Implanted microelectrode arrays hold immense therapeutic potential for many neurodegenerative diseases. However, a foreign body response limits long-term device performance. Recent literature supports the role of astrocytes in the response to damage to the central nervous system (CNS) and suggests that reactive astrocytes exist on a spectrum of phenotypes, from beneficial to neurotoxic. The goal of our study was to gain insight into the subtypes of reactive astrocytes responding to electrodes implanted in the brain. In this study, we tested the transcriptomic profile of two reactive astrocyte

BACKGROUND: Deep brain stimulation (DBS) is an established treatment for patients with medical refractory movement disorders with continuously increasing use also in other neurological and psychiatric diseases. Early and late complications can lead to revision surgeries with partial or complete DBS-system removal. In this study, we aimed to report on our experience with a frameless x-ray-based lead re-implantation technique after partial hardware removal or dysfunction of DBS-system, allowing the preservation of intracerebral trajectories.
METHODS: We describe a surgical procedure with comple

BACKGROUND: Cardiac troponins are routinely used as markers of myocardial damage. Originally, they were only intended for use in diagnosing acute coronary syndromes; however, we now know that raised serum troponin levels are not always caused by ischemia. There are many other clinical conditions that cause damage to cardiomyocytes, leading to raised levels of troponin. However, the specificity of cardiac troponins towards the myocardium is absolute. Our work focuses on mechanical damage to the myocardium and on monitoring the factors that raise the levels of cardiospecific markers after primo-

BACKGROUND: Stereoelectroencephalography (SEEG) is a well-established technique for localizing epileptogenic zones in patients with drug-resistant epilepsy, including children. While considered safe, rare but serious complications can occur.
CASE PRESENTATION: We report the case of a girl with tuberous sclerosis complex and drug-resistant epilepsy who underwent SEEG implantation as part of pre-surgical evaluation. Electrode placement was uneventful. However, during SEEG monitoring, the signal was lost on two electrodes, raising suspicion of fracture. During explantation, these electrodes were

BACKGROUND: The purpose of this study was to develop an effective method of reducing metal artifacts in cochlear implant (CI) electrodes.
METHODS: The temporal bones of 30 patients (34 ears) after CI were examined with 320-detector row computed tomography, which was evaluated by two senior radiologists using a double-blind method. Noise, artifact index, signal-to-noise ratio, and the subjective image quality score were compared before versus after using single-energy metal artifact reduction (SEMAR). The electrode position, single electrode visibility, and electrode count were evaluated using

PURPOSE OF REVIEW: Frequency-to-place mismatch is the discrepancy between the frequency information delivered by a cochlear implant electrode and the natural cochlear place frequency location that is stimulated. Frequency-to-place mismatch has been shown to negatively influence cochlear implant outcomes. Anatomy-based fitting (ABF) aligns filter frequencies with cochlear frequency place to minimize or eliminate frequency-to-place mismatches. This article reviews the current evidence on variability in electrode array placement, influence of frequency-to-place mismatches on cochlear implant reci

Cochlear implantation is anatomically more difficult in postoperative ears than in normal ears due to chronic otitis media. In cochlear implantation after canal wall down mastoidectomy, the electrodes must be protected. We report a case of cochlear implantation with partial mastoid obliteration using a retrofacial approach in an 81-year-old woman. Bilateral tympanoplasty was performed for bilateral chronic otitis media, but no improvement in hearing was observed. Hearing aids were used, but their effect was insufficient and cochlear implantation was needed. Since the patient was an older adult

HYPOTHESIS: Perimodiolar intracochlear electrodes with contacts facing towards the modiolus have limited current flow towards the outer wall of the cochlea and therefore, may reduce the occurrence of facial nerve stimulation (FN) in cochlear implant subjects.
BACKGROUND: Facial nerve stimulation is a well-known complication in cochlear implant treatment especially in the group of subjects with otosclerosis. The possible explanation of this side effect is a change of the electrical properties of the otosclerotic bone leading to leakage current and resulting in facial nerve stimulation.

Cochlear implant surgery can be highly complex in cases where ossification of the internal ear has taken place. In this case report, we report the use of new technological instruments to optimise the surgical process of implantation. These were the combined use of a surgical approach extended by a subtotal petrosectomy, a pre-operative radiological study with the OTOPLAN software for choosing the most suitable electrode array, and a residual functionality test of the auditory nerve using the ANTS test electrode array prior to inserting the cochlear implant electrode array. These were used to s

The measurement of the electrically evoked compound action potential (ECAP) in cochlear implant (CI) patients is widely used to provide evidence of a functioning electrode-nerve interface, to confirm proper location of the electrode array and to program the sound processor. In patients with an auditory brainstem implant (ABI), a likewise versatile measurement would be desirable. The ECAP measurement paradigm "Alternating Polarity" was utilized to record responses via the implanted ABI electrode array placed on the cochlear nucleus. Emphasizing on the different location of stimulation and recor

Unmyelinated fibers account for a remarkable fraction of the peripheral nervous system and their activity is linked to many autonomic and somatic functions. While electrical recording of such activity from human-sized peripheral nerves holds significant potential for neuroengineering applications, it has been shown only in acute settings via microneurography. This leaves unclear whether current implantable electrodes could achieve the same outcome. To address this matter, we simulated recordings from the human vagus nerve through a transverse intrafascicular multichannel electrode (TIME), a mi

This case reports on an 81-year-old man with implanted dual-chamber pacemaker for binodal disease who required ventricular lead revision due to loss of ventricular capture. Successful placement of a new lead via the subclavian vein despite of high degree stenosis of the medial part of the vein is described using a very thin bipolar ventricular pacing lead. Currently available pacing electrodes for this purpose are reviewed.

A 42-year-old male with advanced multiple sclerosis had severe left-sided trigeminal neuralgia in the maxillary and mandibular divisions that was extremely difficult to control with medications. Glycerol injection in the gasserian cystern provided only temporary results. Two electrodes were implanted epidurally at the C1-2 level, one in the midline and the other to the left of midline. Electrical stimulation produced complete relief from the painful paroxysms.

Neural prostheses that stimulate the neocortex have the potential to treat a wide range of neurological disorders. However, the efficacy of electrode-based implants remains limited, with persistent challenges that include an inability to create precise patterns of neural activity as well as difficulties in maintaining response consistency over time. These problems arise from fundamental limitations of electrodes as well as their susceptibility to implantation and have proven difficult to overcome. Magnetic stimulation can address many of these limitations, but coils small enough to be implante

OBJECTIVES/HYPOTHESIS: In pediatric patients with congenital malformations of the inner ear, anomalies within the anatomy may facilitate unintentional insertion of the cochlear implant electrode into the internal auditory canal. Revision procedures for removal and replacement of cochlear implant electrodes following internal auditory canal insertion are fraught with potential danger, including the theoretical risk of injury to vasculature within the internal auditory canal, repeat insertion within the internal auditory canal, and cerebrospinal fluid leak. The objective of this presentation is
