Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease characterized by recurrent and painful nodules and abscesses in intertriginous skin areas, which can progress to sinus tract formation, tissue destruction, and scarring. HS is highly debilitating and severely impairs the psychological well-being and quality of life of patients. The therapeutic approach to HS is based on medical therapy and surgery. BACKGROUND: Alternative therapies are favored by some patients. In skin cancer, this may be to try to avoid surgery out of fear for the procedure and its functional or cosmetic consequences. Frequently, use of these therapies is not curative and can cause harm. BACKGROUND: The earlobe is a location with a high risk of keloid scar formation. Keloid scars pose a surgical challenge from recidivation. The objective of this study was to investigate a new surgical approach for the treatment of auricular keloids.
METHODS AND RESULTS: In the past 4 years, 11 earlobe keloids of 9 patients have been excised by fistulectomy (perforation operation). All of the patients were followed up for at least 12 months without recurrence.
CONCLUSIONS: As a new surgical approach, a perforation operation together with fistulectomy is suitable for lobular keloids.

Skin dimples are a common occurrence in children. Besides being of cosmetic significance, they may give an important clue to an underlying genetic or metabolic problem. A simplified location-based algorithmic approach to diagnose the underlying cause of skin dimples is presented. Clinical significance of medically important dimples, especially sacral dimples, its association with occult spinal dysraphism, and a cost-effective diagnostic strategy for its imaging is discussed.

Amputees cannot feel what they touch with their artificial hands, which severely limits usefulness of those hands. We have developed a technique that transfers remaining arm nerves to residual chest muscles after an amputation. This technique allows some sensory nerves from the amputated limb to reinnervate overlying chest skin. Subungual chondroma is a rare occurrence of periosteal chondroma, which mainly affects short tubular bones of the hands and feet. Transformation towards malignancy has been described in a small number of cases; however histologic diagnosis can prove to be difficult, especially with regard to well differentiated chondrosarcoma. We report the case of a 14 year old girl with a one year history of subungual chondroma of the fourth toe with good clinical response to surgical removal.

OBJECTIVE: To analyze the incidence and the clinical and histopathological features of non-inflammatory scrotal lesions with special reference to the non-neoplastic lesions, which are the most common.
METHODS: The cases of scrotal surgical pathology of the Hospital Nuestra Señora del Pino from 1988-1997 were reviewed. Skin biopsies for dermatological conditions, Fournier's gangrene, inflammatory-infectious lesions, hydroceles, lesions that could cause secondary involvement of the scrotal wall (e.g., spermatoceles) and invasive lesions from adjacent tumors were excluded from the study.

A nine-year-old male Ibizan hound had a network of large tortuous pulsating blood vessels on the prepuce that enlarged gradually over a five month period. A diagnosis of arteriovenous fistula was based on clinical signs, angiography and Doppler ultrasonography. Ligation of the major vascular supply to the fistula resulted in only temporary improvement. Definitive treatment was by wide excision, with penile amputation and scrotal urethrostomy.

Keloid scarring presents a difficult therapeutic problem. It is a recognized sequel to acne vulgaris, and may be extensive and disfiguring. The data from this double-blind study, in which intralesional triamcinolone or cryosurgery were used as treatment for keloids, demonstrate that by treating early, and in particular vascular, keloids with the latter, 85% will show a moderate to good response in terms of flattening. Treatment with intralesional triamcinolone was also beneficial, but the response to cryosurgery was significantly better in early, vascular lesions.
