Effects of the COVID-19 pandemic on SCUBA diving experience in marine protected areas.

UNLABELLED: Faced with the need for isolation of most people to reduce the transmission of COVID-19, a great concern for the well-being of the population has resurfaced. Considering the numerous benefits of outdoor activities to human health and well-being, we assessed whether the quality of the experience of recreational divers in a marine protected area (MPA) was impacted during the pandemic. We applied a semi-structured questionnaire to divers, collected their socioeconomic profile, preferences, motivations, experiences, and compared the results with those obtained in the previous season. W

Vascular Reactions of the Diving Reflex in Men and Women Carrying Different ADRA1A Genotypes.

The diving reflex is an oxygen-saving mechanism which is accompanied by apnea, reflex bradycardia development, peripheral vasoconstriction, spleen erythrocyte release, and selective redistribution of blood flow to the organs most vulnerable to lack of oxygen, such as the brain, heart, and lungs. However, this is a poorly studied form of hypoxia, with a knowledge gap on physiological and biochemical adaptation mechanisms. The reflective sympathetic constriction of the resistive vessels is realized via ADRA1A. It has been shown that ADRA1A SNP (p.Arg347Cys; rs1048101) is associated with changes …

Food safety risk perceptions and mitigation techniques in the dumpster diving community in Sweden.

An online questionnaire was used to investigate behaviors and habits relating to food retrieval by Swedish dumpster divers. Respondents were also asked to share information on their background, reasons for engaging in dumpster diving, and perceptions of potential bacterial hazards associated with the consumption of discarded foods. The questionnaire was mainly distributed to Swedish social media groups focusing on dumpster diving, and a total of 92 responses were received. The most common reason given for engaging in dumpster diving was to reduce food waste and associated negative environmenta…

First report of Yokenella regensburgei isolated from external auditory canal after diving in valley.

Yokenella regensburgei is a Gram-negative, oxidase-negative motile rod which is rarely isolated from human caused a few opportunistic infections in immunocompromised patients so far. We report the first case of otitis media combined with externa caused by Y. regensburgei in an immunocompetent patient. A 56-year-old male patient visited the outpatient clinic of the Otolaryngology Department due to otorrhea of the right ear started after diving in mountain valley in Korea 3 days ago. He was immunocompetent adult and clinical examination revealed swelling and debris accumulation in the right exte…

Electrical Stimulation of the Infraorbital Nerve Induces Diving Reflex in a Dose-Controlled Manner.

The "diving reflex" (DR) is a very powerful autonomic reflex that facilitates survival in hypoxic/anoxic conditions and could trigger multifaceted physiologic effects for the treatment of various diseases by modulating the cardiovascular, respiratory, and nervous systems. The DR can be induced by cold water or noxious gases applied to the anterior nasal mucosa and paranasal regions, which can stimulate trigeminal thermo- or chemo-receptors to send afferent signals to medullary nuclei which mediate the sympathetic and parasympathetic nervous systems. Although promising, these approaches have ye…

Abdominal decompression illness following repetitive diving: a case report and review of the literature.

The complete pathophysiology of decompression illness is not yet fully understood. What is known is that the longer a diver breathes pressurized air at depth, the more likely nitrogen bubbles are to form once the diver returns to surface [1]. These bubbles have varying mechanical, embolic and biochemical effects on the body. The symptoms produced can be as mild as joint pain or as significant as severe neurologic dysfunction, cardiopulmonary collapse or death. Once clinically diagnosed, decompression illness must be treated rapidly with recompression therapy in a hyperbaric chamber. This case

Vomiting and aspiration of gastric contents: a possible life-threatening combination in underwater diving.

Vomiting and aspiration of gastric contents into the airways and lungs is a common and well-known clinicopathological entity. This phenomenon might also occur in underwater diving, where it can lead to life-threatening or fatal situations. This article presents two incidents involving diving-related vomiting with associated aspiration of gastric contents. One case, a 39-year-old commercial diver using a full-face diving mask, was fatal and the other was a 33-year-old, female recreational scuba diver in whom underwater vomiting was complicated by pulmonary aspiration of a solid foreign body (a …

Bone Mineral Density and Osteoporotic Vertebral Fractures in Traditional, Unassisted, Free-Diving Women (Haenyeos).

