A patient with focal convulsions of sudden onset had cerebrospinal pleocytosis and electroencephalographic evidence of a focal lesion in the right hemisphere. Except for fever, there was no clinical evidence of a viral infection. Measles antibodies were detected in the serum and CSF. The CSF abnormalities resolved, and the patient has been well for 4 years. This case is unusual because of the focal CNS involvement and the lack of any other clinical features of measles infection.

We studied 158 children with culture-proven shigellosis, 37 (23.4%) of whom had convulsions. Historical, clinical, and laboratory data were compared between patients with and without convulsions to define risk factors for the development of seizures. Age was the most important predisposing factor. The highest incidence of shigellosis associated with convulsions was found in children between 6 months and 4 years of age. Peak body temperature and a family history of convulsions also independently affected the development of seizures. Sex and Shigella strain showed differences between the groups

In 23 children with febrile convulsions the concentration of gamma-aminobutyric acid (GABA) in lumbar cerebrospinal fluid (CSF) was measured by a radioreceptor assay. The mean CSF GABA concentration of 134 (range, 73-294) pmoles/ml was significantly lower than that of 16 seizure-free children serving as controls, who had 210 (range, 117-475) pmoles/ml. The reduction in CSF GABA levels in patients with febrile convulsions was not reflected in plasma GABA concentrations. These data provide further evidence that impairment of GABA neurotransmission may contribute to an increased seizure propensit

Hydroxykynurenine/hydroxyanthranilic acid ratios were measured in children with febrile convulsions, afebrile fits, and fever, as well as in healthy controls. Increased ratios were found not only in the children who had fits but also in the children who were febrile and did not have fits. It is suggested that a raised hydroxykynurenine/hydroxyanthranilic acid ratio does not necessarily indicate vitamin B6 deficiency but may represent a nonspecific response of tryptophan metabolism to stress.

The participation of genetic factors in the origin of convulsive disorders is a controversial matter. In an attempt to study the influence of heredity in a selected group of patients, we evaluated 128 subjects that presented febrile febrile convulsions. Of these, 18 (14,3% of the sample) had at least one relative affected by the same problem. Even though a more definite conclusion depends on sample enlargement and on setting up a control group, pedigree observation suggests the participation of an autosomal dominant gene, with incomplete and probably low penetrance, in the etiology of febrile

Thirty children with simple febrile convulsions were treated with sodium valproate following their second convulsion. Twenty-two of the 30 (73%) had no further convulsions during the one-year period of observation compared with 17 of 28 in the control group (61%). This was not a statistically significant difference. Side effects attributed to sodium valproate treatment were noted in 7 patients (23%), although 4 of these showed only mild transient gastrointestinal symptoms at high dosage. The study did not confirm any advantage in the use of sodium valproate as a prophylaxis for febrile convuls

To find out about the medical and parental management of children having their first febrile convulsion a hospital-based questionnaire study was carried out in which parents were asked what they did at the time. Fifty-four out of 89 parents brought their child directly to hospital while the remainder attempted to contact their general practitioner, usually successfully, and were then referred to hospital. Whichever course of action the parents chose the outcome was satisfactory. Eighty-seven per cent of convulsions lasted for less than 15 minutes and in only two instances did they last longer

The abnormal theta-rhythm of the EEG can be identified by use of spectral analysis. The rhythms appear as peaks in the power spectrum and can be considered as given, if the intensity of this peak is significantly higher than the average intensity and if a left-right coherence is present. Abnormal theta-rhythms in the parietal region are significantly more frequent in children with a histroy of febrile convulsions than in controls. This difference becomes especially obvious in the records with open eyes. According to previous investigations, the abnormal theta-rhythm is correlated with a geneti
