Untangling letter confusability and word length effects in pure alexia.

Pure alexia is an acquired neuropsychological disorder that follows damage to the occipito-temporal lobe. This brain damage results in a severe reading impairment in which previously literate individuals are no longer able to efficiently read words, but are still able to perform other language tasks. The present study sought to identify factors of words that make it more difficult for pure alexic individuals to read, such as letter confusability and word length. Eye-tracking methodology was paired with a naming task to examine whether word length or letter confusability is a better predictor o

The treatment of actinomycosis mimicking a retained root tip: a confusing case.

Actinomycosis is an infection caused by the actinomyces genus and is associated with trauma or previous infection. A 58-year-old male patient was referred from a private dental clinic for root extraction of the lower right molar. The x-ray showed fractured root-like material distal to the distal root of the lower right second molar. A biopsy during extraction of the root-like material was performed, which revealed a sequestrum with actinomycosis by a pathological examination. In this case, the radiopacity of the suspicious lesion was higher than that of the surrounding alveolar bone, which con

Risks in situations that are experienced as unfamiliar and confusing - the perspective of persons with dementia.

An increasing number of people with dementia are ageing at home in Sweden and in other countries. In order to meet the safety requirements, knowledge about how persons with dementia experience risks is required. The aim of the study was to explore and better understand how persons with dementia, living at home, experience risks in their daily life and how they handle these situations. Twelve persons with dementia were interviewed using open-ended questions, and the data were analyzed using a content analysis approach. Findings showed that participants experienced situations fraught with risks

Mirror-image confusions: Implications for representation and processing of object orientation.

Perceiving the orientation of objects is important for interacting with the world, yet little is known about the mental representation or processing of object orientation information. The tendency of humans and other species to confuse mirror images provides a potential clue. However, the appropriate characterization of this phenomenon is not entirely clear, in part because the stimuli used in most previous studies were not adequate for distinguishing various forms of mirror-image and non-mirror-image error. In the present study we explore the nature of mirror-image confusion and what the phen…

Whipple disease: unusual presentation of a protean and sometimes confusing disease.

OBJECTIVES: To describe an unusual case of Whipple disease (WD) with confusing clinical features at onset and to discuss the diagnostic challenges for the clinician. METHODS: Description of a new case of this rare disease and thorough discussion of the atypical clinical manifestations at onset. A literature review, concerning the unusual onset, by means of a MEDLINE search from 1966 to 2007 was done. RESULTS: A 39-year-old man with sudden bilateral blurred vision due to retinal vasculitis and concomitant rapidly evolving symmetrical neurosensory bilateral hearing loss as initial features of

[Doubts and confusions in the diagnosis and treatment of positional plagiocephaly. Management protocol].

INTRODUCTION: Positional plagiocephaly is currently the most frequent cause of consultation at pediatric neurosurgical departments in Spain and other western countries. There is considerable confusion in the literature on the terminology and physiopathology of this deformity, as well as its differential diagnosis with true synostosis and treatment recommendations. OBJECTIVES: To clarify these concepts and present a protocol for the management of positional plagiocephaly, which was recently requested by the Health Administration of the Community of Madrid. PROTOCOL: The protocol aims to achie…

Paroxysmal eyelid movements: a confusing feature of generalized photosensitive epilepsy.

BACKGROUND: Persistent, frequent, nonepileptic paroxysmal eyelid movements were observed in 19 children and adults with well-controlled gene … e generalized spike-wave discharges on EEG. Paroxysmal eyelid movements were a source of diagnostic confusion, but direct examination and video during EEG recording distinguished the attacks from absence seizures. In all cases, the epilepsy is completely or nearly completely controlled with AEDs, but the paroxysmal eyelid movements have not resolved with ag …

Emanuel Miller lecture: confusions and controversies about Asperger syndrome.

