[Unusual electrocochleography cases].

The electrocochleographic findings of 3 unusual cases from about 350 performed on patients with various hearing loss are presented; they demonstrate the value of electrocochleography as a clinical tool in otological investigation. The first case was an electrocochleographically detected acute impairment of the first neuron, the second a hereditary hearing loss with endolymphatic hydrops, and the last shows the effect of an endolymphatic shunt on a hydropic hearing loss.

Furosemide loading vestibular evoked myogenic potential for unilateral Ménière's disease.

OBJECTIVE: To investigate whether endolymphatic hydrops in Ménière's Disease could be diagnosed by comparing vestibular evoked myogenic potential (VEMP) before and after furosemide administration (F-VEMP). STUDY DESIGN: Retrospective study. SETTING: University hospital. INTERVENTION: Furosemide administration. PATIENTS: Twenty-five affected ears of patients with unilateral Ménière's Disease were studied. The patients had received confirmed diagnoses Ménière's Disease; thus, the presence of endolymphatic hydrops was suggested. For the control, 22 ears of 11 normal healthy volunteers who had…

Menière and Friends: Imaging and Classification of Hydropic Ear Disease.

BACKGROUND: Over 75 years ago, endolymphatic hydrops was discovered as the pathologic correlate of Menière's disease. However, this pathologic finding could be ascertained only in postmortem histologic studies. Due to this diagnostic dilemma and the variable manifestation of the various audiovestibular symptoms, diagnostic classification systems based on clinical findings have been used hitherto. METHODS: A review of the literature of magnetic resonance (MR) imaging of hydropic ear disease. RESULTS: Recent developments of high resolution MR imaging of the inner ear have now enabled us to vis…

Urea and hearing in patients with menière's disease.

Transient hearing improvement was recorded in around 60% of patients with Menière's disease 2 hours after the ingestion of 20 g of urea perorally. Urea slightly increased the serum osmolality, which is thought to cause a transient reduction of the endolymphatic hydrops. Urea has no inconvenient side effects and is recommended primarily as a test substance for the verification of endolymphatic hydrops, in conformity with glycerol.

Treatment of hydropic patients by immunoglobulin with methyl B12.

Recently several investigations have been reported suggesting that the cause of endolymphatic hydrops might be an immunologic disorder of the endolymphatic space, including the endolymphatic sac. As the first choice in a conservative treatment by medication, the authors have used a combination of prednisolone and furosemide per os, which is rather safe and effective for hydrops patients in a subacute stage. However, some patients do not respond to this treatment or gradually become immune to this medication. With these patients, we have tried an intravenous administration of immunoglobulin G w…

Cochlear Neural Degeneration Is Widespread in Ménière's Disease: A Temporal Bone Study.

PURPOSE: Ménière's disease presents with endolymphatic hydrops and disproportionately poor word-recognition compared to other forms of sensorineural hearing loss, yet the underlying patterns of cochlear degeneration are not well defined. This study aimed to characterize cochlear pathology in Ménières, compare it to age-matched controls and clinically unaffected contralateral ears, and evaluate how hydrops severity and clinical endotypes relate to tissue degeneration. METHODS: We analyzed 97 human cochleas, including 43 Ménière's ears, 10 contralateral (clinically unaffected) ears, and 44 age-…

A Micro-CT and Synchrotron Imaging Study of the Human Endolymphatic Duct with Special Reference to Endolymph Outflow and Meniere's Disease.

Meniere's disease remains enigmatic, and has no treatment with sufficient evidence. The characteristic histopathological finding is endolymphatic hydrops, suggesting either an overproduction or decreased reabsorption of endolymph in the human inner ear. This study presents the first analysis of the vascular plexus around the human endolymphatic duct using micro computed tomography and coherent synchrotron radiation with phase contrast imaging. Using a software program, data were processed by volume-rendering with scalar opacity mapping to create transparent three-dimensional reconstructions. A…

Ultrastructural changes in the hydropic cochlea of the guinea-pig.

Transmission electron microscopy of the cochlear organ of Corti in experimental endolymphatic hydrops revealed two principal features. Starting 1 month after induction of hydrops, osmiophilic inclusions thought to represent lipofuscin accumulation were frequently observed in the subcuticular cytoplasm of the outer hair cells along the length of the cochlea. Starting 3 months after induction of hydrops the efferent terminals on the outer hair cells appeared to be vacuolated. These data suggest that oxidative insult is likely to contribute to the pathology associated with endolymphatic hydrops a…

[Towards a cellular pathophysiology of Meniere's disease].

