BACKGROUND: Malaria remains a leading cause of morbidity and mortality among children under five years of age in Nigeria. While microscopy is the World Health Organization (WHO) gold standard for malaria diagnosis, rapid diagnostic tests (RDTs), such as the Standard Diagnostic Bioline Malaria Antigen Plasmodium falciparum (SD Bioline Malaria Ag P.f.), are increasingly used in healthcare and field settings. However, recent reports show variable performances of these tests, and evidence on their performance metrics from large, nationally representative Nigerian samples is limited. This study, th

We evaluated the diagnostic agreement of expired malaria rapid diagnostic tests (mRDTs) with nonexpired mRDTs in a clinical setting in western Uganda. Expired mRDTs (SD Bioline Ag Pf/Pan and First Response Ag pLDH/HRP2) were administered to each participant in parallel with a nonexpired mRDT serving as the reference. Of 200 participants, 21 (10.5%) tested positive and 179 (89.5%) tested negative for malaria. All tests up to 9 months postexpiration were valid and consistent with reference mRDTs. All mRDTs 10-12 months postexpiration were invalid. These preliminary findings support further resea

Rapid diagnostic tests (RDTs) have revolutionized malaria diagnosis, playing a crucial role in improving timely treatment and supporting surveillance efforts, especially in resource-limited settings. However, the performance of RDTs can vary widely due to factors such as parasite genetic diversity, environmental conditions, and operational challenges. Understanding these variations is essential to ensuring accurate and reliable malaria diagnosis. This systematic review and meta-analysis critically evaluate the diagnostic performance of malaria RDTs across sub-Saharan Africa, identifying key ga

Human strongyloidiasis is an important intestinal parasitic disease that affects public health globally and is transmitted through contact with infective larvae on contaminated soil. Immunocompromised hosts can experience hyperinfection, which can lead to fatal systemic strongyloidiasis. Here, an innovative point-of-care (POC) test kit, the strongyloidiasis immunochromatographic blood test (the NIE-SsIR whole-blood ICT) kit is described. The kit was used to detect anti-Strongyloides IgG antibody in whole-blood samples (WBSs) instead of serum to diagnose strongyloidiasis. The kit is based on a

The World Health Organization's 2035 vision is to reduce tuberculosis (TB) associated mortality by 95%. While low-burden, well-equipped industrialised economies can expect to see this goal achieved, it is challenging in the low- and middle-income countries that bear the highest burden of TB. Inadequate diagnosis leads to inappropriate treatment and poor clinical outcomes. The roll-out of the Xpert(®) MTB/RIF assay has demonstrated that molecular diagnostics can produce rapid diagnosis and treatment initiation. Strong molecular services are still limited to regional or national centres. The del

Tuberculosis (TB) remains the leading cause of death worldwide from a single infectious disease. An estimated quarter of the world's population, about 2 billion people, has an immune response to Mycobacterium tuberculosis (Mtb) without clinical, microbiological, or radiological signs of TB disease. This condition is known as TB infection (TBI) and carries a lifelong risk of reactivation with 5%-10% of individuals eventually developing TB disease during their lifetime. Interferon-γ release assay and skin-tests are World Health Organization (WHO)-approved tests for TBI diagnosis and allow to ide

By the end of the nineteenth century, ammonia had been identified as the main factor responsible for hepatic encephalopathy syndrome. Ammonia is one of the principal products of hepatic metabolism and high concentrations are toxic to the body. Under physiological conditions, the main way by which the body restricts the blood concentration of ammonia to a nontoxic level is by converting it to urea in the liver via the Krebs cycle. The synthesis of glutamine represents an alternative pathway for ammonia detoxification in cirrhotic patients. Although high concentrations of ammonia have been stron

OBJECTIVE: This study investigated the reliability of the malaria rapid diagnostic test (mRDT) kit for detecting Plasmodium falciparum histidine-rich protein 2 (PfHRP2) in dried blood spot samples preserved under different conditions and durations.
RESULTS: Sixty whole blood samples (approx. 750 mL), which contained the PfHRP2 protein, were collected from patients with uncomplicated malaria. Fifteen dried blood samples (DBS) were made from each sample, making a total of 900 DBS. A set of five DBS each were stored at room temperature (25 °C), 6 °C and - 60 °C, for 30, 60, 90, 120 and 150 days.

