Pylephlebitis of a variant mesenteric vein complicating sigmoid diverticulitis.

Pylephlebitis--suppurative thrombophlebitis of the portal and/or mesenteric veins--is a rare complication of abdominal infections, especially diverticulitis. It can lead to severe complications such as hepatic abscess, sepsis, peritonitis, bowel ischemia, etc., which increase the mortality rate. Here we present a case of suppurative thrombophlebitis of the inferior mesenteric vein, as a complication of sigmoid diverticulitis. The epidemiology, clinical and radiological features as well as treatment strategies are discussed. We also review the anatomy of the mesenteric vein given its anatomic v…

Two cases of Ruminococcus gnavus bacteremia associated with diverticulitis.

We report two cases of bacteremia with the anaerobic bacterium Ruminococcus gnavus. In both cases, the bacteremia was associated with diverticular disease. Preliminary conventional identification suggested peptostreptococci, and matrix-assisted laser desorption ionization-time of flight mass spectrometry (MALDI-TOF MS) analysis did not produce scores high enough for species identification. Finally, the bacteria were identified by 16S rRNA gene sequencing.

Timely diagnosis of perforated jejunal diverticulitis by computed tomography.

BACKGROUND: Perforated jejunal diverticulitis is an unusual entity that can present with acute abdominal pain. Due to the non-specific clinical findings, this condition is rarely diagnosed clinically. CASE REPORT: We present the case of a 75-year-old man in whom a perforated jejunal diverticulitis complicated by abscess formation was diagnosed by computed tomography (CT). He underwent a laparotomy with resection of the involved jejunal segment. He had an uneventful recovery and was subsequently discharged. CONCLUSIONS: This case report demonstrates the utility of CT in accurately diagnosing …

Chronic diverticulitis vs. colorectal cancer: findings on CT colonography.

PURPOSE: The purpose of this update article is to evaluate findings on CT colonography in patients with chronic diverticulitis and to compare the findings in patients with colorectal carcinoma. MATERIALS AND METHODS: Different morphological criteria retrieved from a literature review were retrospectively analyzed in a series of 13 patients with proven chronic diverticulitis. The findings were compared with a series of 10 patients with colorectal carcinoma. RESULTS: Overall, the findings in chronic diverticulitis resemble the findings in acute diverticulitis. The advantage of virtual CT colon…

Perforated Meckel's diverticulitis on the mesenteric side: MDCT findings.

Meckel's diverticulum (MD) is the most common congenital anomaly of the gastro-intestinal tract (approximately 2% of population), and arises from improper closure and absorption of the omphalomesenteric duct. Very few cases of Meckel's diverticulitis on the mesenteric side have been reported in the surgical literature, and no reported cases have been documented on preoperative imaging. We report a 65-year-old woman presenting symptoms and signs of acute abdomen with an initial suspicion of acute appendicitis. MDCT imaging revealed a mesenteric abscess in the right lower quadrant at the level o…

Nonoperative management of right colonic diverticulitis using radiologic evaluation.

OBJECTIVE: Although a few reviews have been conducted, nonoperative management may be the mainstay of therapy for uncomplicated right colonic diverticulitis. With increasing use of radiological evaluation for diverticulitis, the status of the disease is becoming more accessible. In this study, clinical outcomes of nonoperative management for right colonic diverticulitis were assessed according to disease status using radiological evaluation. METHOD: From April 2000 to March 2007, 296 patients were admitted for acute right colonic diverticulitis upon first attack and were treated with nonopera…

Coloduodenal fistula: a rare complication of right-sided diverticulitis.

Coloduodenal fistula is an uncommon disease entity. It can be caused by either a malignant or benign disease. We report the fifth case of coloduodenal fistula secondary to colonic diverticulosis. Our patient, a 60-year-old man, presented with severe diarrhoea and recurrent severe hypokalaemia. A partial colectomy with en bloc excision of the fistula was performed, and the duodenal defect was closed primarily. A literature review was carried out on the aetiology, presentation, diagnosis and management of coloduodenal fistula.