BACKGROUND: A modest increase in the hydrostatic pressure within the endolymphatic space may induce an endolymphatic hydrops which in turn is typically associated with Meniere's disease. However, there is no convincing pathophysiological model that might explain, on a cellular level, how a pressure increases may cause disturbed functions of vestibular hair cells and vertigo attacks. METHODS: So far, models involve osmotic mechanisms leading to electrolyte imbalances in the endolymphatic space. Alternatively, impaired longitudinal flow in the endolymphatic space is considered important. RESUL…

Menière's symptoms resulting from bilateral otosclerotic occlusion of the endolymphatic duct: an analysis of a causal relationship between otosclerosis and Menière's disease.

An association between otosclerosis and Menière's disease has been proposed on both a clinical and temporal bone histopathologic basis for well over three-quarters of a century. Controversy persists over a causal relationship between these two entities, however, and the underlying pathophysiologic mechanisms relating capsular otosclerosis with Menière's disease remain speculative. The first case of total, bilateral endolymphatic duct occlusion resulting from extensive capsular otosclerosis is presented in a woman manifesting Menière's symptoms. The severe endolymphatic hydrops resulting from o…

Meniere's disease: the incidence of hydrops in the contralateral asymptomatic ear.

OBJECTIVE: This study analyzes the incidence of endolymphatic hydrops in the asymptomatic contralateral ear of patients with classic Meniere's disease. STUDY DESIGN: A retrospective study of 3000 subjects who underwent electrocochleography (ECOG) from 1988 to 1998. METHODS: The presence of endolymphatic hydrops was determined by use of ECOG recordings, which were made through a transtympanic recording needle situated in the round window niche. Analysis was made of the 1-kHz toneburst summation potential (SP), and comparison was made between asymptomatic contralateral "Meniere's ears" (n = 14…

Adenoma of the endolymphatic sac.

A woman, who presented with symptoms of unilateral Ménière's disease, was found to have an adenoma of the endolymphatic sac, when an endolymphatic sac decompression was performed. Such a finding has not previously been reported. The possible role of the tumor in causing endolymphatic hydrops is discussed.

Histopathology after endolymphatic sac surgery for Ménière's syndrome.

BACKGROUND: The putative goal of sac surgery in Ménière's syndrome is to promote the flow of endolymph from the labyrinth to the endolymphat … placed within the lumen of the sac in 2 cases; both cases failed to experience relief from vertigo. Endolymphatic hydrops was present in all 15 cases. CONCLUSION: Endolymphatic sac surgery does not relieve hydrops in patients with Ménière's syndrome. Yet, sac surgery relieves vertigo in some patients, but the mechanism of such symptomatic relief remains unknown.

Clinical manifestations of hydropic ear disease (Menière's).

INTRODUCTION: Hydropic ear disease, initially described by and named after Prosper Menière, is one of the most frequent vertigo disorders and one of the most frequent inner ear disorders. It is the syndrome of endolymphatic hydrops which until 2007 could be diagnostically confirmed only by post-mortem histology. In the past, various attempts to formulate clinical diagnostic criteria have been undertaken but were hampered by the inability to ascertain the diagnosis in living patients. With the milestone achievement of endolymphatic hydrops imaging, today the pathology can be ascertained. In thi…

Low-salt diet increases mRNA expression of aldosterone-regulated transporters in the intermediate portion of the endolymphatic sac.

The endolymphatic sac is a small sac-shaped organ at the end of the membranous labyrinth of the inner ear. The endolymphatic sac absorbs the endolymph, in which the ion balance is crucial for inner ear homeostasis. Of the three sections of the endolymphatic sac, the intermediate portion is the center of endolymph absorption, particularly sodium transport, and is thought to be regulated by aldosterone. Disorders of the endolymphatic sac may cause an excess of endolymph (endolymphatic hydrops), a histological observation in Meniere's disease. A low-salt diet is an effective treatment for Meniere…

Endolymphatic duct and sac in patients with Meniere's disease. A temporal bone histopathological study.

The endolymphatic ducts and sacs of 25 temporal bones with idiopathic endolymphatic hydrops from individuals with Meniere's disease were studied and compared with the same number of control bones without endolymphatic hydrops from individuals with no premortem history of otologic disease. The control bones were selected so that the sizes of their vestibular aqueducts matched those temporal bones from individuals with Meniere's disease. The endolymphatic ducts and sacs of all bones were studied by the medial view graphic reconstruction method and/or histological observation under a light micros…

Evolution of understanding and clinical practice of Ménière's disease.

Since Prosper Ménière's original description more than 160 years ago, concepts of Ménière's disease (MD) pathophysiology and treatment have evolved through distinct phases. For much of the twentieth century, the field was dominated by the hydrodynamic paradigm, which attributed symptoms to pressure-driven endolymphatic hydrops and episodic membrane rupture. Over the past three decades, however, accumulating pathological, physiological, genetic, and imaging evidence has undermined this model. This review traces the transition from the classic hydrodynamic era to a modern framework that conceptu…
