Histopathological study on temporal bone and eustachian tube in trisomy 22.

Two temporal bone-eustachian tube (ET) specimens, one from a 34-gestational week-old male newborn and the other from a 3-month-old female infant, both with trisomy 22, were studied histopathologically. Both cases demonstrated severe inner-ear anomalies including hypoplastic cochlea, atrophy of the saccular macula, and dislocated endolymphatic sac and vestibular aqueduct. In addition, they had several anomalies in the ET including widely developed medial lamina of the ET cartilage, undeveloped lateral lamina of the ET cartilage, and absence of attachment of the tensor veli palatini muscle to th

Techniques for improving ear definition in microtia reconstruction.

Surgeons involved in microtia repair recognize the difficulty in creating a natural appearing ear. One key to successful reconstruction is to provide sufficient relief between the helix, scaphoid fossa and antihelix to create the illusion of thin skin overlying thin cartilage. Problems such as thick skin, hair-bearing skin and poor-quality cartilage serve to frustrate the surgeons attempt to achieve the desired result. Surgical techniques to improve cartilaginous framework definition in microtia repair are discussed.

Arteriovenous Malformation MAP2K1 Mutation Causes Local Cartilage Overgrowth by a Cell-Non Autonomous Mechanism.

Extracranial arteriovenous malformation (AVM) is most commonly caused by MAP2K1 mutations in the endothelial cell. The purpose of this study was to determine if local tissue overgrowth associated with AVM is caused by direct or indirect effects of the MAP2K1 mutation (i.e., cell-autonomous or cell-non autonomous). Because cartilage does not have blood vessels, we studied ear AVMs to determine if overgrown cartilage contained AVM-causing mutations. Cartilage was separated from its surrounding tissue and isolated by laser capture microdissection. Droplet digital PCR (ddPCR) was used to identify

The bacteriology of children prior to 1st stage autologous ear reconstruction.

In virtually all surgical specialities the use of peri-operative antibiotic prophylaxis to minimise wound site infection is routine practice. Antibiotic selection is targeted towards the pathogens most commonly encountered at the surgical site. The surgical management of microtia is by autologous rib cartilage reconstruction, a process that involves at least two surgical stages. The pits and recesses of the microtia ear are difficult to clean and may shelter unusual pathogens not routinely found as skin commensals, requiring modified prophylaxis. This retrospective review of 37 patients underg

Traumatic separation of the annular cartilage from the external auditory meatus in a cat.

A 13-year-old cat was evaluated because of anorexia and otic discharge of 1 week's duration. A left head tilt and left facial nerve deficit were evident. Pink fleshy tissue was seen in the area where the tympanic membrane was expected during otoscopic examination. The left ear canal was laterally displaced from the skull on a rostrocaudal (open mouth) radiographic view, suggesting ear canal separation. During surgery, the annular cartilage was found to be separated from the external auditory meatus, and total ear canal ablation was performed. Complications did not develop, and facial nerve fun

Could ionizing radiation forestall cauliflower ear?

Repeated trauma to the ear very often results in "cauliflower ear." Many methods have been suggested to prevent an injured ear from demonstrating a cauliflowerlike deformity. The principles of treatment are evacuation of the hematoma, control of the reaccumulation of fluid, and maintenance of the cartilage contour. The authors studied the effect of ionizing radiation on deformed rabbit ears induced by repeated trauma. Twenty ears (10 rabbits) were used in the experiment. The animals were divided into four groups (control, preradiation, low dose, and high dose). Hematoma was produced by poundin

An anatomical investigation of alkaptonuria: Novel insights into ochronosis of cartilage and bone.

Ochronotic pigmentation of connective tissue is the central pathological process in the rare metabolic disease alkaptonuria (AKU). Tissue pigmentation in AKU occurs due to unmetabolised homogentisic acid (HGA) in the circulation, caused by an enzyme deficiency in the liver. Ochronotic pigmentation, derived from HGA, has previously been reported and described in large joints obtained from arthroplasty surgeries, which typically have advanced disease. Many tissues that are affected by ochronosis are not accessible for study during life, including tissues subjected to early and mid-stage disease.

Rat ear transplantation: a feasibility study.

The feasibility of rat ear replantation and its inherent advantages as an experimental model have previously been demonstrated. The purpose of this study was to validate the technical feasibility of rat ear transplantation. To eliminate immunologic rejection as a complicating factor, highly inbred Lewis rats (strain LEW/CRIBR) were utilized. The external, internal, or common carotid artery served as the arterial pedicle, and the posterior facial vein as the venous pedicle; tubal cartilage was anastomosed for structural support. Four of five transplanted ears were viable at 10 to 14 weeks posto

Histopathologic study of congenital aural atresia in the human embryo.

