Lifestyle interventions in women with polycystic ovary syndrome: A scoping systematic review of randomised evidence.

OBJECTIVE: Lifestyle interventions (LSI) are recommended as first-line treatment for polycystic ovary syndrome (PCOS), yet the strength of evidence underpinning LSIs effectiveness remains unclear. We systematically reviewed the literature on LSIs in PCOS, evaluated evidence quality and summarised recommendations for clinical practice. MATERIAL AND METHODS: We searched MEDLINE, EMBASE and CENTRAL for all randomised trials evaluating any LSI in PCOS until April 2021. We extracted data on the LSIs' characteristics, dietary composition, duration, implementation, compliance assessment, and reporte

The food restriction wars: Proposed resolution of a primary battle.

Research regarding the definition and consequences of dieting has generated controversy for years. This controversy has spilled over into the public domain, especially as eating disorders and obesity have become more prevalent. One of the earliest and longest-lasting controversies involves the restrained eating framework, which was originally developed by Herman and Polivy and also strongly influenced the development of the cognitive-behavioral model of bulimia nervosa. An alternative framework for understanding the role of dieting in nonclinical and clinical groups, called the Three-Factor Mo

Rhabdomyolysis in an adolescent with nonpurging bulimia nervosa.

A 14-year-old girl with a history of nonpurging bulimia nervosa (BN) was presented with muscular pain and weakness after several days of fasting and vigorous exercise. On hospital admission, the serum creatine kinase was 3,277 U/L and serum potassium was 2.5 mmol/L. The creatine kinase level reached a peak of 4,588 U/L before declining after intravenous hydration and correction of electrolytes. Renal function remained normal throughout hospitalization. Rhabdomyolysis has not been previously reported as a complication of BN in an adolescent or in nonpurging BN. Practitioners who treat patients

[Pharma clinics. How I treat...a diabetic patient with severe obesity].

Obesity plays a crucial role in type 2 diabetes pathophysiology and a major weight loss markedly improves glycaemic control. The common failure of classical treatments leads to the use of more aggressive weight-reduction approaches, such as very-low-calorie diets (VLCDs), anti-obesity drugs or even bariatric surgery. VLCDs are very successful in the short-term but rather disappointing in the long-term. Anti-obesity compounds only induce a modest mean weight reduction, even if some patients appear to be better responders. Interestingly, serotoninergic agents increase insulin sensitivity and gly

Psychological characteristics of dieters and bulimics.

Two studies examined the continuum and discontinuity models of the relation between dieting and bulimia nervosa. In Study 1, 21 dieters, 19 bulimics, and 33 nondieting controls were compared on 24 measures. Multivariate analyses revealed that each of the groups differed significantly from the other two. Univariate analyses found 18 significant differences between bulimics and dieters and 1 significant difference between dieters and controls. In Study 2, 86 subjects completed measures of psychological functioning, dieting, and bulimia nervosa. On the basis of regression analyses using factor sc

Three-year follow-up data in overweight treatment.

Reassessed 3 years after the termination of a weight reduction program the body weight of former participants to control long-term efficiency of behavior-oriented weight reduction programs. Of 31 former participants, 21 could be rechecked. Most of the Ss stabilized their body weight or continued to reduce weight. Only 4Ss regained weight over the 3-year period. On an average, the group of former Ss lost 4.4 kilograms over the follow-up period. The whole reduction (therapy time and follow-up) resulted in 11.3 kilograms. Three results are encouraging, but difficult to discuss because no member o

Increased meal-induced neurotensin response predicts successful maintenance of weight loss - Data from a randomized controlled trial.

BACKGROUND: The gut derived anorexigenic hormone neurotensin (NT) is upregulated after bariatric surgery which may contribute to the sustained weight loss. In contrast, diet-induced weight loss is most often followed by weight regain. We therefore investigated whether diet-induced weight loss impacts levels of circulating NT in mice and humans and whether NT levels predicts body weight change after weight loss in humans. METHODS: In vivo mice study: Obese mice were fed ad-libitum or a restricted diet (40-60 % of average food intake) for 9 days to obtain similar weight loss as observed in the

Intermittent fasting interventions to leverage metabolic and circadian mechanisms for cancer treatment and supportive care outcomes.

Intermittent fasting entails restricting food intake during specific times of day, days of the week, religious practice, or surrounding clinically important events. Herein, the metabolic and circadian rhythm mechanisms underlying the proposed benefits of intermittent fasting for the cancer population are described. We summarize epidemiological, preclinical, and clinical studies in cancer published between January 2020 and August 2022 and propose avenues for future research. An outstanding concern regarding the use of intermittent fasting among cancer patients is that fasting often results in c

Heterogeneity in the uptake, attendance, and outcomes in a clinical trial of a total diet replacement weight loss programme.

BACKGROUND: Trials have shown total diet replacement (TDR) programmes are safe and effective for weight loss in primary care. However, it is not clear whether participant characteristics affect uptake, attendance, or effectiveness of the programme. METHODS: We used data from 272 trial participants who were invited to participate in a clinical weight loss trial via a letter from their GP. We used a Cochran-Mantel-Haenszel analysis to assess whether accepting an invitation to participate in the trial differed by gender, age, BMI, social deprivation, and the presence of a diagnosis of type 2 dia

Experiences of using very low energy diets for weight loss by people with overweight or obesity: a review of qualitative research.

Although very low energy diets (VLEDs) are the most successful non-surgical, non-pharmacological treatment for obesity, they are underutilized, and little is known about experiences of people using VLEDs for weight loss. This systematic review synthesizes qualitative studies investigating participants' experiences of undertaking a VLED composed of total meal replacement products to lose weight. Of the 4,911 articles screened, three studies met criteria for inclusion. Thematic synthesis was used to analyse the study findings. Health and appearance were the main motivators to use a VLED for weig

Ventricular arrhythmias complicating weight reduction therapy in a patient with a prolonged QT interval.

Serious ventricular arrhythmias are known to occur in patients with long QT intervals. We describe a case of torsade de pointes occurring in a patient with a prolonged QT interval while taking a 1000 calorie diet, diethylpropion hydrochloride (Tenuate Dospan) and bendrofluazide. In patients with long QT intervals, hypokalaemia and drugs which further delay repolarization may facilitate the development of life threatening arrhythmias.
