Transient increase in nuchal translucency thickness and reversed end-diastolic ductus venosus flow in a fetus with trisomy 18.

In a twin pregnancy discordant for trisomy 18, the affected fetus at 13 weeks' gestation had an increased nuchal translucency thickness and reversed end-diastolic ductus venosus flow. At 20 weeks' gestation there was no nuchal edema and Doppler study of the central venous vessels demonstrated normal waveforms. The findings support the hypothesis that one of the mechanisms in the development of increased nuchal translucency is temporary cardiac strain.

Beyond hypertrophy: four-chamber myocardial involvement in ATTR cardiac amyloidosis diagnosed by multimodality imaging.

Background: Cardiac amyloidosis results from the deposition of amyloid fibrils in the myocardial extracellular matrix, leading to restrictive cardiomyopathy and heart failure. Diagnosis has traditionally been challenging owing to non-specific clinical features and reliance on endomyocardial biopsy. Advances in cardiovascular imaging have, however, improved early detection. Case summary: We present a 74-year-old man with progressive dyspnea on exertion and bilateral pedal edema of three months' duration. Cardiac amyloidosis was suspected on the basis of discordance between electrocardiographic

Permanent Pacemaker Timing Dilemma in Recovered Lymphocytic Myocarditis.

BACKGROUND: The timing of permanent pacemaker implantation in myocarditis-related complete heart block remains challenging because no established criteria exist for reversibility of the conduction system. CASE SUMMARY: A previously healthy 50-year-old Asian woman presented with lymphocytic myocarditis complicated by cardiogenic shock and complete atrioventricular block (CAVB). Despite successful hemodynamic stabilization with Impella CP support and complete recovery from left ventricular dysfunction, CAVB persisted. A permanent pacemaker was implanted on day 28. Afterward, cardiac enzymes and

Delayed Diagnosis of Cardiac Amyloidosis Secondary to Multiple Myeloma.

Cardiac amyloidosis is a rare condition with only 2,500 new cases reported yearly in the United States of America (USA). The prognosis of cardiac amyloidosis is particularly grim. The median survival is 6 months from onset of congestive heart failure (CHF) symptoms. CHF is a common presentation as the second leading cause of hospitalization in the senile population in the USA. We report a case of an 83-year-old man who presented with the classic symptoms of CHF including bilateral lower extremity edema, shortness of breath, and weight gain. Upon further workup, an echocardiogram revealed strai

Delayed post-operative cardiac tamponade manifesting as cardiogenic shock.

We describe the case of a 32-year-old woman with a history of cardiac surgery two weeks previously who presented with an upper gastrointestinal bleed. She also had symptoms of syncope and abdominal pain, as well as elevated liver enzymes. She had cool, clammy extremities, bilateral lower extremity edema, and oliguria. An echocardiogram was obtained, which revealed a large posterior pericardial effusion with evidence of tamponade physiology, and an emergent pericardiocentesis was performed with insertion of a drainage catheter, which drained bloody fluid, with subsequent hemodynamic improvement

[Congestive heart failure due to chronic alcoholism (author's transl)].

In rare instances, chronic alcoholism leads to the congestive heart failure which is characteristic of alcoholic beriberi with edema. The affected patients is usually a young man with longstanding alcoholic intoxication and often with neurologic features. Onset is often sudden, with dyspnea on exertion, orthopnea, and palpitations. The clinical sings of cardiac failure are unequivocal. Roentgenography shows cardiomegaly mainly due to enlargement of the right cavities and of the pulmonary artery. ECG shows sinus tachycardia and abnormal repolarisation in the precordium. The cardiac output and t

[Chronic cardiac actinomycosis. Case report and review of the literature].

We report the case of a young man in whom a right paracardiac mass was resected. The histopathologic study was compatible with actinomycosis. He developed severe cardiac failure, pulmonary edema and pericardial effusion. He was treated with penicillin, during 12 months. He did well afterwards, however, the echocardiographic follow-up revealed chronic pericardial effusion and thickening and fusion of the pericardium to the epicardial wall.

A case of nonimmune hydrops fetalis with a rare cardiac anomaly in a rhesus monkey.

A case of nonimmune hydrops fetalis in a rhesus monkey was identified by ultrasound. The 68-day fetus exhibited generalized edema, pleural effusion, and mild ascites. Intrauterine fetal demise occurred between 75 and 80 days gestation. Necropsy revealed marked anasarca and a rare cardiac anomaly characterized by aortic and left atrioventricular valve atresia, hypoplasia of the ascending aorta and arch, and absence of the left ventricle.

Association of Urinary Plasminogen-Plasmin with Edema and Epithelial Sodium Channel Activation in Patients with Nephrotic Syndrome.

BACKGROUND: Previous animal experiments and small human studies suggest that urinary plasmin can activate the epithelial sodium channel (ENaC) and contribute to sodium retention in nephrotic syndrome (NS), but this however is not well studied in clinical settings, and its relevance to edema formation is not well characterized in humans. We have investigated the association between urinary plasmin and clinical phenotypes in a large group of patients with NS from multiple etiologies, aiming to assess the role of urinary plasmin in sodium handling and edema formation. METHODS: Two hundred and th

Cardiac function and survival are affected by crude oil in larval red drum, Sciaenops ocellatus.