Leptospirosis is an endemic zoonotic disease with protean clinical manifestations caused by pathogenic spirochetes of the Leptospira genus. The microscopic agglutination test (MAT) is the gold standard for leptospirosis diagnosis and can only be conducted in a reference laboratory. Therefore, alternative tests, such as the IgM anti-Leptospira rapid diagnostic test (RDT), are preferred for general use. In this study, we aimed to compare the accuracy of four products of anti-Leptospira IgM detection RDTs, which are available in Indonesia, against the gold standard of Leptospira MAT. This study w

Parasitic infections present a significant health risk to the public, affecting millions of people, particularly in underdeveloped and developing countries. In developing countries, these infections are also responsible for causing significant economic challenges due to elevated healthcare expenditure. Accurate diagnosis and effective treatment methods are essentially required to combat this global issue. For decades, traditional diagnostic methods such as microscopy, serological testing, histopathology, and culturing have been used for the diagnosis of these parasitic infections. While these

BACKGROUND: Malaria is one of the most important parasitic infectious diseases for which almost half of the world's population is at risk. Although several diagnostic methods are now available to detect the infection, more sensitive and applicable tests are still required in the field. The loop-mediated isothermal amplification (LAMP) method is a DNA amplification tool in which the DNA amplification can be achieved by incubation at a stable temperature. A malaria detection kit based on this methodology has already been commercialized and is being used in some countries. The kit includes two re

BACKGROUND: The World Health Organization recommends GeneXpert MTB/RIF Ultra (Xpert Ultra), a fully automated polymerase chain reaction (PCR) assay, as the initial tuberculous meningitis (TBM) diagnostic test. The assay's PCR cycle threshold (Ct) values represent the number of PCR cycles required for probe signal to be detected (low Ct value = high bacillary load) and may approximate tuberculosis (TB) bacillary load. We measured the relationship between cerebrospinal fluid (CSF) TB bacillary load with mortality.
METHODS: We prospectively enrolled 102 human immunodeficiency virus (HIV)-positiv

Estimates for the year 2000 suggested that there were approximately 21.5 million infections and 200,000 deaths from typhoid fever globally each year, making the disease one of the most serious infectious disease threats to public health on a global scale. However, these estimates were based on little data, especially from Africa. Global prominence and high-profile outbreaks have created the perception in Kenya that typhoid is a common cause of febrile illness. The Widal test is used widely in diagnosis. We have reviewed recent literature, taking the perspective of a healthcare provider, to col

Model-based economic evaluations rely on results of diagnostic test accuracy (DTA) studies to assess the value-for-money of a new diagnostic or prognostic test. The 30-item Standards for Reporting of Diagnostic Accuracy Studies (STARD) 2015 guidelines are the gold-standard guidelines for DTA studies. In this Perspective paper, we conducted a rapid review of publicly available economic evaluations of a new diagnostic test between 2012 and 2025 to formulate eight elements that we believe could be added to STARD 2015 to help enhance the credibility of DTA-based economic evaluations. Examples from

BACKGROUND: The World Health Organization calls for the development of new diagnostics to support large-scale deworming programs against strongyloidiasis. To better steer research and development (R&D) of new diagnostics, it is imperative to identify the minimal requirements that new diagnostics should meet, the so-called target product profiles (TPPs). While diagnostic TPPs exist for other major soil-transmitted helminthiases, none exist for strongyloidiasis.
METHODS: We investigated a range of potential diagnostic TPPs using our previously developed simulation framework for the effect of im

Dengue is an arboviral infection that poses a substantial public health concern, with early diagnosis being a critical factor in effective management. However, limited diagnostic expertise in developing countries contributes to the under-reporting of dengue cases. This review compares the accuracy of rapid diagnostic tests (RDTs) and the tourniquet test (TT) in diagnosing dengue fever (DF) in non-laboratory-based settings. Relevant original articles on the use of RDTs and TT for dengue diagnosis were retrieved from PubMed, Scopus, and ScienceDirect. The STARD and QUADAS-2 tools were employed t
