Application of antimicrobial drugs in Mycobacterium tuberculosis and research progress.

Treatment regimens for Tuberculosis (TB) primarily rely on first-line drugs such as isoniazid, rifampicin, ethambutol, and pyrazinamide. Although these drugs have been pivotal in the treatment of drug-resistant tuberculosis, their high costs limit widespread use in low- and middle-income countries. However, the emergence of multidrug-resistant TB and extensively drug-resistant TB necessitates the development of new anti-TB drugs and therapeutic strategies. In terms of treatment, new drugs such as bedaquiline, delamanid, and pretomanid have shown significant efficacy against MDR-TB and XDR-TB.

Runx2 Deficiency in Osteoblasts Promotes Myeloma Resistance to Bortezomib by Increasing TSP-1-Dependent TGFβ1 Activation and Suppressing Immunity in Bone Marrow.

Multiple myeloma is a plasma cell malignancy that thrives in the bone marrow (BM). The proteasome inhibitor bortezomib is one of the most effective first-line chemotherapeutic drugs for multiple myeloma; however, 15% to 20% of high-risk patients do not respond to or become resistant to this drug and the mechanisms of chemoresistance remain unclear. We previously demonstrated that multiple myeloma cells inhibit Runt-related transcription factor 2 (Runx2) in pre- and immature osteoblasts (OB), and that this OB-Runx2 deficiency induces a cytokine-rich and immunosuppressive microenvironment in the

Heterogeneity of Candida bloodstream isolates in an academic medical center and affiliated hospitals.

Invasive Candida bloodstream infections (candidemia) are a deadly global health threat. Rare Candida species are becoming increasingly important causes of candidemia. Phenotypic data, including patterns of antifungal drug resistance, are limited. There is a geographic variation in the distribution of Candida species and frequency of antifungal drug resistance, meaning that regional data can have significant clinical value. Here, we report on the species distribution, frequency of antifungal drug resistance, and phenotypic variability of Candida bloodstream isolates from an academic medical cen

Overcoming drug resistance in multi-drug resistant cancers and microorganisms: a conceptual framework.

Resistance development against multiple drugs is a common feature among many pathogens--including bacteria such as Pseudomonas aeruginosa, viruses, and parasites--and also among cancers. The reasons are two-fold. Most commonly-used rationally-designed small molecule drugs or monoclonal antibodies, as well as antibiotics, strongly inhibit a key single step in the growth and proliferation of the pathogen or cancer cells. The disease agents quickly change or switch off this single target, or activate the efflux mechanisms to pump out the drug, thereby becoming resistant to the drug. A second prob

Natural Products as Modulators of ABC Transporters in Breast Cancer.

Breast cancer remains a significant global health challenge, with high incidence and mortality rates despite advancements in early detection and treatment. Multidrug resistance (MDR), particularly in aggressive subtypes like triple-negative breast cancer (TNBC), continues to hinder effective therapy. MDR is primarily driven by the overexpression of ATP-binding cassette (ABC) transporters such as P-gp, BCRP, and MRP1. ABC transporters actively efflux chemotherapeutic agents, reducing their efficacy and complicating treatment outcomes. This narrative review explores the potential of natural prod

Genomic analysis of IncH plasmids reveals their role as drivers of antimicrobial resistance and adaptive traits in enterobacterales.

Plasmids of the incompatibility group H (IncH) are large mobile elements that confer multidrug resistance and are prevalent in Enterobacterales from clinical and environmental sources. We analyzed 1308 globally distributed IncH plasmid sequences to assess their genomic features and functional potential. IncH plasmids were classified into IncHI1 and IncHI2, with IncHI1 subdivided into IncHI1B and IncHI1AB based on co-occurring replication proteins. These subtypes exhibited distinct host preferences and genomic patterns. IncH plasmids carried antimicrobial resistance genes and other adaptive det

Genomic determinants across cross- and co-resistance events of the environmental resistome of Klebsiellapneumoniae.

The emergence and spread of multidrug resistance (MDR) in Klebsiella pneumoniae are significantly influenced by environmental factors; yet, little is known about the full range of resistance gene diversity in environmental settings. In the present study, we characterized the resistome and co- and cross-selection dynamics of publicly available environmental K. pneumoniae genomes. We identified metal/biocide-resistant genes (MRGs/BRGs) and antibiotic-resistance genes (ARGs) in K. pneumoniae isolates. We then assessed co-resistance (occurrence of ARG-MRG/BRG pairs on the same plasmid) and cross-r

Phenotypic and molecular characterization of Vibrio cholerae from aquatic products.

Vibrio cholerae, a waterborne pathogen, causes global cholera pandemics. Data on its prevalence and antimicrobial resistance in Chinese aquatic products are limited, highlighting an urgent need for systematic surveillance. We isolated fifteen V. cholerae (25.86%, 15/58) strains isolated from Macrobrachium rosenbergii (giant freshwater prawn) collected in Zhejiang Province, China, between 2024 and 2025. The antimicrobial resistance phenotype and genotype of V. cholerae strains were characterized. Ampicillin (100%), colistin (100%), trimethoprim-sulfamethoxazole (93.33%), ceftiofur (93.33%), cef

Novel heterospirocyclic antimalarials with activity against artemisinin- and multidrug-resistant P. falciparum malaria.

Malaria is an infectious disease that imposes a significant global health burden. Increasing drug resistance creates an urgent demand for novel treatment options. We have previously synthesised a new class of heterospirocyclic compounds with novel chemical linkages, which have shown preliminary antimalarial activity. Compounds 25 and 26 display antimalarial activity within 24 h against a panel of drug-resistant strains of Plasmodium falciparum, the most virulent of human malaria parasites. Untargeted metabolomics analysis of P. falciparum-infected red blood cells revealed that the mechanism of

Overcoming Cancer Multi-drug Resistance (MDR): Reasons, mechanisms, nanotherapeutic solutions, and challenges.