Following exposure to weathered and non-weathered oil, lethal and sub-lethal impacts on red drum larvae were assessed using survival, morphological, and cardiotoxicity assays. The LC50 for red drum ranged from 14.6 (10.3-20.9) to 21.3 (19.1-23.8) μgl-1 ΣPAH with no effect of exposure timing during the pre-hatch window or oil weathering. Similarly, morphological deformities showed dose responses in the low ppb range. Cardiac output showed similar sensitivity resulting in a major 70% reduction after exposure to 2.6μgl-1 ΣPAH. This cardiac failure was driven by reduced stroke volume rather than b

Causes and consequences of 93 fetuses with cardiomegaly in a tertiary center in Thailand.

OBJECTIVES: To assess etiology, perinatal mortality and associated factors of fetal cardiomegaly (FC). METHODS: A retrospective study of fetuses with cardiomegaly was conducted. Demographic data, ultrasonographic findings, and outcomes were collected and analyzed. RESULTS: Ninety-three fetuses were analyzed. The causes of FC were cardiac causes 49.5%, Bart's hemoglobinopathies 28%, and non-Bart's anemia 15%. Ascites, pericardial effusion, and hydrops were more prevalent in fetuses with anemia than those with cardiac abnormalities. The overall perinatal mortality was 69.9%. Although all Bart'

Is the ultrasound definition of fluid collections in non-immune hydrops fetalis helpful in defining the underlying cause or predicting outcome?

The ultrasound findings in 132 patients with non-immune hydrops referred for echocardiographic assessment were retrospectively reviewed. Structural assessment was combined with evaluation of pleural and pericardial effusions, ascites and skin edema. Patients were divided into groups based on the underlying etiology of hydrops. Seventy patients were found to have cardiac abnormalities, including 45 with structural heart disease. Other causes included. chromosomal anomalies, infection, extra-cardiac anomalies, genetic syndromes and, in 40 patients, unknown cause. Analysis of patterns of fluid ac

Systemic complications of fluid resuscitation.

Fluid administration in critically ill individuals is frequently a major component of their therapy. There are important effects on blood pressure and maintenance of cardiac output and oxygen delivery, as detailed elsewhere in this text. There are also potentially negative side effects of this therapy, which have been less well defined. Edema of the gastrointestinal tract has been well described, primarily with crystalloid infusions. Gastrointestinal edema may have very complicated effects on albumin kinetics, fluid flux, and ion flux. It may lead to development of ileus. Increased nasogastric

Pulmonary Edema and Stunned Myocardium in Subarachnoid Hemorrhage.

Aneurysmal subarachnoid hemorrhage is a life-threatening event that can cause permanent disability. This life-threatening event can be further complicated by subsequent cardiac and pulmonary disability. The presence of a neurogenic cardiomyopathy and pulmonary edema increases the morbidity and mortality of patients who suffer from aneurysmal subarachnoid hemorrhage.  In this paper, we discuss a 39-year-old woman who presented to the emergency department (ED) with a chief complaint of a pounding headache with associated nausea and vomiting for the past three days. She had a past medical history

Lymphedema of the breast as a symptom of internal diseases or side effect of mTor inhibitors.

A common situation presented in any clinical facility is a woman with swelling and redness of the breast. Diagnosis upon suspicion is often mastitis or inflammatory breast cancer, which are popular and well-known diseases of the breast. However, there is one main differential diagnosis which has to be taken into consideration: lymphedema of the breast. Twenty patients with internal diseases presented in our Breast Care Unit over a 4-year period with breast-affecting lymphedema. The patients suffered from cardiac failure, nephrotic syndrome, liver failure, lymphadenopathy, and central vein occl

Post coital hemoptysis.

Ischemic mitral regurgitation is a complication of coronary artery disease specifically in the settings of prior myocardial infarction. It is a dynamic lesion and its severity may increase with different forms of exercise leading to flash pulmonary edema. We present the case of an 81-year-old man with ischemic mitral regurgitation who presented with long standing history of hemoptysis limited to sexual intercourse only. After a thorough negative work-up for the usual causes of hemoptysis, comprehensive right and left cardiac catheterization studies revealed marked increase in baseline mitral r

Ventricular Function and Tissue Characterization By Cardiac MRI in Children Following Hospitalization for Multisystem Inflammatory Syndrome in Children (MIS-C): A Prospective Study.

Background Multisystem Inflammatory Syndrome in Children (MIS-C) is a severe life-threatening manifestation of SARS-CoV-2 infection. Acute cardiac dysfunction and resultant cardiogenic shock are common in children with MIS-C. While most children recover rapidly from acute illness, the long-term impact on the myocardium and cardiac function is unknown. Methods In this prospective study, cardiac MRI (CMR) was performed on patients <21 years of age with a history of MIS-C, 6-9 months following hospitalization. Per institutional protocol, patients with any history of LVEF<50%, persistent cardiores
