[Results of microsurgery in ear malformations].

Small middle ear malformations consist of bony bridges compromising the vibration possibilities of the ossicular chain. In 17 out of 21 cases the air bone gap could be closed by microsurgery. The results in surgery of extended ear malformations with microtia, atresia of the outer ear and a malformated middle ear were not as good. In cases with aerated middle ear space the air bone gap could be reduced to 20 dB in 32 out of 45 cases (71%). When there was no air in the middle ear fenestration of the horizontal semicircular canal was performed. An improvement of hearing could be measured in 3 out…

[Accessory external ear in the oropharynx].

This report describes the case of a one-day old girl with an accessorial ear lobe in the region of the left soft palate. The girl was referred to us by the children's clinic because of shortness of breath and difficulties in swallowing. On inspecting the cavity of the mouth and the pharynx a pedunculated, ball-shaped, whitish, membranous tumour was identified. The size of the tumour was comparable to a cherry, hanging down from the left velum into the pharynx. It was possible to remove the tumour from its base by using the sling technique. The operation itself did not present any complications…

Morning pressure in the middle ear.

Tympanograms were taken in the early morning in 25 persons with healthy ears. A first tympanogram was obtained shortly after waking up while the subject was still recumbent, and a second tympanogram was taken in the upright position after chewing and swallowing. Positive middle-ear pressure was found in a majority of ears, and the pressure was reduced after swallowing. The present results indicate that gas absorption from the middle ear is not important in quantitative terms during sleep. Otherwise the morning pressure would be expected to be negative. Two other experiments showed that the mid…

[When should an ear not be operated on].

Despite the low morbidity of modern middle ear surgery there are some limitations in indication. The age of the patient, the function of the Eustachian tube, the fibrotic middle ear, the cleft palate, the glomus tumour, Menière's disease and "the last hearing ear" of the patient are discussed as far as middle ear surgery is concerned.

The chronic discharging ear in Nigeria.

A study of 386 Nigerian patients with chronic suppurative otitis media is presented. The disease usually starts in early life, even though presentation in hospital may be delayed until adult life. The important aetiological factors were found to be essentially socio-economic. Tubo-tympanic disease dominates the clinical picture and the rarity of the attic retraction type of chronic ear disease would suggest exceptionally good eustachian function in the population studied, but further study is required in this respect. It is suggested that complicated tympano-plastic procedures are in general i…

Spontaneous encephalocele of the middle ear.

Mature glial tissue was seen in the middle ear of a patient with no previous pathologic precedents, and a clinical diagnosis of serous otitis media was made. That anomaly was detected, and its corresponding clinical, roentgenographic, and pathologic findings are described. The results led us to diagnose it as a congenital (spontaneous) encephalocele of the middle ear. To our knowledge, other case of pure congenital encephalocele of the middle ear has been reported.

Trauma of the ear from infrasound.

Twenty-eight chinchillas received continuous or intermittent infrasound (1, 10, 20 Hz) at 150, 160, or 170 dB SPL; there were 7 controls. Serial sections of the temporal bones were examined using light microscopy. Pathologies noted were tympanic membrane perforation, stapes subluxation, bleeding from the middle ear mucosa and tensor tympani, strial pathology, Reissner's membrane rupture, endolymphatic hydrops, saccular wall rupture, hair cell damage, and blood in the cochlear scalae. Continuous infrasound was more damaging than intermittent. Only continuous infrasound produced saccular patholo…

Verrucous carcinoma of the middle ear.

A case of a highly destructive, cytologically nondysplastic squamous epithelial lesion of the middle ear is presented. The cranial nerve involvement and bone destruction are more extensive than has been seen in cholesteatoma. Cultures are negative for Pseudomonas, and the patient does not have the reported diathesis for malignant otitis externa. The gross and microscopic features are those of verrucous carcinoma. To our knowledge, the middle ear has not been previously reported as a site of involvement by verrucous carcinoma.

Reliability of the middle ear examination.

Interobserver reliability of physical signs elicited and diagnoses made on middle ear examination were studied. Effect on reliability of variation in observer knowledge, patient age and instrument were defined. Two board-certified, color-seeing pediatric specialists who worked in the same clinic examined consecutively 350 ears with the same otoscope for each case. Over a period of four months, four different otoscopes were used to examine at least 50 ears with each. The following groups of findings were graded independently by the pediatrician according to pre-set criteria: color, landmarks an…

[Diabetes and diseases of the internal ear].

The authors present a series of 30 cases of vertigo where diabetes and hyperlipidemia were found. The literature on the subject is reviewed, and evidence points to a diabetic lesion of the inner ear. Our cases illustrate that lesion and its clinical presentation.

The contralateral ear in Méniére disease.

We conducted a survey of 292 patients who had Meniere disease to determine whether there was either some subjective complaint like tinnitus or hearing impairment or an objective symptom like hearing loss or recruitment of the contralateral ear. In 212 (73%) patients, signs of disturbed hearing in the second ear were present. If only the combination of sensorineural hearing loss, recruitment, and tinnitus is accepted as a definite proof of the diagnosis of Meniere disease, at least 10% of the patients we studied suffer from bilateral Meniere disease. Usually, there is an interval between the on…

A method of stapedectomy in the fenestrated ear.

Two consequences of a previous fenestration militate against successful stapledectomy. They are the lessened depth of the middle ear over the stapes, and the missing incus. Twelve years' experience of using the stapedectomy for fenestrated ears in 62 patients has given generally disappointing results until, in 1962, technique of using cartilage blocks to increase the depth of the middle ear was devised.

Primary adenocarcinoma of the middle ear.

Primary adenocarcinoma of the middle ear is rare. It may or may not be associated with chronic suppurative otitis media. After the exclusion of a primary site elsewhere, the tumor should be managed by surgery and postoperative radiotherapy, especially if tumor excision is incomplete. We report a case in which primary papillary adenocarcinoma developed in the middle ear, possibly due to aberrant ceruminous gland elements. The findings prior to histologic examination were unusual in that they were consistent with a tumor of the glomus tympanicum or glomus jugulare.