Multi-drug resistance (MDR) in cancer cells, either intrinsic or acquired through various mechanisms, significantly hinders the therapeutic efficacy of drugs. Typically, the reduced therapeutic performance of various drugs is predominantly due to the inherent over expression of ATP-binding cassette (ABC) transporter proteins on the cell membrane, resulting in the deprived uptake of drugs, augmenting drug detoxification, and DNA repair. In addition to various physiological abnormalities and extensive blood flow, MDR cancer phenotypes exhibit improved apoptotic threshold and drug efflux efficien

High prevalence of antimicrobial resistance and multidrug resistance among bacterial isolates from diseased pets: Retrospective laboratory data (2015-2017).

Laboratory surveillance and the monitoring of antimicrobial resistance (AMR) trends and patterns among local isolates have been highly effective in providing comprehensive information for public health decision-making. A total of 396 cases along with 449 specimens were received for antibiotic susceptibility testing at a public university veterinary diagnostic laboratory in Malaysia between 2015 and 2017. Escherichia coli was the most frequently isolated (n = 101, 13%) bacteria, followed by Staphylococcus pseudintermedius (n = 97, 12%) and Streptococcus canis (n = 62, 8%). In cats, S. pseudinte

High incidence of multi-drug resistance and heterogeneity of mobile genetic elements in Escherichia coli isolates from diseased ducks in Sichuan province of China.

Harmonious ecological environment is a major concern with rising feeding and consumption of ducks, as these waterfowl birds can promote the spread of antibiotic resistant genes (ARGs). Therefore, this study was conducted to know diversity of antimicrobial resistance (AMR), integrons, and mobile genetic elements (MGEs) in Escherichia coli (E. coli) isolated from intestinal contents or pericardial effusion of diseased ducks from 2018 to 2020 in Sichuan, China. The AMR phenotype was determined via disk diffusion test in 165 E. coli isolates. Further, the integrase genes of integron (intI1, intI2

Multiple mechanisms contributing to ciprofloxacin resistance among Gram negative bacteria causing infections to cancer patients.

Fluoroquinolones have been used for prophylaxis against infections in cancer patients but their impact on the resistance mechanisms still require further investigation. To elucidate mechanisms underlying ciprofloxacin (CIP) resistance in Gram-negative pathogens causing infections to cancer patients, 169 isolates were investigated. Broth microdilution assays showed high-level CIP resistance in 89.3% of the isolates. Target site mutations were analyzed using PCR and DNA sequencing in 15 selected isolates. Of them, all had gyrA mutations (codons 83 and 87) with parC mutations (codons 80 and 84) i

The chemical biology of new drugs in the development for tuberculosis.

With the worldwide emergence of multidrug-resistant (MDR) and extensively drug-resistant (XDR) strains of Mycobacterium tuberculosis (Mtb), there are serious concerns about the continued ability to contain this disease. We discuss the most promising new drugs in late-stage development that might be useful in treating MDR and XDR forms of the disease. These agents have novel mechanisms of action that are not targeted by the standard drugs used presently to treat susceptible strains.

Conflicting requirements of Plasmodium falciparum dihydrofolate reductase mutations conferring resistance to pyrimethamine-WR99210 combination.

Plasmodium falciparum strains bearing quadruple mutations of dihydrofolate reductase-thymidylate synthase (PfDHFR-TS) at codons 51, 59, 108, and 164 are highly resistant to pyrimethamine (PYR), a diaminopyrimidine, but sensitive to WR99210 (WR), a cycloguanil analog, suggesting different enzyme-inhibitor binding interactions. A combination of these inhibitors to delay the onset of antifolate resistance is proposed. Using error-prone PCR, libraries of random mutants of wild-type PfDHFR and PfDHFR-TS were generated and used to transform Escherichia coli, and transformants were then selected for

Efflux-mediated antimicrobial resistance.

Antibiotic resistance continues to plague antimicrobial chemotherapy of infectious disease. And while true biocide resistance is as yet unrealized, in vitro and in vivo episodes of reduced biocide susceptibility are common and the history of antibiotic resistance should not be ignored in the development and use of biocidal agents. Efflux mechanisms of resistance, both drug specific and multidrug, are important determinants of intrinsic and/or acquired resistance to these antimicrobials, with some accommodating both antibiotics and biocides. This latter raises the spectre (as yet generally unre

Antimicrobial resistance pattern and plasmid profile of Salmonella typhi isolated from an outbreak in Tehran province.

The antimicrobial resistance patterns and plasmid profiles of Salmonella typhi isolates from sporadic cases (n = 33) and an outbreak (n = 48) were compared. Of 28 sporadic drug-resistant isolates, 24 (85.7%) were multiply resistant. The predominant antibiotic resistance pattern was TeCmSmSxTAp, which was also the most common pattern of the outbreak isolates. 13 drug-resistant strains isolated before the outbreak (46.4%) were able to transfer the whole resistance pattern or part of it to Escherichia coli K 12 by conjugation. Although 20 of the sporadic strains contained plasmid DNA, transferabl

Assessing emergence risk of double-resistant and triple-resistant genotypes of Plasmodium falciparum.

Delaying and slowing antimalarial drug resistance evolution is a priority for malaria-endemic countries. Until novel therapies become available, the mainstay of antimalarial treatment will continue to be artemisinin-based combination therapy (ACT). Deployment of different ACTs can be optimized to minimize evolutionary pressure for drug resistance by deploying them as a set of co-equal multiple first-line therapies (MFT) rather than rotating therapies in and out of use. Here, we consider one potential detriment of MFT policies, namely, that the simultaneous deployment of multiple ACTs could dri
