CT-scan is the preferred investigation in skull trauma and in any patient with suspected meningeal haemorrhage. Any suspicion of compression of the medulla or of a cauda equina requires emergency investigation by MRI. This investigation is also standard when cerebral thrombophlebitis or meningeal encephalitis is suspected. A future increase in available MRI equipment is desirable in order to enlarge the indications of such imaging, particularly in the case of recent cerebral vascular accident.

Alteration of mental status secondary to medical illness may occasionally be incorrectly attributed to a psychiatric problem. The cases of 64 patients with unrecognized medical emergencies inappropriately admitted to psychiatric units from emergency departments were reviewed to determine the cause of the misdiagnoses. OBJECTIVE: Our goal was to present a case series of pregnancy-associated malignant brain tumors.
STUDY DESIGN: A review was conducted from 1978-1998 at 5 hospitals.
RESULTS: Ten women were diagnosed with a malignant brain tumor during pregnancy (n = 8) or post partum (n = 2). Patients diagnosed antenatally exhibited severe symptoms, manifest between 27 and 32 weeks' gestation. Although it remains controversial as to whether diabetics have an overall increased incidence of infection as compared to nondiabetics, several potentially life-threatening infections do appear to be uniquely associated with diabetes. These infections generally occur in older diabetics with less than optimal glucose control. For each entity, selected symptoms and signs may suggest the diagnosis but confirmation of via tissue biopsy with culture and histopathology or radiography is usually necessary. Management typically require both antimicrobial treatment and surgery.

Although the majority of psychiatric patients present to the ER in chronic crisis, the clinician should also be familiar with the assessment and management of acute emergencies. Because emergency patients reflect the entire spectrum of psychiatric psychopathology, the primary care physician should consult with a mental health clinician whenever possible. Following a medical model, a thorough diagnostic evaluation should be completed and specific treatment initiated. Most often, the treatment consists of some combination of psychological, pharmacologic, and physical approaches.