BACKGROUND: Water pressure and muscle contraction may influence bone mineral density (BMD) in a positive way. However, divers experience weightlessness, which has a negative effect on BMD. The present study investigated BMD difference in normal controls and woman free-divers with vertebral fracture and with no fracture. METHODS: Between January 2010 and December 2014, traditional woman divers (known as Haenyeo in Korean), and non-diving women were investigated. The study population was divided into osteoporotic vertebral fracture and non-fracture groups. The BMD of the lumbar spine and femora…

Prevalence of cardiomegaly and left ventricular hypertrophy in scuba diving and traffic accident victims.

Although frequently asymptomatic, left ventricular hypertrophy (LVH) is an independent predictor of sudden cardiac death (SCD). We hypothesized that diving may increase the propensity for pre-existent LVH to cause a lethal arrhythmia (and SCD) and therefore the prevalence of LVH may be greater among scuba fatalities than among traffic fatalities. We compared autopsy data for 100 scuba fatalities with 178 traffic fatalities. Extracted data contained information on age, sex, height, body mass, heart mass (HM), left ventricular wall thickness (LVWT), interventricular wall thickness (IVWT), and de…

Cerebellar infarction presenting as inner ear decompression sickness following scuba diving: a case report.

INTRODUCTION: Inner ear decompression sickness following scuba diving is not uncommon and the characteristic features of this disorder are acute peripheral vestibular syndrome, sometimes associated with cochlear signs, requiring urgent hyperbaric oxygen therapy. Cerebellar infarction can also mimic isolated peripheral vestibulopathy. CASE REPORT: The authors report the case of a 47-year-old man in good general health admitted with acute left vestibular dysfunction suggestive of inner ear decompression sickness 6 hours after scuba diving. Normal videonystagmography and delayed onset of occipit…

Congenital pericardial cyst in a naval special warfare candidate; clearance for diving after resection.

INTRODUCTION: We report the case of a 19-year-old male military recruit who presented for a screening physical for U.S. Naval Special Warfare and Diving Duty. During his screening physical examination, an exophytic pericardial cyst was discovered. Subsequent work-up revealed normal cardiopulmonary function despite this large 7-cm mass, but the candidate was disqualified due to concerns regarding the risk of complications. He underwent successful elective surgical resection without post-operative complications. One year post-operatively, he repeated his cardiopulmonary work-up with normal resul…

The nontechnical causes of diving accidents: can U.S. Navy divers learn from other industries?

Although U.S. Navy diving is remarkably safe, because of the high-risk environment in which military divers work, accidents and mishaps do occur. Failures in leadership and situation awareness (particularly in risk and time assessment) were found to be the two most common causes of fatal and nonfatal U.S. Navy diving accidents and near misses. Responses to an attitude survey showed that junior divers want to ask questions, but senior divers do not desire to be questioned. In other high reliability industries (e.g. aviation, medicine) methods have been developed to identify, analyze and mitigat…

CNS oxygen toxicity in closed-circuit diving: signs and symptoms before loss of consciousness.

INTRODUCTION: There is a dearth of information regarding CNS oxygen toxicity accidents in closed-circuit oxygen diving. The aims of the present study were to report the sensations and symptoms that accompany CNS oxygen toxicity accidents, and to evaluate whether loss of consciousness can occur without any warning signs. METHODS: We documented 36 CNS oxygen toxicity accidents in closed-circuit oxygen diving. The full accident inquiry included the first report from the diving unit, an interview of the victim and his buddy by the researchers, and an examination of the diving equipment. RESULTS:…

Facial baroparesis secondary to middle-ear over-pressure: a rare complication of scuba diving.

A facial nerve palsy, as a result of middle-ear high pressure, is a rare complication of sub-aqua diving. It may occur as a result of an acute pressure change in the middle ear during ascent in those patients who have experienced difficulty equalizing their middle-ear pressure during the prior descent. We present the case history of this occurring in a 21-year-old diver and discuss the pathophysiology, management and the previous literature. The correct diagnosis of this condition is important if unnecessary, and potentially hazardous, recompression treatment is to be avoided.
