Diagnosis of myocarditis: Current state and future perspectives.

Myocarditis, i.e. inflammation of the myocardium, is one of the leading causes of sudden cardiac death (SCD) and dilated cardiomyopathy (DCM) in young adults, and is an important cause of symptoms such as chest pain, dyspnea and palpitations. The pathophysiological process of disease progression leading to DCM involves an ongoing inflammation as a result of a viral-induced auto-immune response or a persisting viral infection. It is therefore crucial to detect the disease early in its course and prevent persisting inflammation that may lead to DCM and end-stage heart failure. Because of the hig

Varicella zoster myocarditis progressing to cardiomyopathy and cardiac transplantation.

The case of a 12 year old schoolgirl with heart failure due to varicella myocarditis is reported. Heart failure and cardiogenic shock were evident 21 days after the appearance of the rash, and cardiac transplantation was performed two weeks later. Myocarditis is a serious complication of varicella zoster infection and heart failure may be fulminant. Endomyocardial biopsy changes consistent with myocarditis were documented six days after the start of heart failure. The histological changes, however, developed into those of idiopathic dilated cardiomyopathy (with anisonucleosis and fibre width v

[A case of cardiomyopathy due to allergic bronchopulmonary aspergillosis].

We report a rare occurrence of cardiomyopathy associated with allergic bronchopulmonary aspergillosis (ABPA). A 49-year-old man with a histo … istological diagnosis of endomyocardial fibrosis were made by cardiac catheterization and biopsy of endocardium. Retrospectively, cardiomegaly had gradually increased during the past several months while peripheral blood eosinophilia had continued. All these data strongly suggested that eosinophilia due to ABPA might cause severe cardiac damage.

[Endocarditis parietalis fibroplastica Löffler].

Report on an endocarditis parietalis fibroplastica Löffler in a 54-year-old male. Discussion of the localisation, pathologico-morphologic findings as well as of the differential diagnosis and the therapy. Particular reference to the ten-year course of the disease, which possibly is the success of the treatment.

Supravalvular mitral stenosis associated with tetralogy of Fallot.

The association of tetralogy of Fallot with supravalvular mitral stenosis is a rare anomaly that has been reported only once previously. The difficulty of preoperative diagnosis is emphasized. Although left-sided obstructive lesions in association with tetralogy of Fallot are rare, their recognition is imperative since these are surgically correctable anomalies and potentially lethal, as proved in this case and the one previously reported.

Aberrant Phase Separation of Endothelial MAML1 Causes Congenital Heart Disease by Suppressing Notch Activity.

BACKGROUND: Congenital heart disease (CHD), the most common birth defect and a leading cause of infant mortality, is frequently linked to dysregulated Notch signaling. However, the role of the Notch transcriptional coactivator Mastermind-like 1 (MAML1) in CHD pathogenesis and the underlying molecular mechanism remain unclear. METHODS: We investigated the role of MAML1 in CHD by focusing on a patient-derived Q401K mutation with a knock-in mouse model and an endocardium-specific Maml1 knockout mouse model, complemented by CRISPR-edited human heart organoids. Cardiac phenotypes were assessed by …

Frequency and Size of In Situ Thrombus Within Patent Foramen Ovale.

BACKGROUND: High-resolution optical coherence tomography can detect in situ thrombi within patent foramen ovale (PFO), which can become a da … matic (n=25) groups. Optical coherence tomography was used to evaluate in situ thrombi and abnormal endocardium within PFO. Univariable analysis and a logistic model were used to evaluate the association between stroke and in situ thrombus; age, sex, body mass index, and antithrombotic therapy were included as covariates. RESULTS: Antithrombotic therapy was …

Electrophysiology and Arrhythmogenesis in the Human Right Ventricular Outflow Tract.

BACKGROUND: Right ventricular outflow tract (RVOT) is a common source of ventricular tachycardia, which often requires ablation. However, th … that arrhythmia wave fronts and phase singularities (rotors) were relatively more organized in the endocardium than in the epicardium (P=0.006). Moreover, there was a weak but positive spatiotemporal autocorrelation between epicardial and endocardial arrhythmic wave fronts and rotors. Transcriptome analysis (n=10 hearts) suggests a trend that MAPK (mitogen-a …

[Infective fungal endocarditis in patients with no previous cardiac disease].

Infective endocarditis is a serious and uncommon condition affecting the endocardium. Less than 10% of these cases are of fungal origin. A growing number of individuals are at high risk, due to insertion of central venous catheters, total parenteral nutrition and prolonged exposure to broad-spectrum antibiotics, even without previous heart diseases. We retrospectively analysed the records of six children with Candida endocarditis, reviewing the comorbidities, clinical outcome, and treatment. The antifungal agents used were amphotericin B, 5-fluorocytosine and fluconazole. Patients underwent sur…

Multiple Imaging Modalities in the Diagnosis of Infective Endocarditis.

Infective endocarditis (IE) is an inflammatory disease caused by the infection of the endocardium or heart valves by pathogenic microorganisms. It is characterized by diagnostic challenges, difficult treatment, and high mortality. Multimodal imaging techniques, including echocardiography, computed tomography (CT), magnetic resonance imaging (MRI), and nuclear medicine imaging, play a crucial role in the diagnosis of IE. Echocardiography is the first-line imaging modality for suspected IE. Cardiac CT, with its excellent spatial resolution and three-dimensional (3D) reconstruction capabilities, …

Percutaneous Interventricular Septal Access Guided by Subcostal Echocardiography and Fluoroscopy for Ventricular Tachycardia Ablation in a Patient with Aortic and Mitral Mechanical Valves.

Mechanical prosthetic aortic and mitral valves preclude either a retrograde aortic or transseptal approach to the left ventricular (LV) endocardium. Several operators have reported on the application of nonconventional techniques for ventricular tachycardia (VT) ablation including transventricular septal puncture, epicardial approach, transmechanical valve approach, transcoronary venous approach, and transapical approach. Incorporating transventricular access to the LV under intracardiac echocardiography (ICE) guidance has been previously attempted in VT ablation procedures in patients with bo…

Traumatic Endocardial Rupture Causing Bilateral Forelimb Arterial Thromboembolism in a Dog.

OBJECTIVE: To describe a case of traumatic endocardial rupture, without obvious clinical signs of cardiac injury, resulting in acute thrombosis of the forelimbs in a dog. CASE SUMMARY: A young adult male German Shepherd Dog presented with vehicular polytrauma. The dog was diagnosed with hypovolemic shock and traumatic pneumothorax and was stabilized, having displayed no clinical indicators of cardiac dysfunction. For 10 days, the dog was treated with no complications in the hospital for wounds but presented again shortly after discharge with bilateral forelimb paraparesis determined likely to

Endoepicardial transmural lesion formation utilizing multipolar radiofrequency ablation.

BACKGROUND: Conventional unipolar radiofrequency (RF) catheter ablation is limited by maximal lesion depths that fail to eliminate ventricular tachycardias with deep midmyocardial critical components. The use of multipolar mapping catheters as an active part of the ablation circuit may provide a solution to these limitations. OBJECTIVE: This study aimed to evaluate a novel endoepicardial multipolar RF catheter ablation technique for creating transmural lesions in an in vivo porcine model. METHODS: Two catheter configurations were evaluated: (1) standard bipolar configuration with a 3.5-mm ir
