In the last three decades, the management of human immunodeficiency virus (HIV) has improved dramatically. The use of combination antiretroviral therapy (ART) has been successful at preventing death and the myriad infectious, malignant, and immune-mediated complications of HIV. Once considered to be a fatal disease, HIV is now considered by many to be a chronic disease; those affected may now live to experience complications of other coexistent diseases. Liver disease has been increasingly recognized as a leading cause of non-HIV/acquired immunodeficiency syndrome- (AIDS-) related morbidity an

The Kidney Donor Risk Index (KDRI) and Kidney Donor Profile Index (KDPI) assist clinicians with the selection of deceased donor kidneys. This scoring system is based on 10 donor factors including Hepatitis C virus (HCV) status from serological or NAT testing. The donor HCV status (i.e., having either a positive hepatitis C antibody (Ab) or nucleic acid testing (NAT) result) increases the hazard ratio for graft failure by 1.27 and the KDPI by approximately 20%. Whether this increase in KDPI is a true reflection of graft quality for HCV seropositive but not viremic donors is unknown. Further inv

With a growing discrepancy between the number of liver candidates and donors, traditional donor selection criteria have been greatly changed to meet the need for donor livers. Older donors are a readily available source of livers that can significantly augment the donor pool. Our series demonstrated that the course and the outcome of orthotopic liver transplantation were not influenced by donor age, as indicated by the absence of significant differences in the incidence of primary nonfunction, arterial and biliary complications, postoperative function tests, and graft and patient survival. The…

Eligibility criteria in blood donation are essential to protect both donor and recipient health. This study examines variability in donor selection criteria and donor health questionnaires across 19 healthcare institutions in Lebanon amid its transition toward a voluntary and non-remunerated blood donation (VNRD) system. Notable discrepancies were identified across medical, behavioral, demographic, and geographic deferral categories. A comparative analysis against Lebanese Committee for Blood Transfusion guidelines and EU-based standards reveals a lack of centralization and inconsistent adhere…

We report 2 cases of donor-derived Strongyloides stercoralis infection in renal transplant recipients. Despite initial negative serologic testing in donor samples, retrospective testing confirmed transmission. This report underscores the limitations of serologic screening, the need for targeted protocols in endemic-risk populations, and the importance of close posttransplant surveillance.

BACKGROUND: Over time, changes to the deferral policy for males who had sex with other males (MSM) allowed those reporting past MSM to donate if other eligibility criteria were met. We assessed the return rate after the removal of the MSM deferral and transfusion-transmissible infectious disease (TTID) rates in MSM compared to first-time male donors and donors overall.
STUDY DESIGN AND METHODS: Males presenting to donate at the American Red Cross and self-reported MSM between December 13, 2016 and December 31, 2024 were assessed. TTID rates were determined by evaluating all donors with donati

End-stage liver disease (ESLD), stemming from a spectrum of chronic liver pathologies including chronic liver failure, acute cirrhosis decompensation and hepatocellular carcinoma, imposes a significant global healthcare burden. Liver transplantation (LT) remains the only treatment for ESLD. However, the escalating mortality on transplant waitlists has prompted the utilization of marginal liver grafts in LT procedures. These grafts primarily encompass elderly livers, steatotic livers, livers from donation after circulatory death, split livers and those infected with the hepatitis virus. While t

Few case reports have documented the long-term outcomes of liver donor aneurysms, illustrating the apprehension of transplant surgeons about using these grafts. However, the presence of an aneurysm in the donor liver should not be an absolute contraindication for its use. As shown in our described patient, such grafts have the potential to achieve good results.

In 2019 the For The Assessment Of Individualised Risk (FAIR) project began a review of UK blood donor selection policy to determine if a more individualised approach to donor selection could be safely implemented. An evidence base was required to inform selection policy to move from a population to a more individual based policy, specifically what sexual behaviours/indicators should be considered as screening questions to maintain the safety of the blood supply. Eight sexual behaviours/indicators were reviewed: history of bacterial sexually transmitted infections (STIs), chemsex, number of rec…

OBJECTIVE: To propose a rational basis for donor testing in cases of suspected antibody-mediated transfusion-related lung injury (AMT).
BACKGROUND: Anti-leukocyte antibodies in donated blood are established causes of transfusion-related lung injury (TRALI). However, the question of whether to test donors for antibodies is not identical to whether the case meets definition criteria for TRALI. There is a balance between the potential benefits of testing and the costs of donor deferral and investigation. We propose that a decision-making process based on optimising the balance between risk and b

BACKGROUND AND OBJECTIVES: Trypanosoma cruzi is the etiologic agent of Chagas disease (CD), an anthropozoonosis from the American continent that progresses from an acute phase to an indeterminate phase, followed by a chronic symptomatic phase in around 30% of patients. In countries where T. cruzi is not endemic, many blood transfusion services test blood donors who have stayed in an endemic country ('at-risk stay')-even if they do not present with other risk factors. However, the efficiency of this approach has been questioned.
MATERIALS AND METHODS: On 18 September 2023, a worldwide survey w

Even as record numbers of deceased donors are undergoing organ recovery, the global transplant community continues to struggle with a shortage of donor organs and a high organ discard rate. Acute kidney injury (AKI) occurs in many hospitalized patients, including up to 25% of patients in critical condition. Registry studies have shown a significant increase in nonrecovery or organ discard rates in AKI donors, despite most studies reporting similar clinical outcomes compared with non-AKI donors. This review aims to capture the salient information learned from these studies and to summarize the

BACKGROUND: The numberof patients awaiting heart transplantation (HTx) substantially exceeds the number of donor hearts transplanted each year, yet nearly 65% of eligible donor hearts are discarded rather than transplanted.
METHODS: Deceased organ donors listed within the UNOS Deceased Donor Database between 2010 and 2020 were reviewed. Those greater than 10 years old and consented for heart donation were included and randomly separated into training (n = 48 435) and validation (n = 24 217) cohorts. A discard risk index (DSRI) was created using the results of univariable and multivariable ana

Heart transplantation (HTx) is a treatment option for end-stage heart failure in children. HTx is limited by the availability and acceptability of donor hearts. Refusal of donor hearts has been reported to be common with reasons for refusal including preexisting donor characteristics. This review will focus on the impact of donor characteristics and comorbidities on outcomes following pediatric HTx. A literature review was performed to identify articles on donor characteristics and comorbidities and pediatric HTx outcomes. There are many donor characteristics to consider when accepting a donor

BACKGROUND: Targeted donor screening for strongyloidiasis performed at the time of organ procurement can prevent this life-threatening donor-derived infection.
METHOD: The Association of Organ Procurement Organizations surveyed members to determine the number of US organ procurement organizations (OPOs) performing donor screening for Strongyloides infection and their screening practices.
RESULTS: All 58 OPOs responded to the survey. Only 6 (10%) currently screen donors for strongyloidiasis; most OPOs started 6-36 months before the survey and one started 6 years prior. All used risk-based cri

BACKGROUND: Selection of appropriate donors after rigorous evaluation is of paramount importance in living-donor liver transplantation. Despite this, donor surgery may not proceed due to unforeseen reasons. The aim of this paper is to study reasons for "no go" donor hepatectomy in living liver donors.
PATIENTS AND METHODS: Donor operations stopped after surgical start, directly due to donor safety-related reasons, qualified for inclusion as "no go" donor hepatectomy. Living-donor evaluation was performed as per standard protocol. Data for consecutive living liver donors operated between April