Laparoscopic sigmoid colectomy after acute diverticulitis: when to operate?

BACKGROUND: Laparoscopic sigmoid colectomy has become an acceptable method of surgical treatment for diverticulitis. However, an optimal waiting period before attempting elective laparoscopic colectomy has not been established. We sought to evaluate the relationship between the time interval from an acute episode of diverticulitis to laparoscopic colectomy and surgical outcomes. METHODS: All patients undergoing laparoscopic colectomy during a period of 10 years in a single institution were studied. Retrospectively collected data included patient demographics, American Society of Anesthesiolog…

[Diagnosis and surgical therapy of right-sided colonic diverticulitis].

Diverticulosis of the colon is common in the descending and sigmoid part; the right colon is rarely involved. Inflammation of perforation of … ealing was uncomplicated. A malignancy could be histologically excluded in all cases. As in sigmoid diverticulitis, early resection seems to be a safe operative therapy. Laparoscopy can show the correct diagnosis before the laparotomy is carried out. Preoperative diagnostic measures, differential diagnosis and possible surgical procedures are discussed.

[Colon diverticulitis: imaging diagnosis with ultrasound--a prospective study].

In a prospective study, we investigated the value of high resolution sonography for the imaging diagnosis of colonic diverticulitis. A total of 161 patients with abdominal complaints on account of which acute diverticulitis entered into differential diagnosis were included in the study. 74 of these finally had proven colonic diverticulitis (prevalence, 46%). The results of intestinal sonography and the working diagnoses on admission were evaluated on the basis of final diagnoses. According to history, symptoms, and clinical findings on admission, the working diagnosis of diverticulitis was gra…

One-stage repair of colovaginal fistula complicating acute diverticulitis.

Fourteen patients with colovaginal fistula secondary to sigmoid diverticulitis were seen between 1964 and 1988. Thirteen had undergone prior hysterectomy. Three different operative approaches were used. Three patients were treated with colostomy alone; one died and the fistula persisted in one. Five patients underwent staged procedures. One patient died of complications after the second stage of a planned three-stage procedure. Four patients underwent a two-stage procedure (fistula takedown, colectomy with colostomy and colostomy closure), all with good results. Six patients were treated with …

Primary resection without anastomosis for perforation of acute diverticulitis.

With perforation during acute diverticulitis, with or without abscess, the highest mortality occurs in patients in whom a transverse colostomy with drainage alone is done. This is no longer to be considered an acceptable method of management. The advantage of removal of the diseased intestine is paramount. Anastomosis under unfavorable conditions is avoided. The results in this group of severe, acutely ill patients with associated multiple systemic disease encouraged wider application of the Hartmann operation for the complex problems of inflammation of the diverticulum.

Laparoscopic lavage: an option for surgical management of complicated diverticulitis.

BACKGROUND: Acute diverticulitis with perforation and peritonitis is a serious complication affecting up to 12% of patients. Peritonitis is classified into purulent (Hinchey III) or fecal (Hinchey IV) categories. The standard treatment has traditionally involved emergency surgery, such as bowel resection with or without anastomosis or Hartmann's procedure, both of which carry high morbidity and mortality risks. METHODS: In 2008, laparoscopic peritoneal lavage (LPL) emerged as a less invasive alternative for treating purulent peritonitis. This article outlines the LPL technique, emphasizing pa…

Diverticulitis: A Review of Current and Emerging Practice-Changing Evidence.

Acute diverticulitis represents a common colorectal emergency seen in the Western world. Over time, management of this condition has evolved. This review aims to highlight recent evidence and update current recommendations. Notable evidence has emerged in certain aspects of diverticulitis. This includes disease pathogenesis, as emerging data suggest a potentially greater role for the microbiome and genetic predisposition than previously thought. Acute management has also seen major shifts, where traditional antibiotic treatment may no longer be necessary for acute uncomplicated diverticulitis.…
