[Training Present and Future Doctors for Chile: How Many and For What? The Urgent Need to Review the Physician Quota Generation Model].

Before 1990, Chile showed fewer medical schools, six in total, increasing to 13 by 2000. From then on, there was an explosive increase, reaching 29 faculties or schools by 2024. Medical students increased from 1,816 in 2010 to 3,000 by 2024. The number of doctors in the country in 2010 was 24,455, rising to 65,737 by 2023, of which 25% are foreign professionals. In 2010, there was a ratio of 1.43 doctors per 1000 inhabitants, increasing to 3.13 by 2024, with a reasonable projection for 2034 close to 6, in other words, one doctor for every 170 inhabitants, reaching the OECD curve, surpassing WH

Internally versus externally trained residents and fellows hired as attendings at a large integrated healthcare system: a 20-year retrospective study.

BACKGROUND: There remains a question of whether graduates trained internally are different than those trained elsewhere. We examine the difference between physicians trained within our Graduate Medical Education (GME) programs versus physicians trained elsewhere. Our large integrated healthcare system is unique in addressing this objective due to its large physician labor hiring needs across different specialties of GME graduates. METHODS: A retrospective review was performed from Jan 2000 to August 2020 of Kaiser Permanente Southern California (KPSC) physicians hired: KPSC GME trained versus

Longitudinal trends in volume of oculoplastic procedures without ACGME minimum requirements among United States ophthalmology residents: an ACGME case log analysis.

PURPOSE: To our knowledge, there have been no previous reports that investigate trends in ophthalmology residents' exposure to oculoplastic procedures without Accreditation Council for Graduate Medical Education (ACGME) minimum requirements. To fill this knowledge gap, we investigated publicly available ACGME ophthalmology residency case logs from 2009 to 2021. METHODS: National resident averages and standard deviations were collected for the following oculoplastic procedures without minimum ACGME requirements: eye removal and implant, lacrimal surgery, other orbital surgery (e.g. orbitotomy)

Variability in Hand Surgery Training Among Plastic and Orthopaedic Surgery Residents.

BACKGROUND: A career in hand surgery in the United States requires a 1-year fellowship after residency training. Different residency specialty programs may vary in case volume. The purpose of this study was to characterize variation in hand surgery training within and between orthopaedic and plastic surgery residents. METHODS: Publicly available hand surgery case logs for graduating orthopaedic and plastic surgery residents during the 2010 to 2011 to 2018 to 2019 academic years were obtained through the Accreditation Council of Graduate Medical Education. Student t-tests were used to compare

Imaging Informatics Fellowship Curriculum: Building Consensus on the Most Critical Topics and the Future of the Informatics Fellowship.

The existing fellowship imaging informatics curriculum, established in 2004, has not undergone formal revision since its inception and inaccurately reflects present-day radiology infrastructure. It insufficiently equips trainees for today's informatics challenges as current practices require an understanding of advanced informatics processes and more complex system integration. We sought to address this issue by surveying imaging informatics fellowship program directors across the country to determine the components and cutline for essential topics in a standardized imaging informatics curricu

How the COVID-19 Pandemic Has Affected Cardiology Fellow Training.

With the advent of the COVID-19 pandemic in the United States, resources have been reallocated and elective cases have been deferred to minimize the spread of the disease, altering the workflow of cardiac catheterization laboratories across the country. This has in turn affected the training experience of cardiology fellows, including diminished procedure numbers and a narrow breadth of cases as they approach the end of their training before joining independent practice. It has also taken a toll on the emotional well-being of fellows as they see their colleagues, loved ones, patients or even t

The Fellowship Effect: Does Surgical Subspecialty Training Affect Pediatric Surgery Case Volume?

PURPOSE: The Accreditation Council for Graduate Medical Education (ACGME) monitors fellow case numbers. Previous literature has shown new fellowships detrimentally affecting general surgery residency case numbers. We hypothesize that an increase in pediatric otolaryngology and pediatric urology fellowships would decrease overlapping case numbers in pediatric surgery fellowships. MATERIALS AND METHODS: A review of each specialties' fellowship match results was performed. Publicly available pediatric surgery cases logs were reviewed for overlapping cases including thyroidectomy, parathyroidecto

Craniofacial Skills: A 2-Site Validation of Assessments to Aid Plastic Surgery Resident Milestone Achievement in Technical Skills and Instrument Knowledge.

