Epithelial Electrolyte Transport Physiology and the Gasotransmitter Hydrogen Sulfide.

Hydrogen sulfide (H2S) is a well-known environmental chemical threat with an unpleasant smell of rotten eggs. Aside from the established toxic effects of high-dose H2S, research over the past decade revealed that cells endogenously produce small amounts of H2S with physiological functions. H2S has therefore been classified as a "gasotransmitter." A major challenge for cells and tissues is the maintenance of low physiological concentrations of H2S in order to prevent potential toxicity. Epithelia of the respiratory and gastrointestinal tract are especially faced with this problem, since these b

[A case of a villous tumor of the rectum with electrolyte depletion syndrome].

We report a case of a villous tumor of the rectum with electrolyte depletion syndrome (EDS). A 76-year-old woman developed general fatigue, and on investigation, we found a large laterally spreading tumor of the rectum using bowel enema and colon fiber. Pathologically, the biopsy specimen was graded Group 4. We intended to perform a laparoscopy-assisted low anterior resection in the patient. However, the operation was interrupted by a short-run type of ventricular premature contraction (VPC) due to EDS-induced hypokalemia. After treating the hypokalemia, low anterior resection or laparotomy wa

Electrolyte and acid-base disorders in inflammatory bowel disease.

INFLAMMATORY BOWEL DISEASE (IBD) IS A CHRONIC INFLAMMATORY INTESTINAL DISORDER ENCOMPASSING TWO MAJOR ENTITIES: Crohn's disease and ulcerative colitis. Intestinal inflammatory processes reduce the absorption of sodium, chloride and calcium, while they increase potassium secretion. In addition, mild to severe metabolic alkalosis may occur in IBD patients, mainly depending on the severity of the disease and the part of the gastrointestinal tract being affected. The aim of this review is the presentation of the electrolyte and acid-base disturbances in IBD and how the activity state of the diseas

Electrolyte profile of pediatric patients with hypertrophic pyloric stenosis.

OBJECTIVES: Recent investigations have demonstrated that the classic hypochloremic, hypokalemic, metabolic alkalosis of hypertrophic pyloric stenosis (HPS) is not a common finding. Some have suggested a trend over time, but none has investigated factors contributing to laboratory derangement, such as duration of vomiting or patient age at presentation. We sought to determine the proportion of patients with HPS with normal and abnormal laboratory findings as a function of year of presentation, duration of vomiting, and patient age. METHODS: This is a retrospective chart review of 205 patients y

Dutch guideline for the management of electrolyte disorders--2012 revision.

Electrolyte disorders are common and often challenging in terms of differential diagnosis and appropriate treatment. To facilitate this, the first Dutch guideline was developed in 2005, which focused on hypernatraemia, hyponatraemia, hyperkalaemia, and hypokalaemia. This guideline was recently revised. Here, we summarise the key points of the revised guideline, including the major complications of each electrolyte disorder, differential diagnosis and recommended treatment. In addition to summarising the guideline, the aim of this review is also to provide a practical guide for the clinician an

Controlling the assembly of graphene oxide by an electrolyte-assisted approach.

In this work, we studied the effects of salts on the self-assembly of two-dimensional graphene oxide (GO) driven by the dissolution of a sub-microliter droplet. Two kinds of structures were obtained. One was a GO snowball with small salt crystals inserted between sheets, which formed with a low initial concentration of insoluble salt in the GO dispersion. The other was a hybrid nanostructure containing NaCl or KCl crystals on a GO snowball, which formed with a high initial salt concentration in the suspension. In addition, we report the novel nanodent-decorated GO snowballs formed by templatin

Mineral surface charge development in mixed electrolyte solutions.

Effects of competing counterions with different charge-to-size ratios on potential-determining ion (pdi; H(+), OH(-)) adsorption at mineral/water interfaces were resolved in mixtures of aqueous solutions of NaCl and NaClO4 solutions. These effects were monitored on two synthetic goethite (α-FeOOH) particle preparations with distinct charge uptake capacities arising from differences in surface roughness. Charge development at these mineral surfaces was chiefly explored by high precision potentiometric titrations at 25 °C. These measurements confirmed that the greater charge-to-size ratio chlori

Multiple electrolyte disorders in a neurosurgical patient: solving the rebus.

BACKGROUND: It is important to ensure an adequate sodium and volume balance in neurosurgical patients in order to avoid the worsening of brain injury.Indeed, hyponatremia and polyuria, that are frequent in this patient population, are potentially harmful, especially if not promptly recognized.Differential diagnosis is often challenging, including disorders, which, in view of similar clinical pictures, present very different pathophysiological bases, such as syndrome of inappropriate antidiuresis, cerebral/renal salt wasting syndrome and diabetes insipidus. CASE PRESENTATION: Here we present t

[Renin-angiotensin-aldosterone system inhibitors and electrolyte disturbances].

