scale. METHODS: In a pre-registered (ClinicalTrials.gov: NCT06459128, 10 June 2024), parallel, 2-arm, unblinded, randomized controlled trial (N = 540), we evaluate whether a genAI-enabled cognitive behavioral therapy (CBT) app enhan…

The efficacy of cognitive behavioral therapy for persecutory delusions: A systematic review and meta-analysis.

BACKGROUND: Persecutory delusions, prevalent in disorders like schizophrenia, significantly impair daily functioning and cause distress. Although cognitive behavioral therapy (CBT) has shown promise in treating delusions, no meta-analysis has specifically evaluated its effectiveness for persecutory delusions. OBJECTIVES: To identify studies on CBT specifically designed for patients with persecutory delusions and to evaluate its impact on delusion-related symptoms, including the conviction of persecutory delusions, paranoia severity, and psychological well-being. METHODS: A comprehensive sear…

Brief cognitive behavioral therapy for chronic pain rapidly improves pain interference: a randomized controlled trial.

Abbreviated psychotherapies for chronic pain improve access to care by reducing length of treatment and reaching patients outside of chronic pain clinics, such as primary care. However, the evidence for these approaches is limited. We conducted a randomized controlled trial of brief cognitive behavioral therapy for chronic pain (ie, 6, 30-minute sessions of psychoeducation, behavioral and cognitive skills, and relapse prevention delivered individually) with 184 primary care patients with moderate to severe chronic musculoskeletal pain from a Veterans Health Administration medical center. Mean …

Mental Health Assessment and Suitability for Cognitive-Behavioral Therapy Among Patients With Idiopathic Intracranial Hypertension.

BACKGROUND: High rates of mental health conditions have been reported among patients with idiopathic intracranial hypertension (IIH). Cognitive-behavioral therapy (CBT) has been suggested as a complementary treatment to help manage anxiety and headache pain in IIH. This study aims to assess the mental health of IIH patients through a psychiatric interview and to evaluate their suitability and interest in CBT. METHODS: Participants with IIH were recruited from Stanford Byers Eye Institute. Demographic and clinical characteristics were collected, and participants completed self-rated scales for

Virtual Reality Technology in Cognitive Behavioral Therapy Across Multiple Psychiatric Conditions: A Narrative Literature Review.

PURPOSE: Modern psychotherapy increasingly incorporates technology to support patients with mental disorders. Virtual reality (VR) enables realistic, controlled simulations, offering structured, time-limited interventions focused on present problems, which are highly valuable in clinical practice. Cognitive-behavioral therapy (CBT), one of the most widely used psychotherapeutic approaches, can be effectively enhanced by VR. This review aims to summarize the potential of VR as a support tool for CBT and to evaluate its effectiveness across selected mental disorders. METHODS: A narrative review

Effectiveness of low-intensity cognitive-behavioral therapy for obsessive-compulsive disorder in routine psychiatric practice.

BACKGROUND: Cognitive-behavioral therapy (CBT) is a first-line treatment for obsessive-compulsive disorder (OCD). Exposure and response prevention (ERP), the principal CBT technique for OCD, involves repeated exposure to anxiety-provoking stimuli and habituation of anxiety responses. Although weekly CBT sessions over 10-12 weeks are recommended, real-world implementation is often constrained; Japanese health insurance permits shorter and less frequent sessions. However, the effectiveness of such low-intensity CBT (LiCBT) remains unclear. METHODS: This exploratory study used data derived from

Disseminating cognitive-behavioral therapy for body dysmorphic disorder: Evaluating the efficacy of online provider training.

BACKGROUND: Body dysmorphic disorder (BDD) is a severe psychiatric condition affecting approximately 2% of the population, yet access to evidence-based cognitive behavioral therapy (CBT) is limited due to insufficient clinician training. Web-based training offers a solution for disseminating specialized treatment skills, but real-world effectiveness data are lacking. This study evaluated a brief, online CBT training program for BDD delivered under naturalistic conditions. METHODS: Between 2017 and 2023, 166 clinicians enrolled in a 4-week web-based training covering BDD assessment and CBT tre…

Genome-Wide Association Study of Symptom Change Following Cognitive Behavioral Therapy for Common Mental Disorders.

Cognitive behavioral therapy (CBT) is a well-established, evidence-based treatment for common mental disorders such as depression, anxiety disorders, and obsessive-compulsive disorder (OCD). However, treatment outcomes vary widely, and a substantial proportion of patients do not achieve sufficient improvement. Robust predictors of individual differences in symptom change are currently lacking. Genetic differences have been suggested to play a role, but existing evidence is inconclusive. This study investigated the extent to which common genetic variants-single nucleotide polymorphisms (SNPs)-c…

Group Vs Individual Grief-Focused Cognitive Behavioral Therapy for Older Adults: A Randomized Clinical Trial.

IMPORTANCE: Grief-focused cognitive behavioral therapies (GF-CBTs) are found effective in treating prolonged grief disorder (PGD), but the relative efficacy of different delivery formats is unknown. While evidence for individual GF-CBT (GF-CBT individual) is well established, evidence for group GF-CBT (GF-CBT group) is scarce. However, the group format holds possible advantages in a bereavement context. OBJECTIVE: To examine whether GF-CBT group is noninferior to GF-CBT individual in reducing PGD symptoms in older adults. DESIGN, SETTING, AND PARTICIPANTS: Enrollment and data collection for

Manualized versus treatment-as-usual cognitive behavioral therapy for social anxiety disorder: A comparative study.

OBJECTIVE: This study aims to compare treatment outcomes of manualized cognitive behavioral therapy (MAN-CBT) with CBT treatment-as-usual (TAU-CBT) in patients with social anxiety disorder across two separate investigations. It was examined whether treatment effects differ between conditions after the total number of sessions, and whether manualized therapies would show better outcomes with equivalent dose of sessions. METHODS: All patients underwent CBT at the same outpatient treatment center. This secondary analysis utilizes data from two randomized controlled trials for the manualized CBT …
