Lymphoedema management: an international intersect between developed and developing countries. Similarities, differences and challenges.

Lymphoedema is a chronic swelling condition that contributes to disability, dysfunction and lost quality of life. Significant disparities exist worldwide regarding the availability of resources necessary to identify, treat and manage lymphoedema. This disparity transcends socio-economic status and is a common problem in both developed and developing countries. The overall impact of lymphoedema as a public health problem, however, is underestimated, principally due to the lack of epidemiologic data. These problems pose barriers to optimal identification and management of this disabling, lifelon…

Thrombolytic therapy for acute stroke in the elderly: an emergent condition in developing countries.

Elderly patients may represent an important group when considering new stroke treatments, particularly in developing countries. The aim of this study was to analyze the use of recombinant tissue plasminogen activator (rtPA) in elderly Brazilian patients with acute ischemic stroke. Clinical and neuroimaging parameters at admission, frequency of symptomatic intracranial hemorrhage, and outcome were compared between elderly (≥80 years) and nonelderly (<80 years) stroke patients treated with rtPA in the Porto Alegre Stroke Network. We evaluated 183 nonelderly patients (mean age, 63 ± 12 years) and…

An optimization model for reducing typhoid cases in developing countries without increasing public spending.

This article considers the investment case for using the Vi polysaccharide vaccine in developing countries from two perspectives: reducing typhoid cases and limiting new health care spending. A case study is presented using data from South and Southeast Asia. The purpose of the paper, however, is to draw broad implications that may apply to developing countries in general. Typical consumer demand functions developed from stated preference household surveys in South and Southeast Asia are used to predict probabilities of adults and children purchasing typhoid vaccinations at different prices. T…

Oesophageal corrosive injuries in children: a forgotten social and health challenge in developing countries.

PROBLEM: An unsafe environment is a risk factor for child injury and violence. Among those injuries that are caused by an unsafe environment, the accidental ingestion of corrosive substances is significant, especially in developing countries where it is generally underreported. APPROACH: To address this challenging, unmet medical need, we started a humanitarian programme in Sierra Leone. By reviewing the current literature from developing countries and our own experience in the field, we developed a flowchart for management of this clinical condition. LOCAL SETTING: This injury is underrepor…

Are self-reports of health and morbidities in developing countries misleading? Evidence from India.

Self-reported measures of poor health and morbidities from developing countries tend to be viewed with considerable skepticism. Examination of the social gradient in self-reported health and morbidity measures provides a useful test of the validity of self-reports of poor health and morbidities. The prevailing view, in part influenced by Amartya Sen, is that socially disadvantaged individuals will fail to perceive and report the presence of illness or health-deficits because an individual's assessment of their health is directly contingent on their social experience. In this study, we tested w…

Systematic review of the efficacy and effectiveness of complementary feeding interventions in developing countries.

UNLABELLED: [Table: see text] INTRODUCTION: Complementary feeding interventions are usually targeted at the age range of 6–24 months, which is the time of peak incidence of growth faltering, micronutrient deficiencies and infectious illnesses in developing countries. After 2 years of age, it is much more difficult to reverse the effects of malnutrition on stunting, and some of the functional deficits may be permanent. Therefore, interventions that are effective at reducing malnutrition during this vulnerable period should be a high priority. Although several types of interventions can be targ…

Investment in HIV/AIDS programs: does it help strengthen health systems in developing countries?

BACKGROUND: There is increasing debate about whether the scaled-up investment in HIV/AIDS programs is strengthening or weakening the fragile health systems of many developing countries. This article examines and assesses the evidence and proposes ways forward. DISCUSSION: Considerably increased resources have been brought into countries for HIV/AIDS programs by major Global Health Initiatives. Among the positive impacts are the increased awareness of and priority given to public health by governments. In addition, services to people living with HIV/AIDS have rapidly expanded. In many countrie…

Ethical aspects of urinary diversion for women with irreparable obstetric fistulas in developing countries.

