Treatment of manifest latent nystagmus.

Eight patients with manifest latent nystagmus, as noted by ocular movement recordings, were examined for nystagmus reduction after surgical or optical treatment. Seven of the patients had strabismus. Five patients underwent strabismus surgery, had no tropia postoperatively, and the manifest latent nystagmus converted to latent nystagmus. Four of these five patients subsequently showed improvement in binocular visual acuity. Three patients received optical treatment; one had accommodative esotropia and, with appropriate spectacle correction, the manifest latent nystagmus was converted to latent

[A case of three-year-old boy with periodic apnea during waking and sleep, severe psychomotor retardation and hypotonia].

We presented a 3-year-old boy, a product of consanguineous parents, with periodic apnea during waking and sleep states, severe psychomotor retardation and hypotonia. According to polysomnographical recordings, he exhibited frequent central apneas which decreased in frequency and regularity in the stage REM. He showed abnormal background EEG, undifferentiated sleep stage and very short duration of stage REM. The initiation of breathing after apnea was often accompanied with generalized muscles contraction like a startle response. In the waking state the apnea induced generalized muscular hypoto

Abnormalities of horizontal gaze. Clinical, oculographic and magnetic resonance imaging findings. II. Gaze palsy and internuclear ophthalmoplegia.

The site of lesions responsible for horizontal gaze palsy and various types of internuclear ophthalmoplegia (INO) was established by identifying the common areas where the abnormal MRI signals from patients with a given ocular-motor disorder overlapped. Patients with unilateral gaze palsy had lesions in the paramedian area of the pons, including the abducens nucleus, the lateral part of the nucleus reticularis pontis caudalis and the nucleus reticularis pontis oralis. Patients with abducens nucleus lesions showed additional clinical signs of lateral rectus weakness. Lesions responsible for bil

[Retinal angiomatosis in association with Stargardt's disease. A case report].

We present what we believe to be the first reported association between retinal angiomatosis and Stardgardt's disease (Gass type 3). We have had the opportunity of following photographically a patient for the last eleven years. The earliest photos showed the macular problem whilst the latter shots showed the presence of a capillary angioma involving the temporal part of the optic disc and the adjacent retina of the left eye. We believe that this tumour arose in previously normal tissue. Examination and investigation of the patient revealed a peripapillary choroidal rupture at the site of the a

Optic atrophy in Wolfram syndrome.

The clinical and electrophysiologic findings in 11 cases of Wolfram syndrome are presented. These findings suggest that optic atrophy in Wolfram syndrome is not secondary to retinal pathology, but probably represents part of a generalized degeneration of neural structures. The relationship of diabetes mellitus to this process of neural degeneration remains unclear.

Electrophysiological findings in a family with congenital arteriohepatic dysplasia (Alagille syndrome).

Arteriohepatic dysplasia (Alagille syndrome) is a congenital cholestatic disease associated with ocular abnormalities. Three Japanese siblings, a 14-year-old girl, an 11-year-old boy, and a 9-year-old girl with this syndrome were studied. All three patients showed neonatal jaundice, hepatic dysfunction, characteristic facies, and psychomental retardation. The two sisters had cardiac murmurs. Ophthalmological examinations revealed that they had posterior embryotoxon, refractive error, retinochorodial degeneration, and electrophysiological abnormalities. The two sisters showed retinochoroidal de

Gyrate atrophy of the choroid and retina. Early findings.

Examination of two sisters ages 2 years 10 months and 6 years four months with gyrate atrophy of the choroid and retina provided an opportunity for detailed clinical investigation. Although the chorioretinal lesions were confined to the peripheral retina in the older case and were quite minimal in the younger case, there was electroretinographic evidence of marked involvement of the cone and rod systems. These cases offer an opportunity to assess an arginine restricted diet in preventing the progress of the disease.

Dorsal midbrain syndrome: clinical and oculographic findings.

We report clinical and oculographic findings in seven patients with the dorsal midbrain syndrome (Parinaud's syndrome). All presented with limited upward voluntary gaze and convergence nystagmus with attempted upward voluntary gaze. Quantitative analysis of vertical eye movements documented characteristic abnormalities of saccades with relative preservation of reflex eye movements (ie, vestibular, optokinetic, and visual-vestibular). Vertical saccade velocity was only slightly decreased in five patients with tumors, indicating that the vertical burst neurons in the mesencephalic reticular form

Compensatory eye movement and gaze fixation upon passive head- and body rotation and active head rotation.