BACKGROUND: Plastic surgery evaluates residents on milestones. This study defines a model of education including pre and post-test assessments paired with didactics intended for evaluating residents in the unique technical skills of craniofacial surgery. METHODS: At the first institution, instrument identification, and time/accuracy of burr hole placement, craniotomy, and plating on Saw Bones Craniofacial Models were tested before and after a 7.5-hour craniofacial orthognathic surgery workshop. At the second institution, this was refined, removing plating, eliminating assessment of timing, an

Supporting New Physicians and New Parents: A Call to Create a Standard Parental Leave Policy for Residents.

Parents taking leave after the birth of a child is associated with significant benefits for infants, mothers, and fathers. Although nearly 40% of residents have or plan to have children during residency, there is no standard parental leave policy for these trainees. In this Perspective, the authors discuss the benefits of parental leave, synthesize findings about maternity bias and other negative effects of the current variable approaches to parental leave during residency, and explore underlying causes of the lack of a standard parental leave policy for residents. They also call on the Accred

Assessing the role of virtual reality training in Canadian Otolaryngology-Head & Neck Residency Programs: a national survey of program directors and residents.

BACKGROUND: Given mounting pressure of work hour restrictions, resource constraints, and variability of clinical exposure, Otolaryngology-Head & Neck Surgery (OHNS) residency training has shifted away from the apprenticeship model to embrace the Royal College of Physicians and Surgeons of Canada's "Competence by Design" initiative. As a result, appraising both current and potential educational adjuncts has become increasingly important. In this investigation, a national needs assessment survey was performed to identify strengths, weaknesses, and future opportunities of the current training lan

Guidelines for fellowship training in regional anesthesiology and acute pain medicine: third edition, 2014.

Directors for Regional Anesthesiology and Acute Pain Medicine fellowships develop and maintain guidelines for fellowship training in the subspecialty. The first edition of the guidelines was published in 2005 with a revision published in 2010. This set of guidelines updates the 2010 revision. The guidelines address 3 major topics: organization and resources, the educational program, and the evaluation process.

Incentives for scholarly activity at university and independent general surgery residencies: current practices.

BACKGROUND: Teaching physicians and academic medical centers may find it more difficult to meet clinical productivity expectations and still contribute to scholarly activity in the present economic climate of health care. METHODS: A multiquestion survey was developed and distributed via the Association of Program Directors in Surgery list-serve. RESULTS: There were 80 respondents (31% response rate), 29 university program (UP), 43 independent program (IP), and 8 "other." Although most programs had designated teaching faculty (72% UP, 93% IP), the trend was not to compensate for scholarly act

Attitudes of speech and language therapists towards stammering: 1985 and 2000.

BACKGROUND: Past research has indicated that speech and language therapists hold some negative attitudes towards people who stammer, their parents and the treatment of stammering. However, studies on attitudes towards stammering have predominantly focussed on therapists in the USA. Recent trends towards earlier intervention suggest that more therapists in the UK will be involved in working with stammering. AIMS: This study aimed to gather current attitudes of therapists in the UK and to compare them with attitudes measured 15 years previously. It also considered the impact of postgraduate tra

AIDA and medical courseware.

For more than a decade the Department of Medical Informatics has offered one-week training courses on the subject of computer applications in medicine and health care. Since 1983 two courses are given at a rate of one course every two weeks. One course is on programming and problem solving and consists of three modules of increasing complexity in techniques and methods in programming and structured system development. This course focusses on only some aspects of medical informatics: the development of a medical information system, and the problems occurring in the process of automation. These

A retrospective study of attitude change during medical education.

A retrospective attitude questionnaire was administered to 70 of 157 graduating seniors at the end of the medical school year. Students perceived that they became more cynical during medical education. In addition, they perceived that they were more concerned with making money, more concerned for patients, and more helpful. These findings relate to the developmental stressors of medical education as they affect the individual student. Burn-out and impairment are possible consequences; a preventive approach is advocated.

Breast Fellows Forum: A Multidisciplinary Core Curriculum Model for Surgical Oncology Fellowships.

Subspecialty fellowship training is becoming more commonplace and medical practice more multidisciplinary. However, specialized training programs are challenged to provide a core curriculum that covers the interdisciplinary aspects of their field. Here we present a joint, interdisciplinary, interactive didactic program for breast fellowships in surgery, pathology and imaging. A yearlong core curriculum composed of topics of relevance to breast fellows involved in diagnostics and clinical care was created. Two hour long fellow-run interactive in-person sessions occur monthly. Additionally, facu
