Complicated small intestine diverticulitis Background Diverticula do not only occur in the colon but also in the small intestine albeit less frequently. Small intestine diverticula are usually asymptomatic. Acute complications, such as intestinal obstruction, bleeding or diverticulitis with or without abscess or perforation, are rare. Case description An 85-year-old male presented to the emergency department with abdominal pain since two weeks and a palpable mass in the upper left abdomen. Laboratory tests revealed significantly elevated infection parameters. Computed tomography imaging of the

This case report aims to demonstrate that it is not always necessary to resect the sigmoid colon in emergency surgery for perforated diverticulitis. We have found that in several cases, most of the sigmoid is in fact quite healthy, and the perforation is limited to a very small area. This is demonstrated by a recent case where a 44-year-old male presented to our care with severe sepsis and extensive peritonitis with abscess formation. At emergency surgery, the abscess was drained, but the abdominal sepsis was too extensive to permit safe resection and primary anastomosis. The sigmoid was sever

Jejunal diverticulitis is a rare condition that can present with an acute abdomen and be referred for imaging. We present the case of an elderly patient who at CT was diagnosed with an intra-abdominal abscess involving both jejunum and transverse colon. However, the underlying eitiology was not initially clear until small bowel barium follow-through.Pertinent points regarding CT findings in jejunal diverticulitis are discussed, and practical recommendations in small bowel diverticulum recognition and diagnosis are made.

Diverticulitis of the vermiform appendix is an often disregarded disease. Diagnosis follows after appendectomy due to inflammatory complications in the form of diverticulitis and appendicitis. Diverticula can also be found as a reason for complaints in histologically unaltered appendix or as an incidental finding. Diverticula of the vermiform appendix are classified as false (acquired) or true (congenital). We report on the case of a 57-year-old man with a perforated appendix diverticulum as reason for peritonitis and a paralytic ileus of the colon and small intestine.

OBJECTIVE: Diverticular bleeding is the most common cause of lower gastrointestinal bleeding. Arteriovascular disease, metabolic syndromes, non-steroidal anti-inflammatory drugs (NSAIDs), anti-thrombotics, and anticoagulants have been suggested as risk factors. There is a paucity of studies addressing factors associated with diverticular re-bleeding, especially in the United States. The aim of this study is to evaluate factors associated with colonic diverticular bleeding and re-bleeding in a US community-based hospital.
METHODS: We conducted a retrospective case-control study to analyze the

The majority of patients with acute diverticulitis can be managed medically. Some will have a complication of diverticulitis such as free perforation with peritonitis, abscess formation, obstruction, or fistula formation. Perhaps even a larger number will develop recurrent diverticulitis, which is associated with an increased rate of complications. Although the preoperative diagnosis of these problems may be obvious in many patients, elderly or steroid-treated patients may have few manifestations of significant intra-abdominal disease. Of extreme importance in the management of these complicat

Background Diverticula are small, sac-like protrusions that form along the gastrointestinal tract, most often in the sigmoid colon. While some individuals with diverticula remain asymptomatic, some patients experience symptoms, and a minority of patients go on to develop acute diverticulitis. The latter occurs when a fecalith becomes trapped within a diverticulum, leading to colonic wall damage, swelling, bacterial growth, and inflammation. A computed tomography (CT) scan is considered the gold standard imaging technique for diagnosing acute diverticulitis. However, the imaging characteristics

From 1978 to 1993, 220 patients were admitted at the Cl. Chirurgica III of the University of Bologna because of acute colonic diverticulitis. During the same period 108 patients (58 females and 50 males; mean age 65 years) were operated on for complications of the disease; 21 acute diverticulitis, 10 pericolic or pelvic abscess, 30 purulent peritonitis, 7 fecal peritonitis, 19 obstruction, 14 hemorrhage and 7 fistula. Surgical techniques were: resection and primary anastomosis (n. = 46), two or three stage procedure (n. = 49), Hartmann's resection (n. = 13). Mortality rate was 15%, 14,2% and 7

The most common cause of colonic obstruction is adenocarcinoma, followed by diverticulitis, volvulus, and a variety of miscellaneous causes. Most signs and symptoms, from whatever cause, consist of abdominal pain with distention and the inability to pass flatus or stool. The clinical diagnosis is confirmed by x-ray studies. Plain films of the abdomen in various positions, chest films, and the addition of contrast studies verify the cause of the obstruction in most instances. The differentiation between neoplasm and diverticulitis causing the obstruction can be difficult or impossible at times,