BACKGROUND: Hans Asperger drew attention to individuals who show the core symptoms of autism in the presence of high verbal intelligence. METHODS: A review of the literature explores current issues concerning the diagnosis and nature of Asperger syndrome. RESULTS: The behavioural and neurophysiological evidence to date suggests that Asperger syndrome is a variant of autism typically occurring in high-functioning individuals, and not a separate disorder. One of the problems of diagnosis is that the typical impairment of social communication may be difficult to identify in early childhood, and

[Clinical value of intraductal ultrasonography for clarification of confusing ERCP results].

OBJECTIVE: This study assesses the value of intraductal ultrasound (IDUS) when the findings of endoscopic retrograde cholangio-pancreatography (ERCP) are unclear. PATIENTS AND METHODS: IDUS was performed over a two-year period in cases of bile duct or pancreatic duct stenosis of unknown origin found during ERCP or if cholelithiasis was suspected but ERC was seemingly normal. Duct stenoses were classified, before and after IDUS, as benign or malignant ductal or malignant extraductal, the findings then being checked by clinical follow-up,histology or, if bile duct stones had been suspected, by i

CNS listeriosis confused with leptomeningeal carcinomatosis in a patient with a malignant insulinoma.

We describe a case of presumed listeria monocytogenes rhomboencephalitis, which was initially confused with leptomeningeal carcinomatosis in a patient with a malignant carcinoid tumor. Long-term corticosteroid treatment and immunosuppression caused by malignancy predisposed the patient to developing listeriosis. The clinical and radiologic features of this illustrative case are described. Listeriosis is an important treatable differential diagnosis in patients with malignancy presenting with neurologic signs.

Assessment of delirium in advanced cancer: the use of the bedside confusion scale.

We conducted a prospective assessment of 50 consecutive admissions to an acute palliative medicine unit, using the bedside confusion scale (BSCS). Information including age, gender, diagnosis, and the presence or absence of brain metastasis was collected. Possible predisposing factors for delirium were recorded. Forty-one of 50 consecutive admissions were screened. There were 18 men and 23 women with a median age of 65 years (average: 60-75). The most common diagnoses among all were lung and breast cancer. Thirteen patients were delirious (BSCS score of > or = 2), 10 borderline (BSCS score = 1…

[Confused dream-like state and hypophyseal apoplexy: neurovascular hypothesis].

INTRODUCTION: Pituitary apoplexy has been defined classically as a nosologic syndrome of predominant headache, visual deficits and decreased level of consciousness, but there are not any psychopathological associations described in the scientific literature directly related to it. The disorder of sensorium or arousal is a variable sign, with a rank of features from moderated lethargy to coma and stupor, revealing a torpid evolution. CLINICAL CASE: We present a case with behavioral changes, mental dissociation and temporal illusions. In turn, infections or endocrinologic morbidity are rejected

[Pericarditis and pulmonary embolism. A difficult differential diagnosis and a confusing association].

The association of pericarditis and pulmonary embolism may be the source of diagnostic error and delay in the administration of anticoagulant therapy. Two cases are reported. Pericarditis occurred late in patients with severe, chronic pulmonary embolism with electrocardiographic changes of acute cor pulmonale. Two physiopathological mechanisms for this association have been proposed. The first, haemodynamic, suggests friction between the pericardium and distended right ventricle and pulmonary artery. The second, an immunological hypothesis, compares the association of pericarditis and pulmonar

Dextran 40-induced coagulopathy confused with von Willebrand disease.

In a patient who had undergone reconstructive surgery on the subclavian artery and was treated postoperatively with aspirin and dextran 40, a bleeding diathesis developed within 24 hours. This was at first thought to be von Willebrand disease, since the bleeding time was longer than ten minutes; the factor VIII level, 28%; and the activated partial thromboplastin time, 50 seconds (normal 30 to 38). The patient's defect responded to discontinuance of the low-molecular-weight, dextran and aspirin therapy and administration of a cryoprecipitate. Later studies of the coagulation mechanism up to tw
