The available experimental and clinical data in the literature on the effect of antialcoholic drugs (disulfiram, cyanamide, pyrazol) on the lipid composition of tissues (blood serum, liver, brain) are systematized and generalized. Drugs used in the treatment of alcoholism were found to cause essential changes in the metabolism of lipid fractions. This indicates the necessity for recording the detected changes in lipid metabolism in pharmacotherapy of alcoholism.

In view of the many non-specific symptoms a diagnosis of alcoholism can only be made by means of a detailed history and a medical checkup. Simple, standardized questions may often be sufficient. After a diagnosis a consultation with a family physician and drugs, such as disulfiram, acamprosate and naltrexone have been proved to be a highly efficient therapy. Therefore, it is worthwhile to treat alcoholics in the general practice setting.

Acamprosate has a different mechanism of action than other drugs used for maintenance of abstinence from alcohol and is generally well tolerated. Diarrhea is the most common adverse effect. Its efficacy in controlled trials has been modest at best, and was poor when patients lacked social support for remaining abstinent. Acomprosate can be used in combination with naltrexone or disulfiram.

Disulfiram is commonly used in the treatment of alcohol dependence. It induces aversive symptoms after alcohol ingestion: the "acetaldehyde syndrome". Most often "acetaldehyde syndrome" is moderate but severe life-threatening symptoms may occur in specific circumstances. We report the case of a serious "acetaldehyde syndrome" with shock compounded by the initial administration of dopamine. Discussion clarifies the pathophysiology and develops the therapeutic management.
