[Basal cell nevus syndrome: the interface between dentistry and dermatology].

Basal cell nevus syndrome is a rare, autosomal dominant disorder, predominantly caused by a mutation in the PTCH1 gene. As basal cell carcinomas and keratocysts are the most common abnormalities, dermatologists, orofacial maxillary surgeons, and dentists play a key role in patient care. From the age of 8, screening for odontogenic keratocysts with an orthopantomogram or MRI is recommended every other year. The intensity increases to annual screening after the development of the first odontogenic keratocyst. If BCNS is caused by an underlying SUFU mutation, screening is not indicated since ther

Bisphosphonates in dentistry: Historical perspectives, adverse effects, and novel applications.

Studies of the potential role of bisphosphonates in dentistry date back to physical chemical research in the 1960s, and the genesis of the discovery of bisphosphonate pharmacology in part can be linked to some of this work. Since that time, parallel research on the effects of bisphosphonates on bone metabolism continued, while efforts in the dental field included studies of bisphosphonate effects on dental calculus, caries, and alveolar bone loss. While some utility of this drug class in the dental field was identified, leading to their experimental use in various dentrifice formulations and i…

Fungal infections in dentistry: Clinical presentations, diagnosis, and treatment alternatives.

Oral candidiasis is a common opportunistic infection that requires knowledge of the various clinical presentations and management strategies for successful treatment. Numerous local and systemic factors contribute to the development of candidiasis, and the infections can range from superficial mucocutaneous overgrowths to invasive bloodstream infections with a high mortality rate. In addition to Candida albicans, various fungal strains have been isolated from the oral cavity, including C. tropicalis, C. glabrata, C. krusei, and many others. Antifungal agents are available in various forms, eac

Horizontal bone-augmentation procedures in implant dentistry: prosthetically guided regeneration.

The rehabilitation of partially or totally edentulous patients with implant-supported prostheses has become routine, with excellent long-term outcome. A proper implant position is mandatory to achieve good functional and esthetic outcome and may require an adequate amount of alveolar bone and surrounding soft tissue. When this is lacking because of atrophy, sequelae of periodontal disease, traumas or congenital malformations, increased bone volume and/or keratinized mucosa can be obtained by guided bone regeneration, bone-grafting techniques and alveolar bone expansion. This article presents a

Soft Tissue Applications of Er,Cr:YSGG Laser in Pediatric Dentistry.

UNLABELLED: Soft tissue laser, such as diode and Nd:YAG lasers were initially used in soft tissue lesions because of its increased success rate. It was because of the fact that these lasers were well-absorbed by chromophores, such as hemoglobin and melanin which are found abundant in the oral mucosa. The introduction of erbium family in 1990 comprising the Er:YAG and Er,Cr:YSGG lasers made the hard tissue laser a boon for dentistry. Erbium, chromium-doped yttrium, scandium, gallium and garnet (Er,Cr:YSGG) was introduced in 1997 for the surgical needs of clinical dentistry in general practice. …

Nerve damage related to implant dentistry: incidence, diagnosis, and management.

Proper patient selection and treatment planning with respect to dental implant placement can preclude nerve injuries. Nevertheless, procedures associated with implant insertion can inadvertently result in damage to branches of the trigeminal nerve. Nerve damage may be transient or permanent; this finding will depend on the cause and extent of the injury. Nerve wounding may result in anesthesia, paresthesia, or dysesthesia. The type of therapy to ameliorate the condition will be dictated by clinical and radiographic assessments. Treatment may include monitoring altered sensations to see if they

Evidence-based dentistry: analysis of dental anxiety scales for children.

OBJECTIVES: To review paediatric dental anxiety measures (DAMs) and assess the statistical methods used for validation and their clinical implications. METHOD: A search of four computerised databases between 1960 and January 2011 associated with DAMs, using pre-specified search terms, to assess the method of validation including the reliability as intra-observer agreement 'repeatability or stability' and inter-observer agreement 'reproducibility' and all types of validity. RESULTS: Fourteen paediatric DAMs were predominantly validated in schools and not in the clinical setting while five of

Advanced concepts in implant dentistry: creating the "aesthetic site foundation".