A 31-year-old male with a history of diverticulitis presented for acute abdominal pain and was found to have several small areas of free air on computed tomography (CT) of the abdomen/pelvis. Due to inflammatory changes seen around the sigmoid colon and small bowel, he was diagnosed with perforated diverticulitis. The patient complained of significant right-sided abdominal pain with significant tenderness on abdominal examination. The patient was initially treated with diagnostic laparoscopy and was actually found to have acute perforated appendicitis with mild appendiceal adherence to the sig

Colo-salpingeal fistulae due to acute diverticulitis are rare entities. A 65-year-old female with a 5-day history of left iliac fossa pain, fevers and diarrhoea was seen in a metropolitan hospital. Initial computed tomography of the abdomen and pelvis characterized a left tubular adnexal structure with an air fluid level and normal-appearing colon and was reported as pyosalpingitis. Worsening sepsis despite antibiotics prompted further imaging where thickening of the sigmoid colon was noted, and the provisional diagnosis was revised to complicated acute diverticulitis with perforation into the

Transverse taeniamyotomy has been in use for over four years in the surgical correction of the muscle abnormality of diverticular disease. Essentially, the technic is to carefully transversely incise the two antimesenteric taeniae coli at 2 cm. intervals, in a stepwise manner, from the rectosigmoid junction proximally up to normal colon. This causes the cut ends of the taeniae coli to pull apart into separate 2 cm. blocks of longitudinal muscle. Thus elongation and widening of the colon occurs, resulting in dilatation of the necks and free drainage of the diverticula obstructed by inflammation

Acute diverticulitis is one of the leading gastrointestinal causes for hospitalization. The incidence of acute diverticulitis has been increasing in recent years, especially in patients under 50 years old. Historically, acute diverticulitis in younger patients was felt to represent a separate entity, being more virulent and associated with a higher rate of recurrence. Accordingly, young patients were often managed differently to older counterparts. Our understanding of the natural history of this condition has evolved, and current clinical practice guidelines suggest age should not alter manag

Internal fistulization of the colon to other organs, such as the urinary bladder, vagina, or small bowel is a relatively common complication associated with inflammatory diseases such as diverticulitis, Crohn disease, as well as neoplasia such as colorectal cancer or lymphoma. However, fistulization of the colon to the spleen is an exceedingly rare condition described by few in the literature.

AIM: The aim of this work was to compare the results of elective minimally invasive surgery between patients with complicated sigmoid diverticulitis and those with uncomplicated disease.
METHOD: An institutional review board-approved database was searched for all consecutive patients who underwent elective minimally invasive surgery, including laparoscopic, hand-assisted and robotic sigmoidectomy, for diverticulitis between 2010 and 2017; they were classified according to the modified Hinchey classification as having complicated (abscess, fistula, stricture, obstruction, bleeding or previous

Appendiceal diverticulosis is a rare condition. Herein reported is a case of appendiceal diverticulosis and diverticulitis clinically masquerading as appendiceal carcinoma. A 62-year-old woman presented with abdominal pain. US and CT showed a tumor measuring 5 × 4 × 4 cm in vermiform appendix. Colon endoscopy showed mucosal elevation and irregularity in the orifice of vermiform appendix. A biopsy of the appendiceal mucosa showed no significant changes. Clinical diagnosis was appendiceal carcinoma and wide excision of terminal ileum, appendix, cecum, and ascending colon was performed. Grossly,

We report 12 cases of right-sided diverticulitis from January 1985 to May 1995 at our community hospital. The preoperative diagnosis of appendicitis was made in 11 of these patients. Only one patient was correctly diagnosed preoperatively. The mean age of patients at presentation was 48 years (range, 34-69). Six patients were male, and six were female. Eight patient (73%) presented with fever, seven (64%) had symptoms for greater than 24 hours, and 100 per cent of the patients had an elevated white blood cell count. All of the patients underwent surgery, without bowel preparation. In all cases

The correlation between the haustral pattern in the sigmoid colon, on the one hand, and the pressure activity and presence of diverticula, on the other, was examined in 184 patients, subjected to both X-ray examination and sigmoid pressure measurement. No correlation was found between an increased haustral pattern and a high pressure activity. Only a special S-shaped haustral pattern showed a slight correlation to an increased pressure activity. An interpretation of colonic haustral pattern in terms of muscular activity, therefore, seems unwarranted. A significant correlation was found between