Recent numerous clinical studies have suggested that the renin-angiotensin-aldosterone system (RAAS) inhibitors have beneficial effects on hypertension, cardiovascular and chronic kidney diseases (CKD). Electrolyte disorder, especially hyperkalemia, is observed after the administration of RAAS inhibitors. This review focuses on the prevalence and severity of hyperkalemia with the use of RAAS inhibitors. Clinical evidences suggest that hypertensive patients with heart failure and CKD, and patients with combination treatment of RAAS inhibition are at higher risk of hyperkalemia, and serum potass

[Fluid and electrolyte disorders following surgery for brain tumors].

INTRODUCTION: Fluid and electrolyte disorders such as diabetes insipidus, salt wasting syndrome (SWS) and syndrome of inappropriate antidiuretic hormone secretion (SIADH) can appear in the immediate postoperative period after surgery for brain tumors. Early diagnosis and treatment are important to prevent the potential adverse effects of these disorders on the central nervous system (CNS). OBJECTIVES: To determine the incidence and characteristics of fluid and electrolyte disorders in the immediate postoperative period after surgery for CNS tumors in children treated in our hospital. The ageing process increases the risk of fluid and electrolyte imbalance.

Water is the largest single component of the body, making up between 45 and 75 per cent of the total body mass. A decrease in body fluid volume typically stimulates thirst which usually leads to increased fluid intake. However, large numbers of patients in A&E departments have a primary diagnosis of fluid and electrolyte imbalance or volume depletion. In the case of older people, this group has a diminished thirst response and decreased renal function making them at greater risk of developing dehydration.

Potassium, magnesium, and electrolyte imbalance and complications in disease management.

Electrolyte balance is a critical issue in managing comorbid conditions in both diseased and elderly patients. Patients with hypertension and diabetes need careful regulation of their calcium and magnesium levels, whereas in patients with congestive heart failure, sodium and potassium levels also are critical. Herein we report the outcome of a round table discussion at which issues of renal magnesium clearance, magnesium and arrhythmic risk, ion balance in heart failure, diabetes, ischemic stress, oxidative stress in the cardiomyopathy of magnesium deficiency, roles of magnesium and potassium

[Water-electrolyte and endocrine disorders after craniopharyngomectomy in a child].

The paper considers a type of three-phase course of diabetes insipidus with the development of the syndrome of inadequate secretion of antidiuretic hormone (SIADH) in a child after radical removal of endosupracellar craniopharyngoma. The parameters of cerebral blood flow, homeostatic fluctuations, the blood levels of thyroid hormones were compared with the time course of changes in the patient's clinical status. SIADH was accompanied by impaired consciousness, convulsion, developed common edematous syndrome, including interstitial edema of the lung, progressive suprarenal and thyroid insuffici

Acute effects of parainfluenza virus on epithelial electrolyte transport.

Parainfluenza viruses are important causes of respiratory disease in both children and adults. In particular, they are the major cause of the serious childhood illness croup (laryngotracheobronchitis). The infections produced by parainfluenza viruses are associated with the accumulation of ions and fluid in the respiratory tract. It is not known, however, whether this accumulation is because of a direct effect of the viruses on ion and fluid transport by the respiratory epithelium. Here we show that a model parainfluenza virus (the Sendai virus), in concentrations observed during respiratory i

Acid-base and electrolyte disorders after urinary diversion.

The use of bowel in urologic reconstructive procedures may result in numerous short and long-term complications, including well-described acid-base and electrolyte disorders. Many of these metabolic alterations are influenced by how solute absorption occurs across the particular bowel segment chosen for reconstruction. Solute absorption is impacted by: (1) the segment of bowel used, (2) the surface area of bowel used, (3) the time of retention of urine, (4) the concentration of solutes in the urine, (5) renal function, and (6) the pH and osmolality of the urine. These factors affect the type a

Acid-base and electrolyte abnormalities in patients with acute leukemia.

Disturbances of acid-base balance and electrolyte abnormalities are commonly seen in patients with acute leukemia. Our study aimed at illuminating the probable pathogenetic mechanisms responsible for these disturbances in patients with acute leukemia admitted to our hospital. We studied 66 patients (24 men and 44 women) aged between 17 and 87 years old on their admission and prior to any therapeutic intervention. Patients with diabetes mellitus, acute or chronic renal failure, hepatic failure, patients receiving drugs that influence acid-base status and electrolyte parameters during the last m
