Bridging the health gap in Uganda: the surgical role of the clinical officer.

INTRODUCTION: A scarcity of trained medical personnel impedes Uganda's ability to deliver healthcare effectively. The role of the Clinical Officer (CO) was established to assist the provision of primary healthcare to rural communities. METHODS: Twenty COs, attending a clinical course, responded to a structured questionnaire to determine their local investigative resources, the cases encountered by them in the two-week period preceding the course and the surgical and obstetric procedures that they felt able to perform. RESULTS: The majority of respondents did not have access to biochemical or

Using community maternity care assistants to facilitate family-focused breastfeeding support.

Increasing breastfeeding rates, particularly in lower socio-economic areas, would have considerable impact upon public health. Social support has been found to have direct and positive effects on breastfeeding rates, and fathers' attitudes have an important influence on the initiation and continuation of breastfeeding. In the UK, trained maternity care assistants (MCAs) are increasingly being used to support community midwives by providing post-natal women with breastfeeding support. The current study aimed to evaluate the feasibility and acceptability of MCAs involving fathers from economical

The Role of Alaska's Tribal Health Workers in Supporting Families.

Alaska's Community Health Aides/Practitioners (CHA/Ps) are often the sole medical workers in their communities in rural Alaska, and are instrumental in providing healthcare services and education to otherwise underserved individuals. This qualitative study explored how CHA/Ps support healthy families. Six CHA/Ps from two rural communities in western Alaska were interviewed about their scope of practice, interactions with mothers, infants, families, and teens, relationship to other medical providers, and perceptions of their work. Using grounded theory, verbatim notes were analyzed in Dedoose s

Community-Level Practice Checklists for Health Protection During Cold Spells in China.

Communities play a crucial role in protecting the health of vulnerable populations such as the elderly, low-income groups, and high-risk individuals during cold spells. However, current strategies for responding to cold spells primarily consist of programmatic policies that lack practicality, specificity, and detailed implementation guidelines for community workers. Therefore, this study aims to identify and analyze the challenges faced by communities in responding to cold spells, review international experiences, and develop a set of practical checklists for community-level health protection.

Impact of proactive malaria community case management (proCCM) on parasite prevalence and incidence from 2021 to 2023: a randomised controlled trial in Chadiza District, Eastern Province, Zambia.

Ensuring prompt and effective case management of malaria remains an ongoing challenge in Zambia, where care is not sought for roughly 40% of febrile children under 5 years of age. To expand access, the Ministry of Health has scaled up routine malaria community case management (mCCM) for all ages over the past decade. As of 2018, nearly a quarter of children who received antimalarials obtained them from a community health worker (CHW), but gaps in treatment seeking remain. Proactive community case management (proCCM), under which CHWs regularly visit households to screen, test and treat individ

Contribution and performance of female Community-Directed Distributors in the treatment of onchocerciasis with Ivermectin in Sub-Saharan Africa: a systematic review.

The African Program for Onchocerciasis Control (APOC) was launched in 1995 with the main goal being to boost the fight against onchocerciasis in Africa. In 2011, over 80 million people benefited from this intervention thanks to the contribution of 268.718 Community-Directed Distributors (CDD). These significant results obscure the role of women CDD in this fight. Indeed, the insufficient involvement of female CDD has been identified as a concern by the APOC partners early in the program. The present study aims to assess the contribution and performance of women involved in a strategy to contro

Tasks to Establish the Role of Hospice and Palliative Care Social Workers in Community Care.

In 2018, the South Korean government launched a community care policy aimed at providing more extensive social services for older adults, the disabled, and mental health patients. This policy also included an expansion of home-based hospice services in response to the growing demand for hospice and palliative care. Despite these initiatives, social workers play a limited role in home-based hospice care, and those specializing in hospice and palliative care are not yet fully equipped to provide effective community care services. This study investigated both domestic and international research t

Strengthening the mpox response: how do we balance pragmatism and equity in resource-constrained settings?

Mpox, previously neglected as an endemic zoonotic disease in West and Central Africa, has become a significant global threat following its second declaration as a public health emergency of international concern. With the growing number of cases, the region faces several challenges, particularly inadequate surveillance and diagnostics, and a lack of vaccine access. Drawing on lessons from public health emergency response in the Economic Community of West African States region, we aim to inspire a new vision with health emergency preparedness and response framework transformed for implementing

The anthrax vaccine and research: reactions from postal workers and public health professionals.

During the 2001 anthrax attacks, public health agencies faced operational and communication decisions about the use of antibiotic prophylaxis and the anthrax vaccine with affected groups, including postal workers. This communication occurred within an evolving situation with incomplete and uncertain data. Guidelines for prophylactic antibiotics changed several times, contributing to confusion and mistrust. At the end of 60 days of taking antibiotics, people were offered an additional 40 days' supply of antibiotics, with or without the anthrax vaccine, the former constituting an investigational

Acceptability and impact of group interpersonal therapy (IPT-G) on Kenyan adolescent mothers living with human immunodeficiency virus (HIV): a qualitative analysis.

BACKGROUND: Task shifting is a well-tested implementation strategy within low- and middle-income countries that addresses the shortage of trained mental health personnel. Task shifting can increase access to care for patients with mental illnesses. In Kenya, community health workers (CHWs) are a combination of community health assistants and community health volunteers and have played a crucial role on this front. In our study, we seek to assess the acceptability and feasibility of Group Interpersonal Psychotherapy (IPT-G) delivered by CHWs among depressed postpartum adolescents (PPAs) living …