We studied the histopathologic features of the temporal bones in a human embryo with unilateral aural atresia. The developmental stage of the embryo was at stage 22 in the Carnegie system, and the estimated ovulation age was 8 weeks. There were severe hypoplastic changes in Meckel's and Reichert's arch cartilages without differentiation of the auditory ossicles, hypoplasia of the tubotympanic recess, and resultant abnormal passing of the facial nerve in the affected ear. Abnormal lateral extension of the cartilaginous otic capsule replaced a posterior half of the middle ear region and seemed t

[Experiences with the palisade technic of tympanoplasty].

This article analyses the advantages of the autogenous cartilage from the tragus split into multiple parallel pieces for the closure of big tympanic perforations and for the reconstruction of the middle ear after treatment of ventilation diseases like tympanic retraction or atelectasis. The cartilage is more stable than any other material and gives a better protection against recurrent retraction process. Dispersion into fractioned pieces produces very good functional results, as has been shown in this study.

Status of the Microbiological Isolates and Antibiotics Susceptibilities in Patients with Auricular Perichondritis.

Auricular perichondritis is a dangerous infection involving the external ear cartilage and its perichondrium. This condition can lead to significant morbidity including cartilage necrosis, deformity and the characteristic "cauliflower ear" if not properly diagnosed and treated in a timely manner. The infection typically develops following trauma, burns, insect bites, ear surgery, or ear piercings through the cartilaginous portion of the ear. Objective of the study was to analyze the microbiological profile and antibiotic susceptibility patterns in auricular perichondritis and to evaluate treat…

Cartilage Conduction Hearing Aids in Clinical Practice.

A relatively loud sound is audible when a vibrator is attached to the aural cartilage. This form of conduction is referred to as cartilage conduction (CC). In Japan, a new type of hearing aid has been developed using CC and has been available in clinical practice since 2017. A clinical study conducted prior to its launch demonstrated its benefits, particularly in patients with aural atresia who were unable to use air conduction hearing aids. Several studies have been published on the benefits of CC hearing aids since their introduction into clinical practice. Most of the patients included in t

Attic reconstruction techniques in cholesteatoma surgery: a comparative analysis of cartilage versus bone pate.

BACKGROUND: Cholesteatoma surgery involves canal wall down (CWD) and canal wall up (CWU) mastoidectomy. CWU is associated with higher cholesteatoma recurrence, often linked to attic retraction pockets. Attic reconstruction with cartilage or bone pate lacks comparative evidence. AIMS/OBJECTIVES: To compare the effectiveness of cartilage and bone pate in attic reconstruction during CWU mastoidectomy for cholesteatoma. MATERIAL AND METHODS: We conducted a retrospective study at King Abdullah University Hospital (KAUH) in Jordan, analyzing surgeries performed from 2011 to 2021. Patients who unde

Elevation of a constructed auricle using the anteriorly based mastoid fascial flap.

Two-stage methods for reconstruction of congenital microtia have been widely utilised. To obtain a desirable auriculocephalic angle and provide a nutrient support to the constructed auricle, elevation of reconstructed ears using a costal cartilage graft, the anteriorly based mastoid fascial flap transfer and a skin graft was performed as the second operation for nine microtia patients. In this procedure, the mastoid fascial flap was used instead of the temporoparietal fascial flap. Following the elevation of the reconstructed ear the anteriorly based mastoid fascial flap was harvested. A carve

Correction of congenital microtia using the tissue expander.

We attempted auricular reconstruction using Radovan-type inflatable silicone expanders in six children and one adult, with the complete hypoplastic, the conchal remnant, and constricted type of microtia. Ear frameworks, including the helix, anthelix, concha, and tragus, were prepared using autologous rib cartilage. Based on the surface area of the normal adult auricle, the silicone expander was tentatively shaped and sized into a rotated semiellipse and expanded with 70 cc saline. Auricular reconstruction on the framework was completed at the time of insertion in four of the seven patients, re

Pulling Strings: Otoplasty Complicated by Wound Dehiscence in the Setting of Prolonged Mask Wearing.

Improved understanding of the psychosocial effects of prominent ears has led to increased intervention in early school-aged children. Cartilage-sparing otoplasty procedures are commonly used in this population, given the effectiveness of sutures in remodeling the pliable pediatric ear. In addition to their efficacy, another advantage of these techniques is their low overall complication rates, with complications beyond 3 months being particularly uncommon. We present the clinical case of a 5-year-old female patient who experienced wound dehiscence 9 months after her cartilage-sparing otoplasty
