Review of calcium hydroxide.

Calcium hydroxide is a material which has been used for a variety of purposes since its introduction into dentistry in the early part of the twentieth century. In its pure form, the substance has a high pH, and its dental use relates chiefly to its ability to stimulate mineralization, and also to its antibacterial properties. A range of products has been formulated with different therapeutic actions, the effects of which are partially dependent upon the tissue to which they are applied. The material is reviewed under the following general headings: biochemical actions; dental formulation; uses

Stepwise excavation in a permanent molar: 17-year follow-up.

The current study presents a 17-year clinical report of stepwise excavation and indirect pulp capping in a lower right first molar, with great dentin destruction and a lack of dentin support of the cusps. At the first appointment, indirect pulp capping with calcium hydroxide and a temporary filling with zinc oxide cement were performed to minimize the risk of pulp exposure during excavation. After 45 days, the remaining carious tissue was removed and a restoration with glass-ionomer lining (Vitrebond) and resin composite (P-50) was performed. Satisfactory morphology and function of the restora

Comparison of five different Light-Cured Pulp Capping Materials to a Hydraulic Calcium Silicate Cement: A Long-Term Physicochemical Study.

OBJECTIVES: Hydraulic calcium silicate cements (HCSCs) are the gold standard for vital pulp therapy, although they have a prolonged setting time. Light-cured flowable composite resins containing calcium silicate or calcium hydroxide powder are promoted as an alternative. The aim of this study was to compare the physicochemical properties of 5 light-cured pulp capping materials (LCPCMs) (Calcimol LC, TheraCal LC, ReViCal, MTA PulpCap, and Pulprotec MTA) with HCSC (Biodentine). METHODS: Cylindrical specimens (51.472 mm3; n = 13/material/time) were stored in 5 mL Hanks' Balanced Salt Solution in

The disastrous effects of the "total etch" technique in vital pulp capping in primates.

PURPOSE: To determine by means of a histopathological study in sub-human primates whether etching of an exposed pulp (the "Total Etch" technique) followed by capping with dentin bonding agents is a viable clinical treatment modality. MATERIALS AND METHODS: In six sub-human primates, 147 Class V preparations were made for five experimental and two control groups. After exposing the pulps, the preparations were intentionally contaminated, rinsed, dried and then disinfected with a 2% chlorhexidine solution for 60 s. In the five experimental groups, the entire preparation including the exposed pu

Effects of diode laser on direct pulp capping treatment : a pilot study.

At the present time, evidence-based, best practices have yet to be established for maintaining the vitality of teeth by managing caries-associated pulp exposure in permanent teeth. In terms of biomechanical and esthetic considerations, pulp capping has proven to be more effective than root-canal therapy. Given the low success rate of conventional methods, new techniques, such as laser-assisted repairs, should be developed. The purpose of this study was to compare the effectiveness of conventional and diode laser-assisted methods in direct pulp capping of carious teeth. Ten patients ranging in

Restorative and endodontic management of an anomalous mandibular molar.

Morphologic variations of dental hard tissue should be recognized and considered during restorative procedures. Overlooking anomalies increases the potential for clinical complications. Amalgam was used erroneously in a patient when the presence of a paramolar structure was overlooked. The clinical complications that resulted from overlooking the anomaly led to restorative, endodontic, and periodontal failures. The present report describes the use of conservative endodontics and a cast restoration, taking into account the presence of the paramolar structure, to correct the case.

Sealing properties of a self-etching primer system to normal caries-affected and caries-infected dentin.

PURPOSE: To compare the ability of an experimental antibacterial self-etching primer adhesive system to seal exposure sites in normal, caries-affected and caries-infected human dentin. METHODS: 30 extracted human third molars were used within 1 month of extraction. 10 intact normal teeth comprised the normal group. 20 teeth with occlusal caries that radiographically extended halfway to the pulp were excavated using caries-detector solution (CDS) and a #4 round carbide bur in a slowspeed handpiece. Half of those teeth were fully excavated free of CDS-stained material without exposing the pulp,

Endodontics: Part 9. Calcium hydroxide, root resorption, endo-perio lesions.

For more than 70 years calcium hydroxide has played a major role in endodontic therapy, although many of its functions are now being taken over by the recently introduced material MTA. Calcium hydroxide may be used to preserve the vital pulp if infection and bleeding are controlled; to repair root fractures, perforations, open apices and root resorptions. Endo-perio lesions are complex and the correct diagnosis is essential if treatment is to be successful. However, root canal treatment will always be the first phase in treating such lesions.