Obstetric fistula formation is a catastrophic complication of prolonged obstructed labor. Obstetric fistulas are common in impoverished countries where access to maternal health care is poor. Although most fistulas can be closed successfully at the time of operation, a small number of women sustain such extensive pelvic injuries that their fistulas are irreparable. Some Western surgeons visiting African countries where fistulas are prevalent have become enthusiastic advocates of performing urinary diversions on these women, transplanting the ureters into the colon. We present a case study of o

Release criteria from hospitals of 131I thyrotoxicosis therapy patients in developing countries--case study.

The current release limit, recommended by the International Atomic Energy Agency (IAEA)(1), from hospitals of patients undergoing 131I thyrotoxicosis therapy is approximately 1100 MBq (approximately 30 mCi). Owing to the difference in socio-economic conditions, literacy rate, family system, etc., this release limit may not be applicable in most of the developing countries like Pakistan. Therefore, the prime objective of this case study was to re-evaluate the release criteria for 131I thyrotoxicosis therapy patients by taking into account their lifestyle, economic conditions and other facilitie…

Effect of infections and environmental factors on growth and nutritional status in developing countries.

Despite numerous advances and improvements in child health globally, malnutrition remains a major problem and underlies a significant proportion of child deaths. A large proportion of the hidden burden of malnutrition is represented by widespread single and multiple micronutrient deficiencies. A number of factors may influence micronutrient deficiencies in developing countries, including poor body stores at birth, dietary deficiencies and high intake of inhibitors of absorption such as phytates and increased losses from the body. Although the effects of poor intake and increased micronutrient …

Should deworming be included in antenatal packages in hookworm-endemic areas of developing countries?

BACKGROUND: WHO recommends antenatal (after the first trimester) deworming for pregnant women who live in areas where the prevalence of hookworm infection exceeds 20-30%. However, deworming has not been included in antenatal care packages in most developing countries. METHODS: A review of articles publishing original data identified primarily through Medline was conducted using subject heading terms and text words for "deworming", "pregnant women", "hookworm", "anthelminthic", "anthelmintic", "albendazole", "mebendazole", "pregnancy" and their combinations. Promotion of smoking cessation in developing countries: a framework for urgent public health interventions.

The rapid rise in smoking in many developing countries will have devastating consequences; by 2030 the developing world is expected to have 7 million deaths annually from tobacco use. Many smokers express a desire to quit, but they often fail because they are addicted to tobacco. Although a number of cessation aids are now available in the developed world, their applicability and affordability in developing countries is less clear. Successful interventions will require many stakeholder groups to take action at the local, national, and international levels. We discuss smoking cessation as a mea…

Women's perceptions of iron deficiency and anemia prevention and control in eight developing countries.

The World Health Organization estimates that 58% of pregnant women in developing countries are anemic. In spite of the fact that most ministries of health in developing countries have policies to provide pregnant women with iron in a supplement form, maternal anemia prevalence has not declined significantly where large-scale programs have been evaluated. During the period 1991-98, the MotherCare Project and its partners conducted qualitative research to determine the major barriers and facilitators of iron supplementation programs for pregnant women in eight developing countries. Research resu…

The epidemiological features of heart failure in developing countries: a review of the literature.

Cardiovascular diseases are increasingly recognised as an important cause of morbidity and mortality in developing countries. This is due to the ageing of the population and better control of communicable disease and malnutrition. We review the published data on the epidemiology of heart failure in such countries. Rheumatic heart disease remains a major cause of heart failure in Africa and Asia, especially in the young. Hypertension is an important cause of heart failure especially in the African and African-American population. Chagas' disease is still a cause of heart failure in South Americ…

Therapeutic and preventive effects of zinc on serious childhood infectious diseases in developing countries.

In children in developing countries, zinc deficiency may be common and associated with immune impairment and increased risk of serious infectious diseases such as diarrhea, pneumonia, and malaria. Studies have evaluated the therapeutic effects of zinc supplementation during acute or persistent diarrhea. In studies of acute diarrhea, the illness duration has been found to be 9-23% shorter in zinc-supplemented than in control children. Diarrhea was also less severe in zinc-supplemented children. In studies of persistent diarrhea, the effect sizes were similar but were often not statistically sig…
