Fabry disease, an under-recognized multisystemic disorder: expert recommendations for diagnosis, management, and enzyme replacement therapy.

Fabry disease (alpha-galactosidase A deficiency) is an X-linked recessive lysosomal storage disorder. Although the disease presents in childhood and culminates in cardiac, cerebrovascular, and end-stage renal disease, diagnosis is often delayed or missed. This paper reviews the key signs and symptoms of Fabry disease and provides expert recommendations for diagnosis, follow-up, medical management, and the use of enzyme replacement therapy. Recommendations are based on reviews of the literature on Fabry disease, results of recent clinical trials, and expertise of the authors, all of whom have e…

Biochemical and functional abnormalities in lymphocytes from an adenosine deaminase-deficient patient during enzyme replacement therapy.

Biochemical and immunological properties of lymphocytes were measured repetitively over a period of 40 mo during enzyme replacement by transfusion in a child with adenosine deaminase (ADA) deficiency and severe combined immunodeficiency disease. Catalytically defective ADA protein is present in the child's cells. ADA activity in his lymphocytes is 7 nmol/min per 10(8) cells with 51 ng of ADA protein/10(8) cells by radioimmunoassay. ADA activities in normal cord and adult lymphocytes average 193 and 92 nmol/min per 10(8) cells, respectively, with 429 and 223 ng of ADA protein/10(8) cells. Deoxy

Progression of sarcopenia among patients with pancreatic ductal adenocarcinoma receiving neoadjuvant therapy and the impact of pancreatic enzyme replacement therapy.

BACKGROUND: Neoadjuvant therapy (NAT) is increasingly used among patients with pancreatic ductal adenocarcinoma (PDAC). Patients with PDAC commonly present with sarcopenia, which may progress during the period of NAT and subsequently impact treatment outcomes. However, data assessing trends in sarcopenia are currently lacking. As such, this audit sought to quantify changes in body composition during NAT, and to assess whether these correlated with patient outcomes or pancreatic enzyme replacement therapy (PERT) usage. METHODS: Computed tomography (CT) scans performed pre- and post-NAT were an…

High-Dose Ambroxol Therapy in Type 1 Gaucher Disease Focusing on Patients with Poor Response to Enzyme Replacement Therapy or Substrate Reduction Therapy.

Ambroxol hydrochloride (ABX), an oral mucolytic drug available over the counter for many years, acts as a pharmacological chaperone for mutant glucocerebrosidase, albeit at higher doses. Proof-of-concept reports have been published over the past decade on all three types of Gaucher disease (GD). Here, we assess the safety and efficacy of 12 months of 600 mg ambroxol per day in three groups of Type 1 GD patients with a suboptimal response to enzyme replacement therapy (ERT) or substrate reduction therapy (SRT), defined as platelet count < 100 × 103/L, lumbar spine bone density T-score < -2.0, a…

Response to Comments on "Increasing Enzyme Mannose-6-Phosphate Levels but Not Miglustat Coadministration Enhances the Efficacy of Enzyme Replacement Therapy in Pompe Mice".

We thank Dr. Weimer and her colleagues for their comments related to our recent work (Anding et al., 2023) and are grateful for the opportunity to further discuss the importance of efficient lysosomal targeting of enzyme-replacement therapies (ERT) for the treatment of Pompe disease. Patients with Pompe disease have mutations in the gene that encodes for acid α glucosidase (GAA), a lysosomal enzyme necessary for the breakdown of glycogen. The first-generation ERT, alglucosidase alfa, provides a lifesaving therapy for the severe form of the disease (infantile onset Pompe disease) and improves o

Gastrointestinal manifestations and enzyme replacement therapy in late-onset Pompe disease: insights from a cross-sectional analysis.

BACKGROUND: Late-onset Pompe disease (LOPD) is an autosomal-recessive disorder caused by acid α-glucosidase (GAA) deficiency, typically presenting after the first year of life and resulting in systemic glycogen accumulation. Beyond motor and respiratory manifestations, gastrointestinal (GI) symptoms have been increasingly reported. Although enzyme replacement therapy (ERT) improves skeletal and respiratory function, its impact on GI symptoms remains unclear. METHODS: There were 124 LOPD patients recruited to complete the Gastrointestinal Symptom Rating Scale (GSRS), together with questionnaire…

Exploring the potential of bacterial-derived EVs for targeted enzyme replacement therapy: mechanisms, applications, and future directions.

Extracellular vesicles (EVs) are membrane-bound vesicles produced by cells which promote intercellular communication by delivering different contents such as DNA, RNA, and proteins. These vesicles, nano-sized and released into the extracellular space, are present everywhere under both normal and pathological conditions. Probiotic-derived EVs can serve as nanocarriers for therapeutic cargo, particularly in enzyme replacement therapy (ERT). Traditional ERT for lysosomal storage diseases (LSDs) faces significant challenges, including the inability of enzymes to cross the blood-brain barrier (BBB)…

Amelioration of Inflammation and Metabolic Blockage in GALC Deficient Mice After Enzyme Replacement Therapy via Extracellular Vesicles.

INTRODUCTION: Krabbe disease (KD) is a fatal lysosomal storage disorder caused by a deficiency in the enzyme galactosylceramidase (GALC), leading to toxic accumulation of psychosine. This results in widespread demyelination, inflammation, and neuronal damage. Early intervention is critical to mitigate disease progression and limit neurological injury. METHODS: To assess the therapeutic potential of early enzyme replacement therapy (ERT), HeLa cells were genetically engineered to overexpress GALC, and extracellular vesicles (EVs) containing GALC were isolated. A single intrathecal injection of…

Engineering memory T cells as a platform for long-term enzyme replacement therapy in lysosomal storage disorders.

Enzymopathy disorders are the result of missing or defective enzymes. Among these enzymopathies, mucopolysaccharidosis type I is a rare gene … tissues and can persist for years, making them an attractive approach for cellular-based, systemic enzyme replacement therapy. Here, we tested genetically engineered, IDUA-expressing Tm cells as a cellular therapy in an immunodeficient mouse model of MPS I. Our results demonstrate that a single dose of engineered Tm cells leads to detectable IDUA enzyme levels in the blood …

Health care costs of home care enzyme replacement therapy for patients with lysosomal storage diseases in Germany.

BACKGROUND: Lysosomal storage diseases (LSDs) can be treated with intravenous enzyme replacement therapy (ERT). ERT is being administered either in specialized clinics or in the home care setting. Studies indicate that home-based ERT can be considered safe and positively effects patient reported outcomes. Due to the higher level of individual care required by nurses when administering ERT at home, questions arise whether this will result in additional costs for health care systems. Since cost data to the German health care system are currently unavailable, this study aims to assess home care E…
