Successful stent placement for cervical artery dissection associated with the Ehlers-Danlos syndrome. Case report and review of the literature.

This 44-year-old man with Ehlers-Danlos syndrome (EDS) Type IV presented with hemiparesis and the Gerstmann syndrome. Left carotid artery (CA) angiography revealed a dissecting aneurysm with severe stenosis located in the common CA; the lesion was successfully treated with a stent graft. The patient's clinical course after endovascular surgery was uneventful, without occurrence of megacolon. The literature for spontaneous CA dissection in EDS Type IV cases is reviewed and points for investigation and treatment are discussed.

[A surgical treatment of ruptured aortic dissection with non-opacified false lumen].

A 63-year-old woman was admitted to our intensive care unit suffering from severe chest pain and shock. Emergency CT scan demonstrated an acute type A aortic dissection with non-opacified false lumen and cardiac tamponade. The aortography showed ulcer like projection at the ascending aorta. An emergency operation was performed to replace the ascending aorta with a woven double-velour Dacron graft of 30 mm in diameter. It seems that an acute type A aortic dissection with non-opacified false lumen has good prognosis. The presence of other complications, however, suggests that surgical treatment

[Simplified operative technique for thoracoabdominal dissecting aneurysm].

A graft with branches is used to reconstruct the extensively dissected thoracoabdominal aorta. Anastomoses of the proximal end of the main graft distal to the left subclavian artery and the side branch to the distal aneurysmal descending thoracic aorta were first undertaken after the preparation of both ends under the iliac vein-artery partial bypass. A main graft with a Y graft extension was anastomosed distally to iliac arteries. This permits the blood flow within the aneurysm and the main graft to the extremities until splanchnic and spinal cord arteries have individually been transferred t

Fenestrated and branched endovascular aortic repair for chronic type B aortic dissection with thoracoabdominal aneurysms.

OBJECTIVE: The treatment of patients with arch and thoracoabdominal aortic aneurysms (TAAAs) and chronic dissections is challenging. We report the results of fenestrated and branched endovascular aortic repair (FEVAR) of such aneurysms. METHODS: A single-center prospective FEVAR trial enrolled 356 patients (2006 to 2011), of whom 30 had chronic dissections with arch aneurysm or TAAAs, or both. Patients were divided into group A, 15 patients (mean age, 58 years) with extensive dissections extending from the arch through the visceral segment, and group B, 15 patients (mean age, 74 years old) wi

Anatomy of the recurrent laryngeal nerve: a redescription.

The recurrent laryngeal nerves on each side of the neck consist of numerous branches. The demonstration of the diverse relationships of the recurrent laryngeal nerve in the thyroid area explains the variation in vocal cord and cricopharyngeal muscle paralysis produced by surgical or other trauma to this area. The anastomosis of the middle cervical ganglion with the recurrent laryngeal nerve adds a new dimension to the already confused theories on vocal cord and laryngeal function. Both the motor and sensory supply of the larynx inferior to the cords, lower pharynx, cervical oesophagus and cerv

Effects of Dacron graft amount used in type A aortic dissections on inflammatory markers and clinical approaches.

INTRODUCTION: Aortic dissection is characterized by an intimal tear resulting in blood flowing into the aortic media, creating a false lumen along the aorta. In acute type A aortic dissection surgery, grafts are used for ascending aortic replacement. METHODS: This retrospective study examined 113 patients who underwent surgery for type A aortic dissection at the Cardiovascular Surgery Clinic of Konya City Hospital between December 2020 and October 2025. After exclusion criteria, 62 patients were included in the study. The cut-off point for graft area was based on the median value of the datas

Delayed visceral malperfusion caused by kinking of a four-branched integrated frozen elephant trunk graft.

An integrated frozen elephant trunk with a four-branched vascular graft may shorten the circulatory arrest time during open distal anastomosis. We report a case of acute type A aortic dissection where postoperative graft kinking led to acute liver failure, kidney injury and lower limb ischaemia corrected by thoracic endovascular aortic repair.

Recipient Aortic Dissection After Orthotopic Heart Transplant: A Case Report.

Aortic dissection following orthotopic heart transplant is a rare but life-threatening event and typically occurs at the donor-recipient aortic anastomosis. In the case described here, a 40-year-old male patient, 10 years after he had received a heart transplant to treat dilated cardiomyopathy, presented with chest pain and dyspnea. Imaging revealed a chronic dissection that originated from the suture line between the donor and recipient aortas and extended to the iliac arteries. Notably, a mismatch was observed between the donor and recipient aortas during the initial transplant surgery. Alth

Combined fenestrated/chimney thoracic endovascular repair for the treatment of blunt traumatic aortic injury: A case report.

Blunt traumatic thoracic aortic injury (BTAI) is an extremely serious medical condition with a high rate of associated mortality. Recent advances in techniques such as thoracic endovascular repair offer new opportunities to manage the critical BTAI patients in an efficacious yet less invasive manner. A 65 year-old-male suffered from multiple injuries after a fall, including BTAI in the aortic arch, which resulted in dissection of the descending thoracic-abdominal aorta and iliac artery, development of an intimal flap in the left common carotid artery, and dissection of the left subclavian arte

Endovascular Treatment of Multiple Ruptures of Postdissecting Thoraco Abdominal Aortic Aneurysm with a Custom Branched Device Used as an Off-the-shelf Device.

Endovascular treatment of postdissection aortic aneurysms (PDAAs) is fascinating and challenging. The Colt branched graft (Jotec/Cryolife, Kennesaw, Georgia), because of its characteristics, can be used as an off-the-shelf device especially in urgent/emergency settings. In this report, we describe the first case of a PDAA successfully treated with a Colt device.

"Double Jacket Wrapping" Root Reconstruction for Acute Type A Aortic Dissection.

This report describes a simple and effective method of aortic root repair and reconstruction in acute type A aortic dissection to reduce the risks of hemorrhage and late aortic root new intimal tear and dilation. "Double jacket wrapping" (DJW) has 2 steps: the first jacket is used for aortic root "sandwich" repair, with a patch between the outer and inner layers; and the second jacket is wrapped outside the reconstructed root at the level of the supracoronary ostium. Compared with aortic root replacement, the DJW method may avoid prosthetic valve-related complications and decrease the complexi

Hostile Thoracic Aortic Aneurysm Treated by Fenestrated Thoracic Stentgraft with Proximal Sealing in Ishimaru Zone 0.

INTRODUCTION: Thoracic endovascular aortic aneurysm repair (TEVAR) is an established treatment for thoracic aortic disease in both the acute and elective setting, with such a widespread use that almost 50% of all thoracic aortic surgery in Europe is performed by these means. Nonetheless, the feasibility of TEVAR is determined by several anatomic factors, and the suitability of the proximal and distal landing zones remain one of the main limitations to its use. The advent of custom-made thoracic stent grafts widened the endovascular options in some challenging anatomies. METHODS: The authors p