To obtain optimal and predictable aesthetics, deficiencies caused by soft and particularly hard tissue loss can be managed by various methods, such as orthodontic tooth eruption, socket preservation, and guided bone regeneration. However, in complex cases, these methods are often insufficient. Here, the authors introduce advanced concepts in aesthetic implant dentistry, such as "Aesthetic Site Foundation", "Aesthetic Guided Bone Regeneration" and "Implant Rectangle" that will guide the clinician in the quest to optimal aesthetic outcomes.

[Research methods in dentistry 7. Diagnostic tests in oral diseases].

A variety of tests are available to arrive at the proper diagnosis of oral and perioral diseases, which may be extremely important if further treatment is indicated. When oral cancer is suspected the main diagnostic tool remains the histopathologic examination of a biopsy specimen. The value of cytologic examination, whether obtained by exfoliation of cells or by a brush technique, is somewhat questionable. A positive result of such examination needs to be confirmed by a biopsy. The same applies to the use of toluidine blue stain. The presence or absence and, if present, the degree of epitheli

Perioral soft-tissue rejuvenation techniques to enhance esthetic restorative dentistry.

Every practitioner has the obligation to offer his or her patients the latest advances in their profession. Dentistry has made a shift over the past 50 years from caries restoration and prosthetic replacement to preventative and esthetic rejuvenation. Related dental specialties have also blossomed with new procedures and an expanded scope of practice. This shift has also included more comprehensive care and treatment of the oral and maxillofacial region. Modern esthetic dentists realize the facial soft tissues serve as a frame for their restorative artwork. Contemporary oral and maxillofacial …

Determination of age from cemental incremental lines for forensic dentistry.

Determination of age from cemental incremental lines was evaluated in intact teeth obtained from 17 individuals aged 23-77 years. Mineralized 100 microm cross sections were subjected to one of three treatments: unstained, stained with Villanueva's blood stain, and stained with acridine orange. Ideal areas were selected by light microscopy and photographed. Countability of incremental lines from photographic enlargements were evaluated. The average number of years required for the eruption of a particular tooth was added to the incremental lines count to determine the estimated age for that ind

[Veterinary dentistry (9). Classification, nomenclature and identification of animal dentition].

This article in the series about the veterinary dentistry describes how the various dentitions of mammals can be distinguished by their form and function. The number of times and manner in which teeth are shed together with the course of tooth eruption also determine the classification. Attention is paid to the nomenclature and differentiation of dental elements, how the dental formula are composed, and how these formulas differ for various species. The importance of a standardized system for classifying the position of the dental elements is stressed. The three-digit notation system of Triada…

What do people want from dentistry? An action research approach.

Issues surrounding access to dentistry will be presented from information gathered during an action research project targeting three population groups. This project was conducted by students in the Dental Hygiene Degree Completion Program at the University of British Columbia and by the dental hygiene diploma students at Camosun College. In contrast to typical empirical dental studies which are validated by statistical presentation, action research uses an interview process and qualitative analysis to identify what people really think, believe and do. College students, independent-living senio…

Congenital diaphragmatic hernia: implications for nitrous oxide use in dentistry.

The development of diaphragmatic hernias, their associated physical and diagnostic signs and symptoms, and the potential complications with nitrous oxide use are presented with a case report. Depending upon the location and extent of the diaphragmatic defect, portions of the stomach, omentum, liver and/or intestine can occupy a portion of the thoracic cavity. Nitrous oxide's solubility properties allow for rapid expansion of the herniated bowel, resulting in compression of the thoracic organs or strangulation of the herniated abdominal viscera. The presence of a diaphragmatic hernia may necess

Integrating planetary health into dentistry: an updated Montreal-Toulouse model.

The Earth has entered a critical phase, with seven of nine planetary boundaries already transgressed. This ecological catastrophe profoundly affects human health, highlighting the links between people's wellbeing and the state of the planet. Accordingly, health professionals are increasingly called to embrace planetary health and adopt sustainable practices. This is particularly relevant for dental clinics, which have a high ecological footprint and a responsibility to provide quality care that is both environmentally and socially sustainable. By integrating planetary health into practice, den
