Colon cast passage, which is the spontaneous passage of a full-thickness, infarcted colonic segment per rectum, is a rare occurrence. Xanthogranulomatous inflammation (XGI) is a well-recognized disease process in the kidney and gallbladder. However, involvement of the colon is extremely rare. We report a patient presenting with a left psoas abscess causing necrotizing fasciitis of the upper thigh. The patient underwent exploration of the left thigh through a medial approach, confirming necrotizing fasciitis of the adductor compartment and the femoral triangle. The infective process also involved the left psoas. This was explored retroperitoneally through a left pararectal laparotomy incision. Further exploration revealed a carcinomatous ulcer of the sigmoid colon. Despite active resuscitation, antibiotic therapy, and further debridement, the patient died three days after admission.

A retrospective review of 637 consecutive colonoscopies with polypectomy in 526 patients was performed to determine the association of small polyps of the rectum and sigmoid colon with more proximal colonic neoplasms. All colonic polyps were proximal to the sigmoid colon in 117 procedures. Proximal neoplasms were found in 32 percent of patients with a single polyp in the rectum or sigmoid colon. The incidence increased to 83 percent for those with three or more polyps. The occurrence of proximal colonic neoplasms was not affected by the size or histologic type of the rectosigmoid polyps. We report a case of perforated adenocarcinoma of the sigmoid colon in an inguinal hernia presenting as Fournier's gangrene. Bowel perforation has been known to cause this disease but, to our knowledge, perforation in an inguinal hernia due to benign or malignant disease has not been reported elsewhere. Gastrointestinal tract perforation should be sought as a potential etiology in patients with necrotizing fasciitis of the genitalia. When such a perforation exists occult malignancy should be ruled out.

Malignant tumors of the gastro-intestinal tract showing both carcinomatous and sarcomatous components are rare. Very few cases were described in the stomach and only one case in the colon. The clinical course of sigmoidvesical fistula due to rupture of colonic diverticulitis is described. The patient, a 72-year-old-man, had chief complaints of cloudy urine, pneumaturia and fecaluria. The fistula was diagnosed by cystoscopy. In this case, primary resection of the sigmoid colon and wedge resection of the bladder including fistula were performed, and the sigmoidvesical fistula was completely cured. Statistical analysis was made on 43 cases of colovesical fistula due to diverticulitis in Japan including our own case.

The problems of extracolic-infiltrative lesions of the sigmoid colon and recto-sigmoid junction are discussed, based on representative case studies.
