Terpenoids and phenols from Trachyspermum ammi (T. ammi) have reported some pharmacological actions. The objective of the work was to isolate the active constituent, its identification by spectroscopic techniques, and evaluation of the antidiabetic and neuroprotective activity from T. ammi on STZ Wistar rats. The dried fruits of T ammi were kept in a hydrodistillation apparatus to collect essential oil. The isolated fraction went through TLC, UV, FTIR, HPLC, HRMS, C13, and 1H NMR for characterization. Two dosage concentrations from the isolated compound were prepared as 10 and 20 mg/kg for tre

Diabetic neuropathy (DN) is a common long-term complication of type 1 (T1D) and type 2 (T2D) diabetes mellitus, with significant morbidity and mortality. DN is defined as impaired function of the autonomic and/or peripheral nervous system, often subclinical, particularly in children and adolescents with T1D. Nerve conduction studies (NCS) and skin biopsies are considered gold-standard methods in the assessment of DN. Multiple environmental and genetic factors are involved in the pathogenesis of DN. Specifically, the role of metabolic control and glycemic variability is of paramount importance.

Neurological complications of diabetic ketoacidosis are considered to be a serious clinical problem. The most common complication is cerebral edema. However, these neurological complications also include less common entities such as ischemic or hemorrhagic stroke, cerebral venous and sinus thrombosis or peripheral neuropathy. We present a case of a 9-year old girl admitted to our intensive care unit with new onset type 1 diabetes, diabetic ketoacidosis, cerebral edema, multifocal vasogenic brain lesions and bilateral lower limb peripheral paresis. The patient developed polydipsia and polyuria

A reduction in the bulk of the extensor digitorum brevis muscle (EDB) may be a sign of diabetic neuropathy. We devised a semi-quantitative scale (normal, reduced or absent) for assessing the bulk of the EDB muscle, and judged it to be a sign of neuropathy in 375 of 395 diabetic patients aged 15-50 years in the county of Umeå, 79% of whom had Type 1 diabetes mellitus (DM), and in 100 healthy controls. Reduced or absent EDB was significantly more common in patients with Type 1 and Type 2 DM than in controls (44 and 48 vs. 12%; P less than 0.001). In patients with Type 1 DM, reduced or absent EDB

The article by Han and associates highlights a frequently neglected consequence of type 2 diabetes mellitus (T2D): Dry eye disease associated with corneal neuropathy. Their findings indicate that patients with T2D exacerbated by dry eye disease exhibit substantial decreases in corneal nerve density, length, and count, accompanied by heightened tortuosity, all of which correlate strongly with clinical severity scores. This study contributes to the expanding literature that regards ocular surface disease not only as a secondary effect of systemic illness but also as a direct indication of neurov

With the global diabetes population projected to reach 783 million by 2045, diabetic peripheral neuropathy (DPN) remains a common and debilitating complication characterized by metabolic stress, inflammation, and microvascular dysfunction. This review summarizes current interventions for DPN from the perspective of therapeutic targets, pharmacological mechanisms, and emerging delivery strategies. Conventional pharmacotherapy mainly provides symptomatic relief; first-line analgesics such as duloxetine and pregabalin offer only moderate benefit, with a number needed to treat of approximately 4-5

BACKGROUND: Diabetic painful neuropathy is a cause of significant disability and sleep disturbance but may be undiagnosed in up to 80% of patients with diabetes. Various neuropathic pain (NP) questionnaires have good diagnostic utility but are impractical in resource-constrained and busy clinical settings. A simple sCreening Tool (ACT) was developed as a concise, patient-led tool to rapidly detect NP. This study evaluates the screening validity and psychometric properties of ACT compared with the DN4 questionnaire (reference standard).
METHODS: We conducted an observational study employing cl

Painful diabetic peripheral neuropathy (PDPN) remains a serious complication of diabetes mellitus (DM). Recent clinical trials have demonstrated promising outcomes for pilavapadin (LX9211), an orally administered selective, potent inhibitor of adapter protein-2-associated kinase 1 (AAK1) and vixotrigine, a broad spectrum voltage-gated sodium channels (Navs) inhibitor. Their beneficial role was reflected in improved average daily pain (ADP) score and Patient Global Impression of Change (PGIC). Gamma-aminobutyric acid (GABA) receptor agonism has yielded favourable outcomes in preliminary studies

Diabetic peripheral neuropathy (DPN) is common, disabling, and associated with substantial risk of foot ulceration and lower extremity complications. A subset of patients develops superimposed peripheral nerve entrapment, which can exacerbate pain, impair protective sensation, and contribute to microcirculatory dysfunction. Although surgical decompression has been studied for more than three decades, awareness of entrapment features remains low in primary care, and guidance on patient selection is limited. This review summarizes current evidence on the clinical identification of focal nerve co

To the Editor, Erection and ejaculation are necessary processes for semen to be deposited naturally in the vagina, allowing sperm to migrate in search of the egg in the female reproductive tract and achieve fertilization. However, in some cases, complete retrograde ejaculation (with absence of ejaculation - aspermia) or partial retrograde ejaculation (decreased seminal volume - hypospermia - and sperm concentration - oligozoospermia) may occur. In cases of aspermia or anejaculation, retrograde ejaculation should be suspected, and the diagnosis is confirmed by finding sperm in the post-ejaculat

AimsSensory tests such as vibration perception threshold (VPT), monofilament, pinprick, and temperature sensation are used to detect diabetic peripheral neuropathy (DPN). This study examined associations between these tests and chronic diabetic complications in type 2 diabetes, focusing on the clinical relevance of VPT.MethodsIn this retrospective study, 652 patients with type 2 diabetes evaluated at a tertiary center (2014-2024) underwent standardized neurological assessments. Based on VPT, patients were categorized as normal (<16 V), borderline (16-24 V), or abnormal (≥25 V). Associations be

Diabetic peripheral neuropathy (DPN) is a prevalent and severely disabling complication of diabetes mellitus characterized by complex pathophysiological mechanisms. Beyond the metabolic disorder induced by glucolipotoxicity, DPN represents an immunometabolic dysregulation arising from the interaction between metabolic abnormalities and immune imbalance. This review comprehensively encapsulates recent advances in the understanding of DPN through the lens of immunometabolism. Initially, classical pathophysiological mechanisms are discussed, demonstrating that persistent hyperglycemia and lipotox
