Ludwig angina remains a potentially lethal disease, rapidly spreading bilateral cellulitis of the submental, sublingual, and submandibular spaces, which bears the threat for rapid airway obstruction. Since the introduction of antibiotics in 1940s, the mortality was reduced significantly. This resulted in the rare occurrence of the disease, leaving many physicians with limited experience of Ludwig angina. Although the occurrence of Ludwig angina in adults is rare, its presence in the pediatric edentulous population is even more uncommon. Because the unfamiliarity with this disease is now increa

BACKGROUND: Drug-related harm is a major global public health concern, with opioids implicated in over 70% of drug-related deaths. Scotland reports the highest drug-related mortality rate in Europe, with remote and rural communities facing additional barriers to accessing harm reduction and treatment services. Assertive outreach is increasingly recognised as a key strategy to engage individuals at risk who are disconnected from traditional services. This study evaluates the use of a Trigger Checklist to identify and outreach individuals within 48 hours of referral in a rural Emergency Departme

BACKGROUND: Sepsis is characterised as life-threatening organ dysfunction due to a dysregulated host response to infection. It carries high mortality and is a major public health issue globally. Among adults presenting to the emergency department with features of suspected sepsis, rapid and accurate differentiation of those at high risk for prompt delivery of key therapies and escalation of care may improve outcomes.
OBJECTIVES: To conduct a comprehensive evidence synthesis assessing the clinical utility of established and novel biomarkers - used individually or in combination - for predictin

BACKGROUND: Blood cultures (BCs) are used in emergency departments (EDs) to investigate and manage suspected sepsis; however, their inappropriate use contributes to low-value care, providing limited clinical benefit while causing avoidable harm. Despite recommendations for more selective testing, low-value blood culture (LVBC) use persists, and there remains uncertainty on the definition, drivers, and impact of LVBCs.
OBJECTIVES: To map the existing literature on LVBCs in adult ED patients, including definitions of LVBCs, associated clinical contexts, contributing factors, consequences and re

Angioedema is a common, potentially life-threatening emergency characterized by swelling of subcutaneous or submucosal tissues. While allergic reactions are common, a substantial proportion of ED angioedema presentations, particularly those without urticaria, involve non-allergic, bradykinin-mediated mechanisms. Differentiating between these pathways is critical, as standard anaphylaxis treatments like epinephrine are often ineffective for non-allergic variants. A key clinical indicator is the presence or absence of urticaria; isolated angioedema is rarely an allergy and suggests a bradykinin-

PURPOSE: The landscape of the medico-legal risk in emergency medicine in Canada is unknown. The objective of this study was to describe and analyze factors associated with medico-legal risk in cases in the emergency department (ED).
METHODS: We conducted a descriptive analysis of closed medico-legal cases (hospital complaints, regulatory authority (i.e., College) complaints, and civil-legal actions) involving physicians practicing primarily emergency medicine in Canadian EDs. We used data from closed cases involving the Canadian Medical Protective Association from January 2018 until December

Hydrocephalus is a frequently encountered neurological disorder of cerebrospinal fluid (CSF) physiology or dynamics, resulting in the abnormal enlargement of CSF spaces and often presenting with signs and symptoms of raised intracranial pressure. The etiology of hydrocephalus is numerous, regardless of the model used to understand its pathophysiology. Differentiating ventricular enlargement due to hydrocephalus or parenchymal atrophy can be difficult in pediatric patients, but certain imaging signs could help differentiate the two entities. The management of hydrocephalus aims to restore norma

Acute mesenteric ischemia is a rare but potentially catastrophic condition, and survival is highly dependent on timely diagnosis in the emergency department. Symptoms can be both variable and subtle. Laboratory studies may be misleading and may provide either false reassurance or misdirection. This review includes evidence-based recommendations on the recognition of the 4 separate types of acute mesenteric ischemia, current guidelines on the diagnostic approach, and essential resuscitative steps that should be initiated in the emergency department.

BACKGROUND: Emergency departments (EDs) are complex and fast-paced clinical settings where a diagnosis is made in a time-, information-, and resource-constrained context. Thus, it is predisposed to suboptimal diagnostic outcomes, leading to errors and subsequent patient harm. Arriving at a timely and accurate diagnosis is an activity that occurs after an effective collaboration between the patient or caregiver and the clinical team within the ED. Interventions such as novel sociotechnical solutions are needed to mitigate errors and risks.
OBJECTIVE: This study aims to identify challenges that

Patients with maxillofacial trauma require careful evaluation due to the anatomical proximity of the maxillofacial region to the head and neck. Facial trauma can lead to life-threatening airway compromise or hemorrhage, or permanent facial deformity. Although the Advanced Trauma Life Support guidelines provide a framework for the management of trauma patients, they do not provide a detailed reference for many subtle or complex facial injuries. In addition to an overview of maxillofacial trauma pathophysiology, associated injuries, and physical examination, this review will also discuss relevan

We report a case of a patient with dermal piercings of the hand that were used to anchor body jewelry. The piercings had become infected and were causing the patient pain. She requested that they be removed in the emergency department (ED). Both piercings were successfully removed by anesthetizing the area and creating a small linear incision at the base of the stud before gentle removal of the dermal anchor with a hemostat. This report outlines a simple way to remove the piercings while minimizing tissue damage.

INTRODUCTION: Multiple myeloma (MM) and its complications carry a high rate of morbidity and mortality.
OBJECTIVE: This review evaluates MM and its complications, including presentation, diagnosis, and management in the emergency department (ED) based on current evidence.
DISCUSSION: MM is the second most common hematologic cancer and associated with monoclonal plasma cell proliferation. The presentation of MM varies, ranging from few symptoms to end organ injury and failure. The most common presenting findings include anemia, bone pain, renal injury, fatigue, hypercalcemia, and weight loss.

Sickle cell disease is a chronic hematologic disease that affects over 100,000 people in the United States. Many of these patients will present to the emergency department seeking treatment for an acute complication. Vaso-occlusive crisis, the most common recurring complication, can be difficult to manage because of the stigma patients face surrounding management of their pain. Patients with sickle cell disease presenting with conditions such as pain, infection, respiratory distress, stroke, or priapism must be given special consideration, as management can differ from that of the general popu

INTRODUCTION: Botulism is a serious condition that carries with it a high rate of morbidity and mortality.
OBJECTIVE: This review highlights the pearls and pitfalls of botulism, including presentation, diagnosis, and management in the emergency department (ED) based on current evidence.
DISCUSSION: Botulism is a neuromuscular disorder caused by toxin production of clostridium species bacteria and is a challenging diagnosis that mimics several other conditions. Children account for the majority of patients, with a foodborne source most common, followed by wound sources, typically from intrave