To detect differences in compensatory eye movement and gaze fixation, normal subjects and patients with bilateral loss of labyrinthine functions were subjected to passive head-and-body rotation and performed active head rotation. In normal subjects, there was no difference between passive and active rotation with respect to compensatory eye movement in the dark and spatial gaze fixation; on the other hand, the ability to gaze fixedly on a head-fixed target was better during passive than active rotation. In our patients, the findings obtained upon passive rotation resembled those already report

Neuro-otology of the lateral medullary infarct syndrome.

Nine patients had a lateral medullary infarction. Examination during the acute phase of the lesion to determine oculomotor and oculovestibular functions showed that all patients had hypermetric saccades and increased gain of the vestibulo-ocular reflex (VOR) to the side of the lesion and hypometric saccades and decreased gain of the VOR to the side opposite the lesion. These abnormalities may be the consequence of the isolation of the vestibular nuclei from the flocculus, with loss of saccadic and vestibular gains by the cerebellum.

[Disseminated sclerosis: diagnostic value of neurophysiological investigations (author's transl)].

Demyelinisation lesions of disseminated sclerosis actually present within the white matter are much more numerous than clinical manifestations would suggest that the majority of these lesions are clinically latent. Ophtalmologists were the first to seek a functional deficit reflecting the demyelinisation which so often affects the optic pathways. In practice, the recording of evoked visual potentials has proved to be the most reliable method. The brain stem is another region where demyelinisation lesions are frequent and quite often latent. Electronystagmography offers a highly sensitive means

[Slowed saccades in various neurological disorders].

4 patients suffering from different neurological disorders are presented, exhibiting a striking slowing down of all saccades. According to their clinical features the problem of slow saccades of supranuclear origin is discussed, to elucidate the question whether these eye movements are saccadic at all. Electro-oculographic traces served to distinguish these particular eye movements, and a computer calculated duration, velocity and acceleration of these saccades, to identify their saccadic nature. 1 patient with Wernicke syndrome showed a remarkable improvement of saccadic velocity following th

Internuclear ophthalmoplegia. I. Saccades and dissociated nystagmus.

Saccades (horizontal and vertical) and dissociated nystagmus were quantitatively assessed in four patients with internuclear ophthalmoplegia. Two patients had bilateral medial longitudinal fasciculus (MLF) lesions associated with multiple sclerosis and two had unilateral lesions associated with brain stem vascular disease. Adducting saccades made on the side of an MLF lesion were slowed in each patient (P less than .01). At the same time, abducting saccades in the contralateral eye had normal velocity, but consistently overshot the target. After the overshoot, the eye returned to the target wi

Cognitive Load Prediction From Multimodal Physiological Signals Using Multiview Learning.

Predicting cognitive load is a crucial issue in the emerging field of human-computer interaction and holds significant practical value, particularly in flight scenarios. Although previous studies have realized efficient cognitive load classification, new research is still needed to adapt the current state-of-the-art multimodal fusion methods. Here, we proposed a feature selection framework based on multiview learning to address the challenges of information redundancy and reveal the common physiological mechanisms underlying cognitive load. Specifically, the multimodal signal features [electro

Large gaze shift generation while standing: the role of the vestibular system.

The functional significance of vestibular information for the generation of gaze shifts is controversial and less well established than the vestibular contribution to gaze stability. In this study, we asked seven bilaterally avestibular patients to execute voluntary, whole body pivot turns to visual targets up to 180° while standing. In these conditions, not only are the demands imposed on gaze transfer mechanisms more challenging, but also neck proprioceptive input represents an inadequate source of head-in-space motion information. Patients' body segment was slower and jerky. In the absence

Electrophysiological correlates of reading in children with attention deficit hyperactivity disorder.

AIMS: To investigate whether or not the deficits in executive functions in the attention deficit hyperactivity disorder (ADHD) affect reading comprehension and identify a potential biological marker of this neuropsychological endophenotype through event-related potentials (ERP). The phenotypic association between reading comprehension and the specific functions of inhibition and working memory is studied. SUBJECTS AND METHODS: The sample consisted of 52 children with ADHD (8-13 years) divided in two groups according to the presence (TDAH-; n = 27; percentile < 30) or the absence (TDAH+; n = 2

Diagnostic value of sleep stage dissociation as visualized on a 2-dimensional sleep state space in human narcolepsy.

BACKGROUND: Type 1 narcolepsy (NT1) is characterized by symptoms believed to represent Rapid Eye Movement (REM) sleep stage dissociations, occurrences where features of wake and REM sleep are intermingled, resulting in a mixed state. We hypothesized that sleep stage dissociations can be objectively detected through the analysis of nocturnal Polysomnography (PSG) data, and that those affecting REM sleep can be used as a diagnostic feature for narcolepsy. NEW METHOD: A Linear Discriminant Analysis (LDA) model using 38 features extracted from EOG, EMG and EEG was used in control subjects to sele
